Clinical outcome of a nonsurgical and surgical treatment protocol in different types of peri-implantitis: a case series.

OBJECTIVE: The replacement of missing teeth with dental implants has been standard practice in dentistry for many years. The success of dental implants depends on many factors, among which the diagnosis, clinical severity, and treatment of peri-implant diseases play a key role. In this prospective case series, the influence of cumulative treatment modalities on peri-implantitis with and without pus formation on clinical outcome was assessed. METHOD AND MATERIALS: During 2010, 28 patients were referred for peri-implantitis treatment. They presented two different types of peri-implant diseases:

Efficacy and safety of adjunctive local moxifloxacin delivery in the treatment of periodontitis.

BACKGROUND: Moxifloxacin exerts excellent antibacterial activity against most putative periodontal pathogens and has been shown to kill bacteria in biofilm and host cells. METHODS: Patients with chronic periodontitis were randomly assigned to receive a single subgingival application of a 0.125%, 0.4%, or 1.25% moxifloxacin gel or placebo gel immediately after full-mouth scaling and root planing (SRP). Clinical efficacy measurements were assessed in sites with baseline probing depth (PD) of ≥5.4 mm at 6 weeks and 3 months and any adverse events were determined. In addition, putative periodonta

Living cellular construct for increasing the width of keratinized gingiva: results from a randomized, within-patient, controlled trial.

BACKGROUND: The standard of care for increasing keratinized gingiva adjacent to teeth that do not require root coverage is the free gingival graft (FGG). A pilot study indicated that the use of a living cellular construct (LCC) could be effective in this clinical scenario. METHODS: A pivotal, multicenter, randomized, within-patient, controlled, open-label trial was conducted (N = 96 patients). After removing the mucosa and keratinized gingiva from the test site, either an LCC or FGG was applied. The primary efficacy endpoint was the ability of the LCC to regenerate ≥2 mm keratinized gingiva a

Therapeutic effect of topical ozonated oil on the epithelial healing of palatal wound sites: a planimetrical and cytological study.

AIM: To evaluate the effect of ozonated oil on palatal wounds. METHODS: Eighteen patients were randomized and allocated to either the ozone group (n = 8) or control (n = 10) group. Free gingival graft surgery was performed, and post-harvested palatal wounds were treated with either 2 mL ozonated oil or control oil daily for 1 week. A planimetrical analysis analyzed the digital image for the wound sizes and shape factor at baseline, at 24 h, and days 5, 7, 14, 21, and 28, postoperatively. A cytological analysis used the keratinization and superficial cell indices at baseline, 24 h, and days 3,

Soft tissues around long-term platform switching implant restorations: a histological human evaluation. Preliminary results.

BACKGROUND: Switching platform restorations seems to reduce the peri-implant bone resorption and to preserve the peri-implant soft tissues. AIM: The aim of the present human study was to compare histologically the peri-implant soft tissue in switching and traditional platform implants 4 years after restoration. MATERIALS AND METHODS: Forty-eight months after implant restoration, 37 peri-implant soft tissue samples from 14 patients were harvested from traditionally restored implants (control group) and from three different platforms mismatching 0.25-0.85 mm (test groups). At the harvesting ti

Peri-implant tissue and radiographic bone levels in the immediately restored single-tooth implant: a retrospective analysis.

BACKGROUND: The purpose of this study was to compare and evaluate bone and soft tissue levels in immediately restored single implants positioned in the esthetic anterior region. METHODS: Forty-five patients requiring single-implant crowns in the anterior region received 52 implants, which were restored immediately. Periapical radiographs and digital images of 368 interproximal sites were taken during regular follow-ups from 24 to 72 months after surgery. The interproximal papilla was assessed and compared to the distances between the bone crest and the contact point between the natural teeth

Postoperative complications following gingival augmentation procedures.

BACKGROUND: Postoperative pain, swelling, and bleeding are the most common complications following soft tissue grafting procedures; however, detailed documentation is sparse in the literature. The aims of this prospective study were as follows: 1) to compare the frequency of complication occurrence after free soft tissue grafting (FSTG) or subepithelial connective tissue grafting (SCTG) procedures; 2) to evaluate the use of an acellular dermal matrix (ADM) as the donor tissue alternative to an FSTG or SCTG; and 3) to identify possible predictors for these complications. METHODS: Seventy-five

Achieving optimal esthetics in a patient with severe trauma: using a multidisciplinary approach and an all-ceramic fixed partial denture.

UNLABELLED: The treatment of traumatic injuries can be difficult, especially when the patient's expectations are too high or unreasonable. Such cases require extremely clear communication with the patient before beginning the case, including an explanation of the final results, based on the patient's goals. Taking over a case from another colleague adds more complications and requires that close attention is paid to the reason for the original dissatisfaction between the former dentist and patient to avoid repeating the same mistakes. Clinically, patients with thin, fragile gingiva are extreme

Mathematical coupling can undermine the statistical assessment of clinical research: illustration from the treatment of guided tissue regeneration.

OBJECTIVES: Previous periodontal literature has shown that there is a strong relationship between treatment effects, such as guided tissue regeneration (GTR), and baseline disease severity. However, relating change to baseline values using correlation or regression is methodologically flawed due to mathematical coupling, where the statistical procedure of testing the null hypothesis-that the coefficient of correlation or slope of regression is equal to zero-becomes erroneous. The aim of this study is to investigate if baseline disease severity is genuinely associated with the treatment outcome

Traumatic lesions of the gingiva: a case series.

BACKGROUND: The most recent classification of periodontal diseases includes a new section on traumatic gingival lesions. Traumatic lesions of the gingiva are thought to be highly prevalent, yet the periodontal literature contains few references on the topic. The purpose of this article is to present a broad spectrum of traumatic gingival lesions of iatrogenic, accidental, and factitious origin. METHODS: Twelve clinical cases were selected to document chemical (due to aspirin, snuff, and peroxide), physical (due to malocclusion, flossing, removable partial denture, oral piercing, and self-infl

Root canal treatment in a population-based adult sample: status of teeth after endodontic treatment.

The purpose of this study was to quantify treatment outcomes of endodontically treated teeth, in a representative, population-based adult sample. The "Florida Dental Care Study" was a prospective, longitudinal, cohort study of 873 subjects who had at least one tooth at baseline and who were 45 yr or older. An in-person interview and clinical examination were conducted at baseline, 24 months, and 48 months after baseline, with 6-monthly telephone interviews between those times. Dental record information was abstracted afterward. Seventy-five teeth had conventional root-canal therapy (RCT) perfo

Interproximal free gingival grafts after membrane removal in guided tissue regeneration treatment of intrabony defects. A randomized controlled clinical trial.

The purpose of this controlled clinical trial was to compare the efficacy of two surgical approaches to protect the regenerated tissue following membrane removal in guided tissue regeneration (GTR) treated intrabony defects. Twenty-eight (28) defects, one each in 28 patients were randomly assigned after placement of the membrane to one of two treatment groups by blocking to prognostic variables. The test group received free gingival grafts on the interproximal regenerated tissue. In the control group the surgical flaps were coronally positioned over the regenerated tissue. The two groups were

Root caries incidence and associated risk factors in middle-aged and older adults.

Data from population-based longitudinal studies required to assess the incidence of root caries and associated risk factors are sparse in the literature. To this end, a group of 130 middle-aged and older adults were examined for root caries at baseline and at a follow-up visit between nine and 24 months (median: 16 months). Dental examinations were conducted by one examiner at a Tufts dental clinic using NIDR defined diagnostic criteria. Fifty percent of subjects in this study population developed one or more new root caries lesions over the follow-up period. Also, an annualised increment of 0
