Permanent Childhood Hearing Impairment: Aetiological Evaluation of Infants identified through the Irish Newborn Hearing Screening Programme.

The Newborn Hearing Screening Programme (NHSP) was established in Cork University Maternity Hospital (CUMH) in April 2011. Between April 201 … ardenburg syndrome, One with Pendred syndrome and one with Pfeiffer syndrome. Five babies underwent cochlear implantation. Through adherence to the recommended protocol a possible cause of PCHI may be determined. This study has identified areas of future improvement for this service in Ireland.

A screening analysis of the GJB2 c.176 del 16 mutation responsible for hereditary deafness in a Chinese family.

OBJECTIVE: To determine whether a new-born child from a family carrying a deafness gene needs cochlear implantation to avoid dysphonia by screening and sequencing a deafness-related gene. RESULTS: Both screening and sequencing results confirmed that the new born child had a normal GJB2 gene despite the fact that she has a brother suffering from hearing loss triggered by an allelic GJB2 c.176 del 16 mutation. We cloned the GJB2 genes derived from their respective blood genomic DNA into GFP fused plasmids and transfected those plasmids into the 293T cell line to test for gene function. While th…

Chronic electrical stimulation of the auditory nerve at high stimulus rates: a physiological and histopathological study.

A major factor associated with recent improvements in the clinical performance of cochlear implant patients has been the development of speech-processing strategies based on high stimulation rates. While these processing strategies show clear clinical advantage, we know little of their long-term safety implications. The present study was designed to evaluate the physiological and histopathological effects of long-term intracochlear electrical stimulation using these high rates. Thirteen normal-hearing adult cats were bilaterally implanted with scala tympani electrode arrays and unilaterally st

Identifying delays via preschool language scales in children who are deaf/hard-of-hearing and use spoken language.

Standardized language tests guide eligibility for speech-language services in children who are deaf and hard-of-hearing (D/HH) and use spoken language. They must be sensitive enough to detect delays during critical early windows. Clinicians have noted that the Preschool Language Scale-Fifth Edition (PLS-5) may lack sensitivity to spoken language delays in young D/HH children, risking under-identification for intervention. We retrospectively compared standard scores from PLS-5 and Oral and Written Language Scales-Second Edition (OWLS-II) in 39 children using hearing aids and/or cochlear implant

Otic organoids: A model to study spiral ganglion neuron characteristics in Tmprss3-deficiency.

Organoids are valuable models to study human diseases. Cochlear implants (CIs) electrically stimulate spiral ganglion neurons (SGNs) to enable severely hearing-impaired people vocal communication. However, some studies found in patients with mutations in the TMPRSS3 gene that speech comprehension with CI was lower than for other etiologies. The reduced CI performance might be associated with reduced SGN excitability, the causes for which are largely unclear. We refined a protocol for generating SGN-like cells in otic organoids from human induced pluripotent stem cells (iPSC) and confirmed thei

The impact of print exposure on deaf student literacy.

Recent studies suggest that benefiting early from both a cochlear implant (CI) and exposure to cued speech (CS, support system for the perception of oral language) positively impacts deaf children's speech perception, speech intelligibility, and reading. This study aims to show how: 1/CS-based speech perception ("cue reading"), and speech intelligibility might also constitute precise measures for determining the impact of CI and CS on deaf students' literary performance; 2/print exposure might also be a predictive factor in this equation. We conducted regression analyses to examine the impact

Identification a novel pathogenic LRTOMT mutation in Mauritanian families with nonsyndromic deafness.

PURPOSE: Although recessive mutations in GJB2 are the common genetic etiology of sensorineural hearing impairment (SNHI), variants in LRTOMT gene were also identified, mostly in Middle East and North African populations. METHODS: Using Sanger sequencing we screened the exon 7 of LRTOMT in a cohort of 128 unrelated Mauritanian children with congenital deafness. RESULTS: Only one biallelic missense mutation, predicted as pathogenic (c.179 T > C;p.Leu60Pro) was found at homozygous state in four families. This variant, not reported before, showed a deleterious effect by SIFT (score: 0.01) and a

Recent advances in genetic etiology of non-syndromic deafness in children.

Congenital auditory impairment is a prevalent anomaly observed in approximately 2-3 per 1,000 infants. The consequences associated with hearing loss among children encompass the decline of verbal communication, linguistic skills, educational progress, social integration, cognitive aptitude, and overall well-being. Approaches to reversing or preventing genetic hearing loss are limited. Patients with mild and moderate hearing loss can only use hearing aids, while those with severe hearing loss can only acquire speech and language through cochlear implants. Both environmental and genetic factors

[Protective effect of allopurinol in the exposure to noise pulses].

Free oxygen radicals cause particularly severe tissues and organ damage. They appear to play an important role in the cochlea, mediating noise-exposure damage. In the present study 16 guinea pigs were implanted with permanent electrodes to record cochlear action potential. Eight animals were exposed to a 2-3 kHz, 125 dB SPL noise pulse, at a rate of 4 stimulations per second for 1.8 hours. Prior to noise exposure four out of eight animals were treated with a known dose of allopurinol. The remaining eight animals were used as controls. Endolymphatic malondialdehyde concentration was used as ind

Reflex threshold shift in chinchillas following a prolonged exposure to noise.

The acoustic reflex is considered to reduce transmission across the middle ear and thereby protect the inner ear from intense sounds. The dynamic properties of this reflex seem to be a function of the duration of the eliciting stimulus. Assessment of the protective action afforded by middle-ear muscle contractions for long-term noise exposures requires the knowledge of how these dynamic properties change under such conditions. Round window electrodes were implanted in eight chinchillas. Changes in the threshold of the acoustic reflex were measured during an eight-hour exposure at 95 dB SPL to

Encapsulated cell device approach for combined electrical stimulation and neurotrophic treatment of the deaf cochlea.

Profound hearing impairment can be overcome by electrical stimulation (ES) of spiral ganglion neurons (SGNs) via a cochlear implant (CI). Thus, SGN survival is critical for CI efficacy. Application of glial cell line-derived neurotrophic factor (GDNF) has been shown to reduce SGN degeneration following deafness. We tested a novel method for local, continuous GDNF-delivery in combination with ES via a CI. The encapsulated cell (EC) device contained a human ARPE-19 cell-line, genetically engineered for secretion of GDNF. In vitro, GDNF delivery was stable during ES delivered via a CI. In the chr

The bone conduction implant--first implantation, surgical and audiologic aspects.

OBJECTIVE: To report on preoperative assessment, surgery, and audiologic outcome of the first patient implanted with the bone conduction implant (BCI). BACKGROUND: The BCI is a bone conduction hearing device with an intact skin solution where the transducer is implanted close to the ear canal opening. By avoiding a percutaneous screw attachment to the skull, the BCI is anticipated to reduce complications associated with the Bone-Anchored Hearing Aid (BAHA) solution. METHODS: The first patient to receive a BCI was a 42-year-old woman with a unilateral mixed hearing loss due to tympanosclerosi

Microvascular reconstruction in salvaging the gastric conduit post-esophagectomy after iatrogenic right gastroepiploic vessel injury.

Main limitation of classical technique is limited access to cochlea in terms of cochlear rotations and related structures, thus causing difficulty in electrode insertion. Veria technique allows full access to cochlea after raising tympanomeatal flap. To our best knowledge this is first clinic-radiological study for Veria technique studying distance between tympanic segment of facial nerve and posterior wall of external auditory canal (EAC) demonstrating facial nerve safety. Prospective study was done on 50 patients having bilateral sensori-neural hearing loss. Patients underwent cochlear impla

Bioelectrical cochlear noise and its contralateral suppression: relation to background activity of the eighth nerve and effects of sedation and anesthesia.

The bioelectrical activity of the cochlea, without any ipsilateral acoustic stimulation, was recorded in awake guinea pigs (GPs) between electrodes chronically implanted at the round window (RW) and the skull. Measuring its power in the band centered around 1.0 kHz (0.5-2.5 kHz) provided an indirect measure of the ensemble background (EBA) activity of the eighth nerve. Contralateral white-noise (CLWN) stimulation reduced this EBA, presumably by activation of medial olivocochlear fibers. The aim of the investigation was to validate measurements of EBA and of its contralateral suppression in ord

Listening Effort Is Difficult to Detect in a Person's Voice: Implications for Audiology Evaluations and Conversation Partners.

PURPOSE: Listening can be effortful for a variety of reasons, including when a person misperceives a word in a sentence and then mentally repairs it using later context. The current study explored whether an external observer (in the role of a tester/clinician) could detect that effort by hearing the listener's voice as they repeat the sentence. METHOD: Stimuli were audio recordings of 13 adults with cochlear implants repeating sentences that were either intact or with a masked word that could be inferred/repaired using context (the latter of which were previously documented to elicit greater

Achondroplasia and severe sensorineural hearing loss: The role of active bone conduction implants.

The BONEBRIDGE is a partially implantable, transcutaneous bone conduction device that can be used to treat conductive or mixed mild-to-moderate hearing loss in patients who do not attain sufficient improvement from conventional hearing aids. The following case report describes sequential bilateral BONEBRIDGE implantation in a 25-year-old patient with achondroplasia and bilateral mixed-hearing loss with a significant sensorineural component in the setting of chronic suppurative otitis media. Although the patient did not meet the approved BONEBRIDGE criteria, implantation was successful with imp
