Epstein-Barr virus infection after pediatric living-related liver transplantation--management and risk factors.

INTRODUCTION: Posttransplant lymphoproliferative disorder (PTLD) is one of the severe complications after pediatric liver transplantation. Epstein-Barr virus (EBV) infection is a major risk factor developing PTLD. This study evaluates the risk factors, incidence, and clinical presentation of EBV infection at our institute. PATIENTS AND METHODS: This study examines 81 children who underwent living-related liver transplantation (LRLT) from November 2005 to December 2009. The immunosuppression protocol consisted of tacrolimus and low-dose steroids, which were withdrawn by 3 months after LRLT. Ad

The erector spinae plane block in 4 cases of video-assisted thoracic surgery.

Multimodal anaesthesia, combining epidural catheter and general anaesthesia, is a common technique in thoracic surgery, however, epidural catheter placement is not always possible. Recently, erector spinae plane block has been described, which provides analgesia like that of the epidural block, although unilateral, and which has been used in various procedures at thoracic level. At present, there are no studies comparing the efficacy or safety of this block with those commonly used in thoracic surgery. However, its safety profile and contraindications seem different from those of the epidural

Imaging in Patients With Visual Symptoms.

PURPOSE OF REVIEW: This article presents an imaging-based approach to the differential diagnosis of visual symptoms. RECENT FINDINGS: Many neurologic disorders may present with visual symptoms. Therefore, neurologists must be familiar with the array of pathophysiologic processes that cause visual symptoms. Orbital imaging is challenging owing to the small structures and different tissue interfaces within and surrounding the orbital compartment. Some of the emerging three-dimensional MRI sequences are promising and compare well with high-resolution two-dimensional sequences in the orbits. SUM

A novel method for evaluation of oxidative stress in children with OSA.

OBJECTIVES: To evaluate the role of adenotonsillar hypertrophy and the outcomes of adenotonsillectomy (AT) on oxidative stress for obstructive sleep apnea (OSA) in children using a new method; thiol/disulfide homeostasis. METHODS: The study is consisted of 45 children with OSA and 38 healthy control subjects with similar age and sex. Children 3-12 years of age with OSA, defined as having an apnea/hypopnea index (AHI) of 5 or more in an overnight polysomnography, underwent adenotonsillectomy. OSA was classified as mild (1 ≤ AHI<10), moderate (10 ≤ AHI<20) or severe (AHI≥20). Venous blood sampl

Multilocus sequence typing of serial Candida albicans isolates from children with cancer, children with cystic fibrosis and healthy controls.

Candida albicans is a fungal pathogen, but also a commensal in many individuals. Since detailed molecular studies of children carrying C. albicans are lacking, we longitudinally investigated fecal and tonsillopharyngeal samples from 10 children undergoing treatment for cancer, six children treated for cystic fibrosis (CF), and seven healthy children during the time period of 1999-2008. Multilocus sequence typing (MLST) was performed on 62 C. albicans isolates. Only three of the 23 children (13%) were colonized with genetically unrelated strains in the longitudinal follow-up. We identified 32 d

CD99 and cytokeratin-20 in small-cell and basaloid tumors of the skin.

Although it is classically a deep soft-tissue tumor of childhood, primitive neuroectodermal tumor (PNET) can occur at any age and may occasionally involve cutaneous sites. Merkel cell carcinoma (MCC) and basaloid neoplasms of cutaneous adnexa are the principal diagnostic alternatives to that tumor. The common expression of CD99 in PNET and cytokeratin-20 (CK20) in MCC suggests that these markers may be of value in this diagnostic setting, but they have not been rigorously examined in other small-cell and basaloid lesions of the skin. Accordingly, we evaluated CD99 and CK20 reactivity in formal

The Epstein-Barr virus is rarely associated with esophageal cancer.

The Epstein-Barr virus is an agent that causes African Burkitt's lymphoma, infectious mononucleosis, and Hodgkin's disease. It is also related to nasopharyngeal carcinoma and gastric carcinoma. The aim of this study was to evaluate the prevalence of the Epstein-Barr virus in esophageal cancer. Polymerase chain reaction and in situ hybridization were used to detect the Epstein-Barr virus. We detected 103 Epstein-Barr virus positive cells out of 107 of KYSE 273 cells using first standard-PCR. Epstein-Barr virus DNA could not be detected in 30 of the esophageal squamous cell carcinoma cell lines

The temporoparietal fascial flap in head and neck reconstruction.

The technique for using the temporoparietal fascial flap in head and neck reconstruction is described. The advantages and applications of this flap are discussed and illustrated with case reports. Donor site morbidity is discussed, and the details of flap harvesting are reviewed. We have found the flap to be useful in the reconstruction of a variety of defects that require thin, vascular, and durable coverage.

Cancer of the breast. Gross and histologic pathlogy.

Increased demands are made upon the pathologist to work closely with the surgeon and the mammographer in the interest of early detection. The smallest of cancers and the very earliest phases of neoplastic development are being detected, necessitating fine discriminations between cancer and noncancer. Agreement is not always uniform at these new frontiers of diagnosis, and accuracy is paramount. Tumors are populations of heterogeneous cells. Their morphology lends itself poorly to simple categorization, and their biology is not always accurately reflected in their gross and histologic appearanc

Tomography in expansile lesions of the nasal and paranasal sinuses.

Of 35 nasal and paranasal sinus masses examined by radiography and tomography, 22 malignant tumors showed purely destructive growth. Bone expansion was observed in 13 benign, and 2 slowly growing malignant masses. Pluridirectional tomography in carefully selected projections is often necessary to reveal expansile growth. The 13 expansile lesions, including mucocele, neurofibroma, dermoid, epidermoid, cementifying fibroma, angiofibroma, inverting papilloma, and cylindroma, are briefly described. Tomographic appearance alone did not characterize histopathologic entities. However, distinctions ma

Integrative Assessment of Cephalometric and Polysomnographic Features in Paediatric Obstructive Sleep Apnea: A Cross-Sectional Study.

OBJECTIVE: This study aimed to characterize pediatric patients with suspected sleep-disordered breathing (SDB) and evaluate whether cephalometric features and clinical indicators can predict obstructive sleep apnea (OSA) severity, as confirmed by polysomnography (PSG). METHODS: A cross-sectional study was conducted among 228 children referred for sleep-related breathing concerns. Data collected included demographics, clinical symptoms (snoring, mouth-breathing, open-mouth posture, malocclusion), and cephalometric measurements (ANB, SNA, SNB angles, and nasopharyngeal soft tissue ratio). Elraglusib, a Glycogen Synthase Kinase 3β Inhibitor, plus Chemotherapy with or without Immunotherapy in Patients with Recurrent, Metastatic Salivary Gland Carcinoma.

PURPOSE: Glycogen synthase kinase 3β (GSK-3β) is a known therapeutic target in cancer. Aberrant nuclear GSK-3β (nGSK-3β) expression has been shown in some salivary gland cancers. Elraglusib is a small-molecule GSK-3β inhibitor with immunomodulatory potential. We hypothesized that elraglusib plus platinum-based chemotherapy and immunotherapy priming would be a novel treatment approach for salivary gland cancer. PATIENTS AND METHODS: This phase II, open-label trial enrolled patients with recurrent or metastatic salivary gland cancer [adenoid cystic carcinoma (ACC) vs. other subtypes] with disea
