[The role of stapedoplasty in the treatment of hearing impairment in the patients presenting with tympanosclerosis].

The present study included the examination and surgical treatment of 50 adult patients presenting with tympanosclerotic fixation of the stirrup. The patients comprising the first subgroup (n=25) underwent the two-stage treatment during the period from 1990 to 2009. The final phase of the treatment consisted of the implantation of different kinds of stirrup prostheses placed on the venous autograft that closed the oval window of the vestibule following partial or total instrumental stapedectomy. The second subgruop was composed of the patients (n=25) who were given the two- or three-stage surgi

The potential of onset enhancement for increased speech intelligibility in auditory prostheses.

Recent studies have shown that transient parts of a speech signal contribute most to speech intelligibility in normal-hearing listeners. In this study, the influence of enhancing the onsets of the envelope of the speech signal on speech intelligibility in noisy conditions using an eight channel cochlear implant vocoder simulation was investigated. The enhanced envelope (EE) strategy emphasizes the onsets of the speech envelope by deriving an additional peak signal at the onsets in each frequency band. A sentence recognition task in stationary speech shaped noise showed a significant speech rec

Cochlear neural degeneration is key to the variation in word scores among people with similar thresholds.

Patients with Ménière's disease show lower word scores than those with other types of sensorineural hearing loss [Grant et al., 2022, Sci. Reports 12(1):8929]. We hypothesized that cochlear neural degeneration is a key driver of these differences among patients with similar audiograms. Here, we quantified histopathology in human cochleas with hearing loss from several etiologies and assessed the contribution of each cell type to word scores in a multivariable regression. We studied Ménière's cases (n = 44), contralateral (unaffected) ears of Ménière's cases (n = 10), age-matched normal-aging c

Progress toward development of a multichannel vestibular prosthesis for treatment of bilateral vestibular deficiency.

This article reviews vestibular pathology and the requirements and progress made in the design and construction of a vestibular prosthesis. Bilateral loss of vestibular sensation is disabling. When vestibular hair cells are injured by ototoxic medications or other insults to the labyrinth, the resulting loss of sensory input disrupts vestibulo-ocular reflexes (VORs) and vestibulo-spinal reflexes that normally stabilize the eyes and body. Affected individuals suffer poor vision during head movement, postural instability, chronic disequilibrium, and cognitive distraction. Although most individua

Blocking the glucocorticoid receptor with RU-486 does not prevent glucocorticoid control of autoimmune mouse hearing loss.

BACKGROUND/AIMS: Glucocorticoids effectively manage autoimmune hearing loss, although the cochlear mechanisms involved are unknown. Previous studies of steroid-responsive hearing loss in autoimmune (lupus) mice showed glucocorticoids and mineralocorticoids were equally effective, suggesting the ion homeostasis functions of glucocorticoids may be as relevant as immunosuppression for control of autoimmune-induced inner ear disease. Therefore, to better characterize the role of the glucocorticoid receptor in autoimmune hearing loss therapy, its function was blocked with the antagonist RU-486 (mif

Hearing outcomes in children with meningitis at Red Cross War Memorial Children's Hospital, Cape Town, South Africa: A silent crisis.

BACKGROUND: Bacterial meningitis is one of the most common causes of hearing loss. Hearing loss develops within the first few days following the onset of meningitis, highlighting the importance of early audiology referrals. Post-meningitis hearing loss may lead to cochlear ossification, making traditional cochlear implantation impossible. OBJECTIVES: To determine the duration of time from meningitis diagnosis to audiology referral. A second objective was to determine the prevalence and severity of meningitis-related sensorineural hearing loss (SNHL) at Red Cross War Memorial Children's Hospit…

Aetiology-specific modification of incision design in OSIA® implantation; a cohort of 46 implants in a tertiary paediatric centre.

OBJECTIVES: Active transcutaneous bone conduction devices have expanded rehabilitation options for children with conductive, mixed, and single-sided deafness (SSD). The Cochlear™ OSIA® system improves hearing while preserving intact skin. We hypothesised that postoperative outcomes vary by operative context and that incision design may influence complication risk. METHODS: A prospectively maintained database of paediatric patients implanted between September 2021 and April 2026 was retrospectively analysed. Patients were grouped by operative context: (1) microtia/craniofacial anomalies, (2) s
