Deep Learning Improves Osteonecrosis Prediction of Femoral Head After Internal Fixation Using Hybrid Patient and Radiograph Variables.

Femoral neck fractures (FNFs) are a great public health problem that leads to a high incidence of death and dysfunction. Osteonecrosis of the femoral head (ONFH) after internal fixation of FNF is a frequently reported complication and a major cause for reoperation. Early intervention can prevent osteonecrosis aggravation at the preliminary stage. However, at present, failure to diagnose asymptomatic ONFH after FNF fixation hinders effective intervention at early stages. The primary objective of this study was to develop a predictive model for postoperative ONFH using deep learning (DL) methods

Surgical treatment of adult mandibular condylar fractures provides better outcomes than closed treatment: a systematic review and meta-analysis.

PURPOSE: The purposes of this study were to identify significant differences in clinical outcomes between open reduction and rigid internal fixation (ORIF) and closed treatment (CT) for adult mandibular condylar fractures (MCFs) and to support or refute the superiority of one method over the other. MATERIALS AND METHODS: To address our purpose, we designed and implemented a systematic review with meta-analysis. A comprehensive electronic search without date and language restrictions was performed in May 2014. The inclusion criteria were studies in humans, including randomized or quasi-randomi

S100A8 contributes to postoperative cognitive dysfunction in mice undergoing tibial fracture surgery by activating the TLR4/MyD88 pathway.

Neuro-inflammation plays a key role in the occurrence and development of postoperative cognitive dysfunction (POCD). Although S100A8 and Toll-like receptor 4 (TLR4) have been increasingly recognized to contribute to neuro-inflammation, little is known about the interaction between S100A8 and TLR4/MyD88 signaling in the process of systemic inflammation that leads to neuro-inflammation. Firstly, we demonstrated that C57BL/6 wide-type mice exhibit cognitive deficit 24h after the tibial fracture surgery. Subsequently, increased S100A8 and S100A9 expression was found in the peripheral blood mononuc

Instability mechanism of deeply buried coal seam floor under mining effects and optimization of extraction roadway layout.

Mining-induced stress redistribution leads to rock deformation, fracture propagation, gas outflow, and floor water inrush, which present significant challenges for the layout of extraction roadways. To optimize the layout in deeply buried coal seams, a stress distribution model for the floor under mining effects was developed based on semi-infinite body theory. This model elucidates the characteristics of floor stress distribution. Simulations were conducted to analyze surrounding rock stress in extraction roadways with various layouts, resulting in a proposed optimized configuration through c

Comparison of Conversion and Primary Total Hip Arthroplasty in a Retrospective Study of 140 Patients: Recommendations for Tailored Interventions.

PURPOSE: Proximal femur fractures are commonly treated with cephalomedullary nails (CMN), dynamic hip screws (DHS), or closed reduction with percutaneous pinning (CRPP). Conversion total hip arthroplasty (cTHA) may be required for complications of these procedures. We compared cTHA outcomes with primary THA (pTHA). MATERIALS AND METHODS: This retrospective study included 140 patients (70 cTHA, 70 pTHA) at Brown University Health Hospitals between September 2022 and February 2024. cTHA patients had prior failed DHS, CMN, or CRPP fixation. pTHA patients underwent surgery for degenerative joint

Radiographic identification method of safe medial pinning zone and its clinical efficacy in pediatric supracondylar humeral fracture fixation.

OBJECTIVES: Preventing iatrogenic ulnar nerve injury remains a critical challenge during medial pinning in pediatric supracondylar humeral fractures (SCHFs). This study seeks to define a safe zone for medial pin insertion and evaluate its effectiveness in minimizing complications. METHODS: Three-dimensional reconstructions of radiographs from a healthy elbow joint were generated using E3d software to delineate a safe medial pinning zone. Pediatric patients with modified Gartland type III or IV SCHFs who underwent closed reduction and percutaneous crossed pin fixation were categorized into two

Reoperation Rate Differences Between Open Reduction Internal Fixation and Primary Arthrodesis of Lisfranc Injuries.

BACKGROUND: Controversy persists as to whether Lisfranc injuries are best treated with open reduction internal fixation (ORIF) versus primary arthrodesis (PA). Reoperation rates certainly influence this debate, but prior studies are often confounded by inclusion of hardware removal as a complication rather than as a planned, staged procedure inherent to ORIF. The primary aim of this study was to evaluate whether reoperation rates, excluding planned hardware removal, differ between ORIF and PA. A secondary aim was to evaluate patient risk factors associated with reoperation after operative trea

Superior Baseplate Inclination Is Associated With Instability After Reverse Total Shoulder Arthroplasty.

BACKGROUND: Instability is the most common complication after reverse total shoulder arthroplasty (rTSA). In the native glenohumeral joint, in addition to full dislocations, more subtle forms of instability exist. However, the incidence of more subtle forms of instability, the factors associated with instability, and the effect of instability on validated outcome scores after rTSA remain poorly understood. QUESTIONS/PURPOSES: (1) After rTSA, what is the risk of instability, including more subtle forms of subjective instability? (2) What are the factors associated with instability? (3) Are mor

Primary stability of three different iliosacral screw fixation techniques in osteoporotic cadaver specimens-a biomechanical investigation.

BACKGROUND: The incidence of osteoporotic and insufficiency fractures of the pelvic ring is increasing. Closed reduction and percutaneous fixation with cannulated sacroiliac screws is well-established in the operative treatment of osteoporotic posterior pelvic ring fractures. However, osteoporotic bone quality might lead to the risk of screw loosening. For this reason, cement augmentation of the iliosacral screws is more frequently performed and recommended. PURPOSE: The aim of the present biomechanical study was to evaluate the primary stability of three methods of iliosacral screw fixation

[Development of the inpatient quality of care of surgically treated patients with a proximal femoral fracture in North Rhine-Westphalia : Analysis of 61,249 treatment courses based on data from external inpatient quality assurance].

BACKGROUND AND OBJECTIVE: Every year in Germany over 100,000 people, mostly of older age, suffer a proximal femoral fracture. Increasing case numbers in connection with an aging population and a relatively large number of concomitant diseases requiring treatment make everyday care more difficult. It is therefore of great importance to observe the quality of care for these patients using relevant quality parameters in order to be able to derive implications for everyday treatment practice. MATERIAL AND METHODS: The data of the external inpatient quality assurance from North Rhine-Westphalia fo

Circadian rhythm disruption with high-fat diet impairs glycemic control and bone quality.

Biological functions, including glycemic control and bone metabolism, are highly influenced by the body's internal clock. Circadian rhythms are biological rhythms that run with a period close to 24 hours and receive input from environmental stimuli, such as the light/dark cycle. We investigated the effects of circadian rhythm disruption (CRD), through alteration of the light/dark schedule, on glycemic control and bone quality of mice. Ten-week-old male mice (C57/BL6, n = 48) were given a low-fat diet (LFD) or a high-fat diet (HFD) and kept on a dayshift or altered schedule (RSS3) for 22 weeks.
