Large-scale evaluation of enzyme-linked immunospot assay and skin test for diagnosis of Mycobacterium tuberculosis infection against a gradient of exposure in The Gambia.

The purified protein derivative (PPD) skin test for Mycobacterium tuberculosis infection lacks specificity. We assessed 2 more specific M. tuberculosis antigens (ESAT-6 and CFP-10) by enzyme-linked immunospot assay (ELISPOT) compared with PPD by ELISPOT and skin test in The Gambia. Of 735 household contacts of 130 sputum smear-positive tuberculosis cases, 476 (65%) tested positive by PPD ELISPOT, 300 (41%) tested positive by PPD skin test, and 218 (30%) tested positive by ESAT-6/CFP-10 ELISPOT. Only 15 (2%) had positive ESAT-6/CFP-10 results and negative PPD results by ELISPOT. With increasing

Local nerve damage in leprosy does not lead to an impaired cellular immune response or decreased wound healing in the skin.

This study investigated whether peripheral nerve damage in patients with leprosy impairs local cellular immune responses, thereby reducing wound healing and leading to chronic skin ulceration. Anesthetic and contralateral sensitive skin sites in 42 patients with leprosy were compared for delayed-type hypersensitivity responses to purified protein derivative (PPD) of tuberculin. Leukocyte recruitment, epidermal activation, keratinocyte proliferation, and rates of wound healing after skin biopsy were compared. No significant differences in PPD-induced induration, epidermal activation and thicken

[Children and tuberculosis--diagnosis, treatment and follow-up].

BACKGROUND: Since the 1980s, there has been an increase in the incidence of tuberculosis (TB) world-wide. Mainly due to immigration, the number of new TB cases in the Norwegian child population is also increasing. In March 2000, a selected group of Norwegian paediatricians interested in infectious diseases held a meeting to discuss the clinical management of tuberculosis in children, and develop recommendations. MATERIAL AND METHODS: The recommendations are based on current British, European and American recommendations. Two recent Norwegian recommendations on the management of tuberculosis i

Treatment of latent tuberculosis infection in patients aged > or =35 years.

Treating patients aged > or =35 years for tuberculosis infection has been controversial because of the hepatotoxic effects of isoniazid. A 2-year retrospective cohort study of outpatient medical records determined the completion rate in this age group and identified risk factors associated with isoniazid-associated hepatotoxicity. Isoniazid preventative therapy was well tolerated. However, toxicity occurred in women receiving concomitant medications and men who used alcohol.

Mycobacteria in prurigo nodularis: the cause or a consequence?

BACKGROUND: Prurigo nodularis (PN) is a chronic skin disorder; its cause remains unknown. OBJECTIVE: We evaluated mycobacteria as a possible cause of PN. METHODS: Forty-three patients with PN were examined. Skin biopsy specimens were obtained for microbiologic and histopathologic studies. The patients were tested for intracutaneous reactivity to 12 mycobacterial antigens with the Mantoux technique. RESULTS: Six specimens (14%) grew mycobacteria in culture: M. avium-intracellulare (3), M. malmoense (1), and Mycobacterium sp. (2). Histopathologically, 12 samples (28%) were positive for acid-f

[Current clinical manifestations of tuberculosis].

Tuberculosis is an enormously serious and growing global disease that is becoming increasingly concentrated in the so-called "developing" nations of the world, where the epidemic is fuelled by the rapid expansion of the population, the presence of extreme poverty, and the coexistence of HIV infection. Most persons who become infected with M. tuberculosis do not develop the disease tuberculosis. In those who do, two types are recognised: 1) early progression of the primary pulmonary lesion or its lymph node component, or a complication from hematogenous dissemination, and 2) late reactivation o

[The clinical characteristics of pleural tuberculosis in patients with and without human immunodeficiency virus infection].

OBJECTIVE: To determine whether or not there are differences in the characteristics of pleural tuberculosis (PT) related to whether patients are or are not infected by human immunodeficiency virus (HIV). METHODS: We conducted a retrospective study of the medical histories of patients diagnosed of PT in our hospital between 1986 and 1993. RESULTS: We found no significant differences in the proportions of tuberculosis patients with or without HIV infection (8% versus 11%) who were diagnosed of PT. Of the 119 patients diagnosed of PT, 10% were also HIV positive. The HIV patients had more seriou

A school- and community-based outbreak of Mycobacterium tuberculosis in northern Italy, 1992-3. The Lodi Tuberculosis Working Group.

In late 1992, three cases of smear-positive tuberculosis were diagnosed among secondary school students in Lodi, Italy. The three attended different schools but travelled on the same bus. Schoolmates, other bus riders, family members, and friends underwent tine testing and X-rays. Of the 3188 students tested, 277 (8.7%) were reactors. Independent risk factors for tine reactivity among students included living in the same town (odds ratio [OR] = 4.8; 95% confidence intervals [CI] = 3.8-6.0); having classroom contact (OR = 4.4; 95% CI = 3.4-5.7); or riding the same bus (OR = 5.4; 95% CI = 4.3-6.

Evidence for cell-mediated autoimmunity in patients with pemphigus vulgaris and bullous pemphigoid.

Lymphoid cells from 4 of 5 patients diagnosed as pemphigus vulgaris (PV) and 6 of 7 patients diagnosed as bullous pemphigoid (BP) demonstrated specific cell-mediated immunity by the production of migration inhibitory factor (MIF) in the presence of autologous epidermal saline extracts. Clinical treatment of these patients with immunosuppressive agents resulted in a state of unresponsiveness of their lymphoid cells to similar concentrations of the antigen. Controls consisted of lymphoid cells from patients with bullous burns or various drug allergies which failed to show significant MIF product

Conditions that mimic skin cancer.

There are several conditions that may clinically and microscopically mimic skin cancer. Some of the more serious and unusual differential problems are infections, neoplasms, non-infectious granulomas, and ulcers. Particular attention is given to giant keratoacanthoma dn the Buschke-Lowenstein wart.

New observations on the epidemiology and pathogenesis of sporotrichosis.

Five uncommon cases of skin sporotrichosis are presented, including a small family outbreak with interesting epidemiological findings and two patients treated with corticosteroids; we attributed the severity of the lesions to the steroids. Mice were used to confirm our hypothesis on the pathogenesis. The pathological findings in case of sporotrichosis in a dog are presented. A group of 359 pulmonary patients was tested with two different antigens of Sporotrichum schenkii.

Sequestration of macrophages in growing tumours and its effect on the immunological capacity of the host.

The effects of rat tumours of various macrophage contents on the syngeneic host's ability to produce either: (1) an inflammatory exudate in response to intraperitoneal oyster glycogen or (2) a cutaneous delayed hypersensitivity (DHS) response to PPD or SRBC after appropriate sensitization, were studied as a function of tumour growth. Both these reactions were found to be markedly decreased as the tumours grew. The suppression was greatest in animals bearing tumours of high macrophage content. The suppression of the DHS response could be reversed by a local injection of normal peritoneal macroph

Cytomorphological diagnosis of sarcoidosis using EBUS-TBNA in a tuberculosis-endemic region.

BACKGROUND: Sarcoidosis, a granulomatous inflammatory disease, exhibits diverse clinical manifestations, often affecting multiple organs. Diagnostic challenges arise due to its similarities with tuberculosis, particularly in high-burden areas. Differentiating between the two relies on clinical judgment, laboratory tests, imaging, and invasive procedures. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) has emerged as a valuable diagnostic tool, enhancing both accuracy and patient care. MATERIAL AND METHODS: This study enrolled 279 suspected sarcoidosis cases, evalu
