[Cystic fibrosis revealed by dehydration with hypochloronatremic alkalosis in 3 infants and a neonate].

BACKGROUND--Acute dehydration with hypochloronatremic metabolic alkalosis is a classical complication of cystic fibrosis of the pancreas. Its progressive development as a revealing manifestation of the disease is rare as is its appearance in newborns. Case n. 1.--A 13 month-old girl was admitted because of status epilepticus. She was severely dehydrated (20% weight loss) but had normal diuresis. Investigations showed metabolic alkalosis, hypochloronatremia and hypokalemia. All the manifestations disappeared within 5 days with treatment but three sweat tests were abnormal. Case n. 2.--A 7 month…

Chronic/semichronic limbic epilepsy produced by microinjection of tetanus toxin in cat hippocampus.

We describe an animal preparation in which a semichronic or chronic limbic epileptiform syndrome can be produced reliably by unilateral micr … e first 3 days of the syndrome, since life-threatening generalized tonic-clonic seizures (GTCS) and status epilepticus would develop if the animal were left untreated. If severe seizures were prevented by antiepileptic drugs (AEDs) there was complete remission of the syndrome and repeat injection was necessary to reinitiate seizures. Animals that experienced severe …

Re-evaluating the neuroprotective landscape of Nigella sativa: Molecular mechanisms, nanomedicine, and clinical translation.

Nigella Sativa (NS) L., commonly known as black seed, and its major bioactive compound, thymoquinone (TQ), have long been used in traditiona … ne-induced neuronal injury, global and focal cerebral ischemia-reperfusion, pentylenetetrazole- and status epilepticus-associated seizures, and toxicant- or trauma-related brain injury. However, the clinical translation of native TQ is limited by its poor aqueous solubility, rapid systemic metabolism, and restricted penetration of the blood-brain barrier. These chal …

METHODS: A cohort of 74 patients with DRE (46 with partial seizures (PS) and 28 with ISE) and 25 healthy controls was enrolled. PD-1 levels in CSF and plasma were quantified by flow cytometry and ELISA. I …

Persistent glutamate increase in TANGO2 deficiency with normalization after vitamin supplementation: first documentation by MR spectroscopy.

BACKGROUND: TANGO2 deficiency is a rare inherited metabolic disorder characterized by recurrent metabolic crises, rhabdomyolysis, arrhythmia … S: A 2-year-old boy with genetically confirmed TANGO2 deficiency presented with a metabolic crisis, status epilepticus, and rhabdomyolysis (CK peak, 282,695 U/L). Despite the absence of structural abnormalities on the magnetic resonance imaging conducted on day 2, MRS revealed markedly increased glutamate (Glu; 11.51 mM; reference: 6.70 ± 0.59) with normal glutamine …

Intrathecal dexamethasone in FIRES.

BACKGROUND: New-onset refractory status epilepticus (NORSE) and its subset, febrile infection-related epilepsy syndrome (FIRES), are rare but severe neurological disorders. Conventional systemic immunotherapies such as intravenous methylprednisolone, intravenous immunoglobulin, and biologic agents often yield suboptimal outcomes. Intrathecal dexamethasone (IT-DEX) has emerged as a potential therapeutic option. Here, we review existing literature on the use of IT-DEX in patients with FIRES. METHODS: Searches of PubMed and Google Scholar (1 January 2010-31 December 2025) was performed, and repo…

Intranasal midazolam vs mouth-dissolving clobazam in terminating seizures: A randomized controlled trial.

OBJECTIVE: Acute seizure crises, such as prolonged seizures and impending status epilepticus, need prompt out-of-hospital treatment with benzodiazepines. However, efficacious and easily administered rescue medications are grossly underutilized and form an unmet need in management paradigms. The aim of the current study was to compare the efficacy of mouth-dissolving clobazam with intranasal midazolam in terminating seizures in the epilepsy monitoring unit (EMU). METHODS: A single-center, prospective, randomized, open blinded end point (PROBE) trial was conducted over a period of 1.5 years. Pa…

Audiogenic Kindling Stimulation Fails to Induce Cerebral Overexpression of P-Glycoprotein and Limbic Crises in the GASH/Sal Model of Epilepsy.

Experimental evidence indicates that a high seizure burden can induce cerebral overexpression of P-glycoprotein (P-gp) at the blood-brain barrier, a phenomenon associated with drug-resistant epilepsy under the "transporter hypothesis", but also at the neuronal level, linked to a reduced seizure threshold, increased seizure severity (SS), status epilepticus (SE), and a high spontaneous death (SD) rate. In contrast, we recently described a progressive reduction in SS and the absence of SE and SD in GASH/Sal hamsters subjected to 45 audiogenic seizures. Here, we examined SS, SE, and the SD, and t…

Antiseizure medication reduction in long-term video-electroencephalographic monitoring for presurgical evaluation: A multicenter safety and efficacy analysis.

OBJECTIVE: In people with refractory focal epilepsy, long-term video-electroencephalographic monitoring (LT-VEEG) is a valuable tool for surgical evaluation. Antiseizure medication (ASM) is often tapered during LT-VEEG. ASM reduction may increase the risk of complications such as status epilepticus, and efficacy has not been established. We characterized contributing factors to successful and safe LT-VEEG within a large multicenter dataset. METHODS: Adults and children were included from eight tertiary epilepsy centers in Europe for whom LT-VEEG was part of the preoperative assessment for epi…
