Leaks From the Tip of the J-pouch: Diagnosis, Management, and Long-term Pouch Survival.

BACKGROUND: The standard of care for surgical treatment of ulcerative colitis is restorative proctocolectomy with ileal J-pouch. Leaks from the tip of the J-pouch are a known complication, but there is a paucity of literature regarding this type of leak. OBJECTIVE: We aimed to describe the diagnosis, management, and long-term clinical outcomes of leaks from the tip of the J-pouch at our institution. DESIGN: This was a retrospective study of a prospectively maintained pouch registry. SETTING: This study was conducted at a quaternary IBD referral center. PATIENTS: Patients included those wit

Long-term outcome and quality of life after laparoscopic treatment of large paraesophageal hernia.

BACKGROUND: Laparoscopy has been widely used for surgical repair of large paraesophageal hernias (PEHs). The technique, however, entails substantial technical difficulties, such as repositioning the stomach in the abdominal cavity, sac excision, closure of the hiatal gap, and fundoplication. Knowledge of the long-term outcome (>10 years) is scarce. The aim of this article was to evaluate the long-term results of this approach, primarily the anatomic hernia recurrence rate and the impact of the repair on quality of life. METHODS: We identified all patients who underwent laparoscopic repair for

Peroneus longus tears associated with pathology of the os peroneum.

BACKGROUND: There is a range of different types of tears and pathology of the peroneal tendons. One of the least common types is the tear of the peroneus longus associated with fracture, enlargement, or entrapment at the cuboid tunnel of the os peroneum. The purpose of this study was to evaluate the pathologic patterns of these uncommon peroneal tendon tears, to review the treatment, and to report the patient outcomes following treatment with excision of the os peroneum, debridement, and tenodesis of the peroneus longus to the peroneus brevis. METHODS: A 5-year retrospective review of all pat

Surgical excision of posttraumatic ossifications at the proximal hamstrings in young athletes: technique and outcomes.

BACKGROUND: Proximal hamstring tears can be associated with chronic disability related to the unusual formation of heterotopic ossifications. The role for operative intervention in these circumstances has not been clearly defined. PURPOSE/HYPOTHESIS: The purpose of this study was to describe the surgical management of young athletes who had chronic disability related to proximal hamstring ossifications after eccentric load injuries. The hypothesis was that after surgical excision of posttraumatic heterotopic ossifications at the proximal hamstrings with concomitant repair of the tendons to th

Temporal Progression of Tracheal Wound Healing After Partial-thickness Rat Injury Analyzed by Histology and qRT-PCR.

BACKGROUND/AIM: Wound healing is difficult to study due to interspecies variation in healing mechanisms, rates, and genetic factors. This study aimed to characterize tracheal wound healing in Wistar rats (Rattus norvegicus) following a thin, partial-thickness injury using a modified Derby-Perry Excavator. MATERIALS AND METHODS: Four male Wistar rats underwent endotracheal wounding under ketamine anesthesia. Each rat was euthanized at one, three, five, or six days post-injury. Excised tracheas were bisected for histological examination (Hematoxylin and Eosin staining) and quantitative RT-PCR a

Proboscis lateralis with trans-ethmoidal meningocele and congenital nostril hypoplasia: a case report and literature review.

Proboscis lateralis is an uncommon craniofacial malformation characterized by the presence of a rudimentary tubular nasal appendage originating from the medial canthal region. The degree of severity varies according to the extent of associated structural involvement, which may include ocular and nasal abnormalities, cleft lip and/or palate, and concurrent intracranial anomalies. Multidisciplinary approach is the mainstay of definite management. We report the case of a 4-month-old infant who presented with a 3-cm tubular lesion arising from the right medial canthus. The lesion contained a centr

DBD-FISH Using Specific Chromosomal Region Probes for the Study of Cervical Carcinoma Progression.

Genomic instability is an important biomarker in the progression of cervical carcinoma. DBD-FISH (DNA breakage detection-fluorescence in situ hybridization) is a sensitive method that detects strand breaks, alkali-labile sites, and incomplete DNA excision repair in cells of the cervical epithelium. This technique integrates the microgel immersion of cells from a vaginal lesion scraping and the DNA unwinding treatment with the capacity of FISH integrated into digital image analysis. Cells captured within an agarose matrix are lysed and submerged in an alkaline unwinding solution that generates …

Chiari malformation type III and its viability. Case report and literature review.

Chiari malformation type III is a rare congenital anomaly. It consists of the posterior fossa contents herniation through an occipital or high cervical encephalocele sac. Although it has traditionally been associated with a high mortality rate, the absence of certain poor prognostic factors and appropriate medical and surgical treatment allow these children to survive and have a remarkable initial functional improvement. Surgical goals are defect repair, preservation of viable brain tissue, adequate skin coverage and hydrocephalus management. Despite all of this, the tendency of these newborns

Amniotic Membrane Graft in the Management of Complex Vaginal Mesh Erosion.

Vaginal mesh erosion is a devastating complication after pelvic floor mesh surgery and it can be treated conservatively or with surgical revision. However, the management options following a failed primary revision or complex vaginal erosions are very limited. The aim of this study is to describe a novel treatment using an amniotic membrane as an inlay graft for such patients. Eight patients who failed conservative or primary surgical revision were enrolled. The complex erosions included vaginal agglutination, multiple vaginal erosions, recurrent erosions, and mesh cutting through the urethra.

Mechanical and structural analysis of the pulmonary valve in congenital heart defects: A presentation of two case studies.

OBJECTIVE: Congenital Heart Disease (CHD) is the leading cause of pediatric mortality, with many cases affecting the right ventricular outflow tract (RVOT) or pulmonary valve (PV). Understanding the mechanics of the disease condition can provide insight into development of durable repair techniques and bioengineered replacement devices. This work presents a mechanical and structural analysis of the pulmonary valve of two pediatric cases. METHODS: Two PV tissues were excised as part of the operative procedure. One PV was obtained from a 9-month-old with Noonan syndrome (Patient 1) and the othe

Intraparotid Neurofibroma of the Facial Nerve: A Case Report.

INTRODUCTION: Intraparotid neurofibromas of the facial nerve are extremely rare and mostly associated with neurofibromatosis type 1 (NF1). CASE REPORT: This is a case of a healthy 40-year-old man, which underwent surgery for a preoperatively diagnosed benign parotid gland lesion. After identification of the facial nerve main trunk, a single large mass (6 x 3 cm) incorporating the upper nerve division was observed. The nerve portion involved in the mass could not be dissected and was inevitably sacrificed with immediate neuroraphy of the upper division of the facial nerve with 6/0 prolene. The

An evaluation of the adhesion of solid oral dosage form coatings to the oesophagus.

There is a requirement for the development of oral dosage forms that are adhesive and allow extended oesophageal residence time for localised therapies, or are non-adhesive for ease of swallowing. This study provides an initial assessment of the in vitro oesophageal retention characteristics of several widely utilised pharmaceutical coating materials. To this end, a previously described apparatus has been used to measure the force required to pull a coated disc-shaped model tablet across a section of excised oesophageal tissue. Of the materials tested, the well-studied mucoadhesive polymer sod

Periodontal plastic surgery associated with treatment for the removal of gingival overgrowth.

BACKGROUND: Excisional biopsies of gingival overgrowths, performed with safety margins, frequently result in mucogingival defects. These defects may produce esthetic problems and increase the chances of dentin hyperesthesia and its possibility of hindering oral hygiene. METHODS: Two clinical cases are reported in which gingival overgrowths were removed by excisional biopsy, resulting in unsightly defects. The first clinical case presents an invasive approach for the treatment of a recurrent pyogenic granuloma in the anterior maxilla, and the second depicts a complete removal of a peripheral o

[Severe complications after percutaneous transaxillary refixation of a glenoid rim fracture].

The common treatment for glenoid rim fractures has been open reduction and internal fixation by a deltopectoral approach. Minimally invasive procedures with percutaneous transaxillary manipulation have a high risk for neurovascular damage. In a single case we demonstrate the possible complications associated with percutaneous refixation of a glenoid rim fracture. A 34-year-old patient with an anterior glenoid rim fracture was referred to our shoulder service after percutaneous transaxillary fixation of the fracture of the glenoid. He presented a dislocated fracture with joint infection and dam

Inteins as targets for potential antimycobacterial drugs.

Protein splicing is a self-catalyzed process mediated by inteins. The observation that inteins occur only in microorganisms and that they often interrupt genes that play an essential role in nucleic acid metabolism makes them attractive as potential antibacterial targets. Because mycobacteria are the only intein-containing bacteria associated with human hosts, inteins would represent highly specific antimycobacterial targets. In Mycobacterium tuberculosis, two important proteins of DNA repair and replication, RecA and DnaB, respectively, are interrupted by inteins that must be excised by prote
