Longitudinal assessment of parotid function in patients receiving tomotherapy for head-and-neck cancer.

BACKGROUND AND PURPOSE: Conventional radiotherapy is associated with high doses to the salivary glands which causes xerostomia and adverse effects on quality of life. The study aims to investigate the potential of helical tomotherapy (Hi-Art Tomotherapy) to preserve parotid function in head-and-neck cancer patients. PATIENTS AND METHODS: Seven consecutive patients treated with helical tomotherapy at the UZ Brussel, Belgium, were included. During planning, priority was attributed to planning target volume (PTV) coverage: > or =95% of the dose must be delivered to > or =95% of the PTV. Elective

Usefulness of heavily T(2) weighted magnetic resonance images for the differential diagnosis of parotid tumours.

OBJECTIVES: To evaluate the usefulness of heavily T(2) weighted (T2W) magnetic resonance (MR) images for the differential diagnosis of parotid tumours. METHODS: Conventional T2W and heavily T2W images obtained from 43 patients with parotid tumours were reviewed retrospectively. Tumours were classified pathologically into three types: pleomorphic adenoma (25 cases), Warthin's tumour (7 cases) and malignant tumour (11 cases). During interpretation of each MR image, special attention was paid to the homogeneity and signal intensity displayed by the solid portions of the tumours. RESULTS: Heavil

Radioprotection of salivary glands by amifostine in high-dose radioiodine treatment. Results of a double-blinded, placebo-controlled study in patients with differentiated thyroid cancer.

BACKGROUND AND PURPOSE: Parenchymal impairment of salivary glands following high-dose radioiodine treatment is a well-known side effect in general caused by free radicals. Therefore, the radioprotective effect of the radical scavenger amifostine was evaluated prospectively in patients receiving high-dose radioiodine treatment. PATIENTS AND METHODS: Parenchymal function was assessed by quantitative salivary gland scintigraphy performed in 50 patients with differentiated thyroid cancer prior to and 3 months after high-dose radioiodine treatment with either 3 GBq 131I (n = 21) or 6 GBq 131I (n =

Deep learning-based computer-aided diagnosis for parotid gland tumors on MRI.

OBJECTIVE: Accurate imaging-based differentiation of benign and malignant parotid tumors is essential for determining appropriate therapy. We investigated whether a deep learning (DL)-based computer-aided diagnosis (CAD) system provides incremental clinical value by improving reader performance in MRI-based evaluation of parotid gland tumors. METHODS: This study included patients who underwent surgery between 2000 and 2022. A DL model (EfficientNet-based CNN) was developed using MRI data from 170 histologically confirmed cases; model evaluation used five-fold cross-validation with patient-wis

Parotid gland radiation dose-xerostomia relationships based on actual delivered dose for nasopharyngeal carcinoma.

Xerostomia induced by radiotherapy is a common toxicity for head and neck carcinoma patients. In this study, the deformable image registration of planning computed tomography (CT) and weekly cone-beam CT (CBCT) was used to override the Hounsfield unit value of CBCT, and the modified CBCT was introduced to estimate the radiation dose delivered during the course of treatment. Herein, the beams from each patient's treatment plan were applied to the modified CBCT to construct the weekly delivered dose. Then, weekly doses were summed together to obtain the accumulated dose. A total of 42 parotid gl

Low prevalence of transfusion transmitted virus (TTV)-like DNA sequences in cystadenolymphoma and pleomorphic adenoma of the salivary glands.

Titers of transfusion transmitted virus (TTV)-like DNA in saliva samples have been reported 100-1,000 times higher than those of the corresponding sera, suggesting viral transmission by saliva droplets. The present study was conducted to determine whether TTV-like DNA sequence elements play a role in the pathogenesis of cystadenolymphoma or pleomorphic adenoma and if the parotid or the submandibular gland is a major source of TTV persistence. Sixty-two archival salivary gland samples (16 cystadenolymphomas, 13 pleomorphic adenomas, and 33 controls) and 23 corresponding saliva samples were exam

Basal cell adenoma of the parotid gland; MR features and differentiation from pleomorphic adenoma.

OBJECTIVES: Among the benign tumours of the parotid gland, basal cell adenoma (BCA) is far less common than pleomorphic adenoma (PA). MR features of BCA, including diffusion-weighted imaging and dynamic contrast-enhanced study, have not been previously described. Assessment of the crucial MR features of BCA appears to offer beneficial clues for distinguishing BCA from PA. METHODS: We retrospectively reviewed 14 BCAs and 179 PAs in the parotid gland, collected between March 2000 and May 2012, from the MRI database. RESULTS: Nearly half of the BCAs had cystic components. The average ratio of t

Treatment planning constraints to avoid xerostomia in head-and-neck radiotherapy: an independent test of QUANTEC criteria using a prospectively collected dataset.

PURPOSE: The severe reduction of salivary function (xerostomia) is a common complication after radiation therapy for head-and-neck cancer. Consequently, guidelines to ensure adequate function based on parotid gland tolerance dose-volume parameters have been suggested by the QUANTEC group and by Ortholan et al. We perform a validation test of these guidelines against a prospectively collected dataset and compared with a previously published dataset. METHODS AND MATERIALS: Whole-mouth stimulated salivary flow data from 66 head-and-neck cancer patients treated with radiotherapy at the British Co

Fine-needle aspiration cytology of a case of HIV-associated anaplastic myeloma.

Plasma cell neoplasia occurs much less frequently than high-grade B-cell non-Hodgkins lymphoma in HIV-infected patients, but is nevertheless an AIDS-associated malignancy. In this report, we describe the fine-needle aspiration (FNA) findings of a mass in the left parotid region with plasmablastic features that occurred in a 41-yr-old HIV-infected homosexual man whom we diagnosed as having anaplastic myeloma. The patient had normochromic, normocytic anemia with a hematocrit of 21%, a white blood count of 2.2 x 10(9)/l with 76% neutrophils, and a CD4 count of 31%. He also had elevated levels of

A clinical concept for interfractional adaptive radiation therapy in the treatment of head and neck cancer.

PURPOSE: To present an approach to fast, interfractional adaptive RT in intensity-modulated radiation therapy (IMRT) of head and neck tumors in clinical routine. Ensuring adequate patient position throughout treatment proves challenging in high-precision RT despite elaborate immobilization. Because of weight loss, treatment plans must be adapted to account for requiring supportive therapy incl. feeding tube or parenteral nutrition without treatment breaks. METHODS AND MATERIALS: In-room CT position checks are used to create adapted IMRT treatment plans by stereotactic correlation to the initi
