[Principle requirements of an electromechanical transducer for implantable hearing aids in inner hearing hearing loss. II: Clinical aspects].

In cases of sensorineural hearing loss, an electromechanical transducer of implantable hearing aids should partially or completely compensate for the cochlea amplifier (CA) failure that occurs in most of these patients. CA failure can be clinically identified by a positive recruitment and amplitude loss in otoacoustic emissions (OAE). In a healthy ear, the basis of the CA is the motility of the outer hair cells. A transducer that replaces functions of the CA can be termed CAI (cochlea amplifier implant). A CAI should fulfill five requirements: (1) production of a vibratory output signal, (2) r

Help for the hearing-impaired. Early evaluation, advanced technology, improved management.

Nearly one in ten Americans has some degree of hearing loss. The primary care physician generally is the first to evaluate patients who complain of hearing loss and thus has the first opportunity to determine the need for treatment or referral to an otolaryngologist or otologist. New technology provides better management of hearing loss than ever before. Virtually every case of hearing loss can now be corrected or improved.

[Prevention in otology-the key to lifelong hearing health].

BACKGROUND: Hearing loss is a major global health issue with potentially severe consequences for speech development, social integration, and … ing screening is a cornerstone of early diagnosis and intervention, enabling superior outcomes with cochlear implantation or emerging gene therapies. Recommended childhood immunizations, noise protection, cautious use of ototoxic medications, and managing lifestyle-related risk factors are effective strategies for preventing acquired hearing loss. Furthermore, auditory …

Recent advancements in bioelectronic devices to interface with the peripheral vestibular system.

Balance disorders affect approximately 30% of the population throughout their lives and result in debilitating symptoms, such as spontaneous vertigo, nystagmus, and oscillopsia. The main cause of balance disorders is peripheral vestibular dysfunction, which may occur as a result of hair cell loss, neural dysfunction, or mechanical (and morphological) abnormality. The most common cause of vestibular dysfunction is arguably vestibular hair cell damage, which can result from an array of factors, such as ototoxicity, trauma, genetics, and ageing. One promising therapy is the vestibular prosthesis,

Surgical and audiological outcomes of a fully implantable middle ear implant: early results from a retrospective multicentre study.

PURPOSE: This paper reports the surgical and audiological outcomes of a large series of patients who received a fully implantable middle ear implant (MEI): carina (Cochlear, Australia). METHODS: This is a multicentre retrospective study involving three tertiary referral centres. Patient data were collected for the first 42 consecutive patients who were fitted with the MEI between 2014 and 2019 (Sheffield from February 2017 to January 2019; São Paulo from April 2015 to September 2017; Porto from December 2014 to May 2017). The main outcome measures included surgical results, free field speech

[Dehiscence syndromes : Diagnosis and treatment].

BACKGROUND: Dehiscence syndromes of the semicircular canals are a relatively new group of neurotological disorders. They have a variety of symptoms with hearing/balance involvement. Younger patients have clinically relevant symptoms in only about one third of cases. In addition to etiology and pathogenesis, the present paper describes diagnostic and therapeutic possibilities using a patient series of the authors. MATERIALS AND METHODS: This nonrandomized prospective study included 52 patients with uni-/bilateral dehiscence syndromes of the superior and/or posterior canal (SCDS/PCDS), diagnose

Neurofibromatosis type 2 (NF2): diagnosis and management.

Neurofibromatosis type 2 (NF2) is an autosomal dominant inherited tumor predisposition syndrome caused by mutations in the NF2 gene on chromosome 22. Affected individuals develop schwannomas typically involving both vestibular nerves leading to hearing loss and eventual deafness. Rehabilitation with brainstem implants and in some cases cochlear implants is improving this outcome. Schwannomas also occur on other cranial nerves, on spinal nerve roots and peripheral nerves, and intracutaneously as plaques. Cranial and spinal meningiomas and spinal ependymomas are other common tumors. Fifty to six

Intraoperative auditory steady state response measurements during Vibrant Soundbridge middle ear implantation in patients with mixed hearing loss: preliminary results.

OBJECTIVE: To optimize intraoperatively the coupling of the floating mass transducer (FMT) of the Vibrant Soundbridge middle ear implant to the round or oval cochlear window in patients with mixed hearing loss. STUDY DESIGN: Intraoperative measurement of objective hearing thresholds using auditory steady state responses (ASSRs). SETTING: Radboud University Nijmegen Medical Centre, tertiary referral hospital. PATIENTS: Four individuals with mixed hearing loss and, at least no incus, in need for a middle ear implant. INTERVENTION: Surgical placement of the Vibrant Soundbridge. ASSR threshold

A novel case of Heimler syndrome in a young child with compound heterozygous PEX26 mutations: clinical and genetic insights with literature review.

Heimler syndrome is an ultra-rare autosomal recessive disorder belonging to the spectrum of peroxisomal disorders. Heimler syndrome is characterized by sensorineural hearing loss, retinal dystrophy and dental anomalies. It is caused by hypomorphic mutations in peroxisomal biogenesis genes, namely PEX1, PEX6, PEX26, and PEX10. Fewer than 30 cases have been reported to date.We report the case of a male infant born at 36 weeks of gestation with congenital bilateral sensorineural hearing loss, confirmed by abnormal auditory brainstem responses and managed with early bilateral cochlear implantation…

Detailed Clinical Features of PTPRQ-Associated Hearing Loss Identified in a Large Japanese Hearing Loss Cohort.

The PTPRQ gene has been identified as one of the genes responsible for non-syndromic sensorineural hearing loss (SNHL), and assigned as DFNA … rogressed from moderate to severe or profound HL. Five patients with profound or severe HL received cochlear implantation, and the postoperative sound field threshold levels and discrimination scores were favorable. These findings will contribute to a greater understanding of the clinical features of PTPRQ-associated HL and may be relevant in clinical practice.

[The Impact of Subtotal Petrosectomy in Cochlea Implantation].

INTRODUCTION: Cochlear Implantation (CI) in patients with chronic otitis media or existing open mastoid cavity can be challenging. Subtotal petrosectomy (STP) is an option to improve the safety of this procedure. MATERIAL AND METHODS: Retrospective study with cases of STP prior CI. RESULTS: 25 patients could be enrolled in this investigation. Over all 26 STP were performed approximately 6 months before CI. The majority of the patients suffered from a chronic otitis media or had a preexisting open cavity; in one case a complex temporal bone fracture with destruction of the external auditory c…

Highly Accurate Facial Nerve Segmentation Refinement From CBCT/CT Imaging Using a Super-Resolution Classification Approach.

Facial nerve segmentation is of considerable importance for preoperative planning of cochlear implantation. However, it is strongly influenced by the relatively low resolution of the cone-beam computed tomography (CBCT) images used in clinical practice. In this paper, we propose a super-resolution classification method, which refines a given initial segmentation of the facial nerve to a subvoxel classification level from CBCT/CT images. The super-resolution classification method learns the mapping from low-resolution CBCT/CT images to high-resolution facial nerve label images, obtained from ma…
