Fixing geriatric ankle fractures: fibular nail versus locking plate in a prospective multicenter study.

PURPOSE: Elderly patients with unstable ankle fractures face a high risk of wound and implant-related complications after open reduction and internal fixation (ORIF). Less invasive intramedullary fibular nail (FN) fixation may reduce soft-tissue trauma and enable earlier mobilization. METHODS: In this prospective multicenter trial with pseudorandomized allocation and protocol-permitted crossover, 55 geriatric multimorbid patients (Charlson Comorbidity Index ≥ 4; mean age FN 79 ± 8 vs. ORIF 74 ± 8 years; p = 0.053) were treated with FN (n = 39) or ORIF (n = 16). Primary outcomes were operative

Low filling ratio of the distal nail segment to the medullary canal is a risk factor for loss of anteromedial cortical support: a case control study.

BACKGROUND: Anteromedial cortical support apposition (positive and/or neutral cortical relations) is crucial for surgical stability reconstruction in the treatment of trochanteric femur fractures. However, the loss of fracture reduction is frequent in follow-ups after cephalomedullary nail fixation. This paper aimed to investigate the possible predictive risk factors for postoperative loss of anteromedial cortex buttress after nail fixation. METHODS: A retrospective analysis of 122 patients with AO/OTA 31A1 and A2 trochanteric femur fractures treated with cephalomedullary nails between Januar

Protocol for a multicentre retrospective observational cohort study in Denmark: association between the intraoperative peripheral perfusion index and postoperative morbidity and mortality in acute non-cardiac surgical patients.

INTRODUCTION: Perioperative haemodynamic instability is associated with postoperative morbidity and mortality. Macrocirculatory parameters, such as arterial blood pressure and cardiac output are associated with poor outcome but may be uncoupled from the microcirculation during sepsis and hypovolaemia and may not be optimal resuscitation parameters. The peripheral perfusion index (PPI) is derived from the pulse oximetry signal. Reduced peripheral perfusion is associated with morbidity in critically ill patients and in patients following acute surgery. We hypothesise that a low intraoperative PP

Treatment of flexion-type supracondylar fractures in children: the 'push-pull' method for closed reduction and percutaneous K-wire fixation.

UNLABELLED: Flexion-type supracondylar fractures are challenging to treat because, unlike extension-type fractures, it is difficult to take advantage of the intact periosteal hinge to stabilize the fracture fragments during percutaneous pinning. Some authors have described closed reduction of these fractures with the elbow in extension, followed by percutaneous K-wire fixation. However, percutaneous pinning with elbow in extension is technically difficult, time consuming, and usually requires the help of a skilled assistant because of persistent fracture instability. To circumvent these diffic

A Prospective Study of Comparison between Stable and Unstable Pediatric Femoral Shaft Fractures Treated With Titanium Elastic Nails.

INTRODUCTION: Femoral shaft fractures represent approximately 1.6% of all bone injuries in children. Angulation, malrotation, and shortening are not always corrected effectively by conservative methods. These also depend on fracture anatomy: Stable (transverse and oblique) and unstable (spiral and comminuted). In recent years, flexible intramedullary nailing has gained wide acceptance for treating pediatric and adolescent femoral fractures because of the lower chance of iatrogenic infection and the prohibitive cost of in-hospital traction and Spica cast care. The present prospective study was

Metal-organic Zn-zoledronic acid and 1-hydroxyethylidene-1,1-diphosphonic acid nanostick-mediated zinc phosphate hybrid coating on biodegradable Zn for osteoporotic fracture healing implants.

Zn and its alloys are increasingly under consideration for biodegradable bone fracture fixation implants owing to their attractive biodegradability and mechanical properties. However, their clinical application is a challenge for osteoporotic bone fracture healing, due to their uneven degradation mode, burst release of zinc ions, and insufficient osteo-promotion and osteo-resorption regulating properties. In this study, a type of Zn2+ coordinated zoledronic acid (ZA) and 1-hydroxyethylidene-1,1-diphosphonic acid (HEDP) metal-organic hybrid nanostick was synthesized, which was further mixed int

A novel retractor-assisted closed reduction combined with percutaneous pinning fixation for the treatment of elderly distal radius fractures: a retrospective cohort study.

BACKGROUND: Percutaneous pinning fixation (PCP) has been used for the treatment of distal radius fractures for decades, especially in the elderly with fragile soft tissue. However, achieving and maintaining a sound anatomic reduction before PCP is difficult if we use the manipulative reduction method alone. Our study innovatively applied the Steinmann pin retractor for closed reduction combined with PCP, to provide a new protocol for the treatment of distal radius fractures. METHODS: From March 2017 to July 2018, 49 patients out of 57 that met the inclusion criteria but not the exclusion crit

Clinical and radiological outcome of the locked plate osteosynthesis in distal metadiaphyseal tibial fracture according to the severity of comminution: Varus reduction would not be preferable with lower clinical outcomes.

BACKGROUND: The optimal treatment for distal tibial fractures remains a matter of debate. Nonetheless, plate osteosynthesis produces favorable results to intramedullary stabilization in aspects of alignment restoration. AIM: The aim of the study was to compare the radiologic and clinical outcomes of distal metadiaphyseal tibial fracture between a simple/wedge fracture (SWF) and a comminuted fracture (CF) using minimally invasive plate osteosynthesis (MIPO). PATIENTS AND METHODS: This retrospective study analyzed patients with SWF or CF of the distal tibial metadiaphysis that was surgically t

Effect of cerclage wiring on humeral shaft fracture associated with the third fragment.

INTRODUCTION: In humeral shaft fractures, the presence of a free bone fragment, called the third fragment or butterfly fragment, is thought to induce non-union. We hypothesised that non-union and subsequent reoperations could be reduced by performing cerclage wiring for fragments, in addition to intramedullary nailing for fractures with the third fragment. This study aimed to examine the effectiveness of cerclage wiring for the third fragments associated with humeral shaft fractures. MATERIALS AND METHODS: This study included patients with humeral shaft fractures involving the third fragment,

Comparison of the RFN-advanced femoral nailing system versus locked lateral plating in the management of distal femur fractures: A matched-cohort analysis.

INTRODUCTION: Distal femur fractures are commonly managed with retrograde femoral nailing or locked lateral plating (LLP). As implant design has evolved, more distal and complex patterns are being treated with intramedullary implants. The aim of the present study was to compare early outcomes in distal femur fractures managed with the novel DePuy Synthes RFN-Advanced (RFNA) Retrograde Femoral Nailing System to a similar cohort treated with locked lateral plating. PATIENTS AND METHODS: This is a retrospective cohort study of operative distal femur fractures that presented to our Level I trauma

[Lesion of the sciatic nerve after antegrade intramedullary nailing of proximal femoral fractures].

Proximal femoral fractures are frequent injuries that typically require surgical treatment. A rare complication is iatrogenic injury to the sciatic nerve, often caused by cerclages or suboptimally reduced fracture fragments. Our retrospective study of 224 cases in 39 months revealed an incidence of 1.8% for postoperative sensory motor deficits. The treatment includes revision surgery, such as neurolysis or improved reduction of fracture fragments. Despite surgical revision, long-term deficits often persist due to slow nerve regeneration. A precise operative technique and early diagnostic work-

Use of medialized trochanteric-entry intramedullary nail to improve alignment in proximal femur nonunions.

PURPOSE: Proximal femur shaft nonunion is a challenging problem, often occurring after malreduction that results in postoperative malalignment and decreased healing potential. Correction of malalignment is critical to achieving osseous union. Here we present a novel technique for treating proximal femur nonunions with varus malalignment using an intramedullary nail inserted through a medialized trochanteric entry point. METHODS: Patients who underwent a proximal femur shaft (subtrochanteric or proximal third shaft) nonunion operation at a single level 1 academic referral centre by two attendi

The Association of Metal Frame Construct of ESIN and radiographic bone healing of pediatric forearm fractures.

INTRODUCTION: The gold-standard surgical procedure of both-bone forearm shaft fracture repair is elastic stable intramedullary nailing (ESIN). Disadvantages effects of ESIN have suggested to be a consequence of inappropriate surgical techniques, while recommendations are not always followed. The purpose of the study was to analyze the effect of inadequate metal frame construct on impaired fracture healing, refracture and changing alignment. MATERIALS AND METHODS: It is a population-based study including all consecutive patients, aged less than 16 years, who had been treated for forearm shaft
