Influence of patellar ligament insertion angle on quadriceps usage during walking in anterior cruciate ligament reconstructed subjects.

Reduced quadriceps contraction has been suggested as an adaptation to prevent anterior tibial translation in anterior cruciate ligament (ACL)-deficient knees. This theory has been supported by a recent study that peak knee flexion moment (thought to be created by a decrease of quadriceps contraction) during walking was negatively correlated with patellar ligament insertion angle (PLIA) in ACL-deficient knees, but not in contralateral, uninjured knees. In addition, the PLIA was significantly smaller in ACL-deficient knees than in contralateral, uninjured knees. However, it is unknown whether AC

Patient-Reported Outcomes and Revision Rates After ACL Reconstruction With Quadriceps Versus Hamstring and Patellar Tendon Autografts: Sex-Stratified Results From the Swedish Knee Ligament Registry.

BACKGROUND: Quadriceps tendon (QT) autografts are increasingly used worldwide in anterior cruciate ligament (ACL) reconstruction (ACLR). However, comparative outcome studies across graft types, particularly by sex, remain limited. PURPOSE: To compare patient-reported outcomes (PROs) and revision rates after primary ACLR with QT autografts in relation to patellar tendon (PT) and hamstring tendon (HT) autografts, stratified by sex. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Patients who underwent primary ACLR (2008-2022) were identified in the Swedish Knee Ligament Registry. T

Association of baseline diaphragm, rectus femoris and vastus intermedius muscle thickness with weaning from mechanical ventilation.

BACKGROUND: To determine whether baseline diaphragm (Tdi), rectus femoris (RF) and vastus intermedius (VI) muscle thickness (TRF and TRF + VI) are associated with weaning success. MATERIAL AND METHODS: Right Tdi, TRF and TRF + VI were measured by ultrasonography within 36 h of intubation and diaphragmatic excursion (DE) was evaluated at the first spontaneous breathing trial in adult critically-ill patients. Reintubation or death within 7 days after extubation was defined as weaning failure. Weaning failure and success groups were compared in terms of ultrasonographic measurements and clinical

Saphenous nerve injury after anterior cruciate ligament reconstruction: Reduced numbness area after ligamentoplasty using quadriceps tendon compared with hamstring tendon.

PURPOSE: Injury to the infra-patellar branches of the saphenous nerve (IPBSN) is the main neurological complication of anterior cruciate ligament (ACL) reconstruction procedures. Surgical technique using quadriceps tendon (QT) autograft allows a less invasive tibial approach potentially protecting the IPBSN. The aim of this study was to compare the numbness surface of the cutaneous area supplied by the IPBSN after ACL reconstruction using either hamstring tendon (HT) or QT autografts. METHODS: This was a retrospective comparative cohort study including 51 patients who underwent ACL reconstruc

Asymmetrical thigh creases or isolated thigh crease may be a false positive sign with low predictive value in the diagnosis of developmental dysplasia of the hip in infants: a prospective cohort study of 117 patients.

BACKGROUND: The clinical importance of asymmetrical thigh creases in the diagnosis of developmental dysplasia of the hip (DDH) in infants remains controversial. The aim of this study is to investigate the association of asymmetrical skinfolds of the thigh as the only clinical finding with the DDH in infants. METHODS: One hundred and thirty-four infants between January 2010 and December 2015 were referred to our clinics for DDH with the only clinical sign being asymmetrical or isolated thigh creases and met the inclusion criteria of the study. Patients with another clinical sign for DDH in our

Medial patellofemoral ligament (MPFL) reconstruction using quadriceps tendon autograft provides good clinical, functional and patient-reported outcome measurements (PROM): a 2-year prospective study.

PURPOSE: The medial patellofemoral ligament (MPFL) is the most important ligamentous stabilizer preventing lateral patella dislocation. Numerous surgical procedures for MPFL reconstruction have been described in the literature. The aim of this study was to investigate the clinical, functional and patient-reported (PROM) outcomes 2 years after minimally invasive MPFL reconstruction performed using an autologous strip of the quadriceps tendon. METHODS: Thirty-six patients (38 knees) were included in the study. For MPFL reconstruction, a partial thickness autologous quadriceps tendon graft was u

Aging of running shoes and its effect on mechanical and biomechanical variables: implications for runners.

This study investigates the effect of running shoes' aging on mechanical and biomechanical parameters as a function of midsole materials (viscous, intermediate, elastic) and ground inclination. To this aim, heel area of the shoe (under calcaneal tuberosity) was first mechanically aged at realistic frequency and impact magnitudes based on a 660 km training plan. Stiffness (ST) and viscosity were then measured on both aged and matching new shoes, and repercussions on biomechanical variables (joint kinematics, muscular pre-activation, vertical ground reaction force and tibial acceleration) were a

Quadriceps tendon rupture in maintenance haemodialysis patients: results of surgical treatment and analysis of risk factors.

BACKGROUND: Reports of spontaneous quadriceps ruptures in end-stage renal disease (ESRD) patients are scarce, and the assessment of risk factors for tendon rupture is poorly addressed in the majority of the studies. The purpose of the present study is to report a series of patients on haemodialysis with spontaneous quadriceps tendon ruptures operated at our institution. The results of the surgical treatment are described and the potential risk factors associated with the rupture are analyzed. METHODS: Our study consisted of retrospective analysis of patient's charts. Clinical and laboratory f

Dermatomyositis associated with acute myelocytic leukemia.

Polymyositis (PM) and dermatomyositis (DM) are inflammatory myopathic diseases characterized by symmetric, proximal myopathy with or without a distinct cutaneous eruption. They have long been recognized to be associated with cancer. The common cancers associated with DM/PM include ovarian, lung, pancreatic, breast, and stomach cancer. PM/DM associated with hematological disorders and especially with acute myelocytic leukemia (AML) is extremely rare with very few cases being reported till date. In our review of literature we could find only seven such reported cases. We present here a case pres

Precision anterior cruciate ligament (ACL) reconstruction in high-level athletes.

Anterior cruciate ligament (ACL) tear is a common injury in high-level athletes and has a significant impact on a professional career. Precision management is necessary for successful return to sports (RTS) and return to a competitive level, referred to as return to play (RTP). ACL reconstruction (ACLR) is considered the gold standard and restores anteroposterior and rotatory knee stability to facilitate a successful comeback in the elite athlete's sport. Despite an overall high RTS and RTP rate in elite athletes, performance and career longevity can be significantly impacted. Management of AC

Comparative analysis of isokinetic parameters in individuals with and without chondromalacia patellae.

INTRODUCTION: Chondromalacia patella (CMP) is characterizsed by cartilage degeneration, affects young adults, more women (2:1) and is responsible for 75% of knee pain complaints in the active population. The etiology is multifactorial and may be related to extrinsic factors (trauma and burden) and intrinsic factors (patellar malalignment and quadriceps weakness). Isokinetic dynamometry (ID) can aid in the detection of the causal factors of knee pain related to CMP. OBJECTIVES: To evaluate and compare the parameters of the ID of individuals with and without CMP and correlate them with the clin

Sesamoid bones also show functional adaptation in their microanatomy-The example of the patella in Perissodactyla.

The patella is the largest sesamoid bone of the skeleton. It is strongly involved in the knee, improving output force and velocity of the knee extensors, and thus plays a major role in locomotion and limb stability. However, the relationships between its structure and functional constraints, that would enable a better understanding of limb bone functional adaptations, are poorly known. This contribution proposes a comparative analysis, both qualitative and quantitative, of the microanatomy of the whole patella in perissodactyls, which show a wide range of morphologies, masses, and locomotor ab

A wearable resistive robot facilitates locomotor adaptations during gait.

BACKGROUND: Robotic-resisted treadmill walking is a form of task-specific training that has been used to improve gait function in individuals with neurological injury, such as stroke, spinal cord injury, or cerebral palsy. Traditionally, these devices use active elements (e.g., motors or actuators) to provide resistance during walking, making them bulky, expensive, and less suitable for overground or in-home rehabilitation. We recently developed a low-cost, wearable robotic brace that generates resistive torques across the knee joint using a simple magnetic brake. However, the possible effects
