The utility of routine pre-chemotherapy screening with cardiac gated blood pool scan for patients at low risk of anthracycline toxicity.

Background Anthracycline-based chemotherapy is used in many malignancies. Current recommendations by several groups suggest cardiac monitoring prior to and during anthracycline therapy. We aim to review the usefulness of baseline cardiac screening for left ventricular ejection fraction to assess if it had any impact on chemotherapy decisions in patients to be treated with anthracycline-based regimens or any beneficial effect upon outcomes. Methods We conducted a retrospective three-year audit of cancer patients who underwent GBPS prior to anthracycline (doxorubicin) chemotherapy. Subjects were

A bailout approach: Transaxillary Impella implantation in toxic cardiomyopathy.

The femoral artery has been the traditional approach for implantation of the Impella left ventricular assist device. We describe the case of a young man with toxic (cocaine-related) cardiomyopathy, severe left ventricular dysfunction, and moderate mitral insufficiency who received a transfemoral Impella left ventricular assist device that was relocated to the right axillary artery due to uncontrolled femoral access bleeding. The patient immediately stabilized post-implantation and was successfully bridged to a full recovery 6 days later.

[Acid-base and potassium balance disorders in neonatal calf diarrhea - Part 2: Therapy and prognosis].

ABSTRACT: Neonatal diarrhea in calves frequently results in severe metabolic disturbances, particularly D-lactic acidosis and hyperkalemia. These conditions are associated with weakness, apathy, and, in the case of D-lactic acidosis, with neurological deficits such as impairment of the palpebral reflex. Severe hyperkalemia can be accompanied by potentially life-threatening cardiac conduction abnormalities. The second part of this review article provides a narrative overview of therapeutic approaches as well as the prognostic relevance of these key metabolic disorders. ABSTRACT: Correction of

Developmental and behavioral disruption in zebrafish larvae by mixtures of anatoxin-a and neonicotinoids.

Harmful algal blooms caused by cyanobacteria have intensified globally due to eutrophication and climate change, posing increasing risks to aquatic ecosystems and public health. During blooms, cyanobacteria can release potent neurotoxins such as anatoxin-a, a nicotinic acetylcholine receptor (nAChR) agonist that causes rapid neurotoxicity. Likewise, neonicotinoid insecticides, including acetamiprid and thiacloprid, also target nAChRs and frequently contaminate aquatic environments. Despite their shared mode of action and environmental co-occurrence, their combined toxicity remains poorly under

Evaluation of Cardiac Function After Stereotactic Body Radiation Therapy for Lung Tumors: Results of the Prospective Explorative Heart-Lung Study.

PURPOSE: Several studies show associations between cardiac radiation dose and cardiac morbidity or overall survival. To evaluate cardiac toxicity of radiation therapy in the absence of another cardiotoxic agent, we set up a prospective pilot study evaluating cardiac function using a wide range of diagnostic modalities in early-stage non-small cell lung cancer (NSCLC) patients with a tumor in close proximity to the heart. METHODS AND MATERIALS: The HALO study was a prospective, single-arm, multicenter observational pilot study. Patients scheduled for stereotactic body radiation therapy for sta

Bidirectional links between heart failure and cancer: shared pathophysiology, clinical outcomes and collaborative management.

Heart failure (HF) and cancer are leading causes of global morbidity and mortality that share a significant bidirectional relationship. Epidemiologic studies reveal that cancer patients and survivors face a substantially elevated risk of HF, largely driven by cardiotoxic systemic therapies including chemotherapy, targeted therapy, immunotherapy and radiation therapy. Conversely, individuals with HF have a markedly increased incidence of cancer. This interconnection is underpinned by age and shared modifiable risk factors, including smoking, hypertension, diabetes and obesity, as well as common

Cardiovascular risk factors and cardiac dysfunction in people with HIV and breast cancer: an observational cohort study in Botswana.

BACKGROUND: HIV, cancer, and their respective treatments are independently associated with cardiovascular risk, but limited data exist on the intersection of these conditions. The purpose of this study was to gain insights into the cardiovascular risk factor burden and cardiac function in people with HIV (PWH) treated for breast cancer. METHODS: In a cohort of PWH and breast cancer treated with anthracyclines and/or trastuzumab (2017-2022) in Botswana, we assessed pre-treatment (baseline) left ventricular ejection fraction (LVEF), and prospectively obtained an echocardiogram at least one year

Stability-indicating study of iguratimod: isolation and characterization of a potential degradant using preparative HPLC-MS, LC-HRMS, and NMR techniques.

Iguratimod (IGRD), a disease-modifying antirheumatic drug, emerged as a promising therapeutic agent due to its potent immunomodulatory effect in autoimmune diseases. In this study, a green and selective stability-indicating RP-HPLC-DAD method was established and validated to quantify IGRD in the tablet dosage form and all degradation products. The method employed an eco-friendly, isocratic approach using an Acclaim™ AmG C18 column (150 × 4.6 mm, 3 µm), achieving excellent separation within a 10-minute runtime and detection at 257 nm. The primary objective of this study focused on isolation and

Development and validation of a UPLC-MS/MS method for real-time neuropharmacokinetic monitoring of iboga alkaloids in rat brain.

Iboga alkaloids, including ibogaine, its primary metabolite noribogaine, and the naturally occurring congener ibogamine, have shown promise for the treatment of substance use and other neuropsychiatric disorders but are limited by cardiotoxic effects. Safer synthetic analogs, such as oxa-noribogaine, are under active development; however, validated analytical methods for quantifying these compounds in brain extracellular fluid are lacking. Existing approaches that rely on tissue homogenate analysis reflect total tissue drug content, encompassing intracellular, protein-bound, and vascular compa

Dialysis-associated cardiovascular disease risk in children: mechanism and management.

Despite modest improvement, the lifespan of a child on dialysis continues to be 40 years shorter compared to healthy children. Cardiovascular disease (CVD) is the leading cause of morbidity and mortality in these patients. Risk factors for CVD are present even in early stages of chronic kidney disease (CKD) and accelerate as the child's renal function deteriorates. As a result, the highest burden of CVD exists in patients on chronic dialysis. Although dialysis is life-sustaining, the dialysis procedure promotes cardiovascular damage. Both traditional and non-traditional CVD risk factors drive

Novel psychoplastogen DM506 reduces cue-induced heroin-seeking and inhibits tonic GABA currents in the Prelimbic Cortex.

Opioid use disorder is a major public health crisis that is manifested by persistent drug-seeking behavior and high relapse frequency. Most of the available treatments rely on targeting opioid receptors using small molecules that do not provide sustained symptom alleviation. Psychoplastogens are a novel class of non-opioid drugs that produce rapid and sustained effects on neuronal plasticity, intended to produce therapeutic benefits. Ibogalogs are synthetic derivatives of iboga alkaloids that lack hallucinogenic or adverse side effects. In the current study, we examine the therapeutic potentia

Role of Stem Cell-Derived Exosomes and microRNAs in Spinal Cord Injury.

Neurological disorders represent a global health problem. Current pharmacological treatments often lead to short-term symptomatic relief but have dose-dependent side effects, such as inducing orthostatic arterial hypotension due to the blockade of alpha receptors, cardiotoxic effects due to impaired repolarization, and atrioventricular block and tachycardia, including ventricular fibrillation. These challenges have driven the medical community to seek effective treatments for this serious global health threat. Mesenchymal stem cells (MSCs) are pluripotent cells with anti-inflammatory, anti-apo

Green Synthesized Zinc Oxide Nanoparticles Attenuate Acrylamide-Induced Cardiac Injury via Controlling Endoplasmic Reticulum Stress-Associated Apoptosis Through ATF3/CHOP/BCL2 Signaling in Rats.

Due to their distinctive properties, several eco-friendly metal oxide nanoparticles were assessed for their possible cardioprotective properties. Acrylamide (ACD), a pervasive chemical in food and the environment, has been linked to cardiac toxicity. Therefore, this study examined the probable protective effect of green synthesized zinc oxide nanoparticles (GS-ZNPs) against ACD-oral exposure-induced cardiac damage in rats. For 60 days, 40 male Sprague-Dawley rats were separated into four sets that orally administered distilled water, 10-mg GS-ZNP/kg b.w., 20-mg ACD/kg b.w., or GS-ZNP + ACD. Th

Impact on clinical outcomes of renin-angiotensin system inhibitors against doxorubicin-related toxicity in patients with breast cancer and hypertension: A nationwide cohort study in South Korea.

BACKGROUND: Although doxorubicin (DOX) is a commonly used potent chemotherapeutic agent in patients with breast cancer, its cardiotoxic effect is a concern, particularly in patients with hypertension. Antihypertensive renin-angiotensin system (RAS) inhibitors may potentially play a role in preventing overt heart failure (HF) due to DOX toxicity. This study aimed to evaluate whether the use of RAS inhibitors improves clinical outcomes in patients with hypertension and breast cancer undergoing DOX-containing chemotherapy. METHODS: A total of 54,344 female patients who were first diagnosed with

Electronic Health Records-Based Cardio-Oncology Registry for Care Gap Identification and Pragmatic Research: Procedure and Observational Study.

BACKGROUND: Professional society guidelines are emerging for cardiovascular care in cancer patients. However, it is not yet clear how effectively the cancer survivor population is screened and treated for cardiomyopathy in contemporary clinical practice. As electronic health records (EHRs) are now widely used in clinical practice, we tested the hypothesis that an EHR-based cardio-oncology registry can address these questions. OBJECTIVE: The aim of this study was to develop an EHR-based pragmatic cardio-oncology registry and, as proof of principle, to investigate care gaps in the cardiovascula
