Nail Fit: Does Nail Diameter to Canal Ratio Predict the Need for Exchange Nailing in the Setting of Aseptic, Hypertrophic Femoral Nonunions?

OBJECTIVES: To evaluate patient-independent risk factors for aseptic femoral hypertrophic nonunion requiring exchange nailing, with particular reference to the fit of the nail at the isthmus within the canal. DESIGN: Retrospective case control study. SETTING: Level 1 trauma center. MAIN OUTCOME MEASUREMENTS: Between 2008 and 2012, 211 patients without any patient-dependent risk factors for nonunion were treated with a locked reamed intramedullary nail for a femoral shaft fracture. Twenty-three cases went on to hypertrophic nonunion requiring exchange nailing (treatment group) and 188 cases

Influence of the bending of the tip of elastic stable intramedullary nails on removal and associated complications in pediatric both bone forearm fractures: a pilot study.

OBJECTIVES: Both bone forearm fractures are efficiently treated by elastic stable intramedullary nail (ESIN). According to the original technique, the tip of the nail must be bent at 90° and buried under the skin. However, skin irritation is commonly reported and may lead to early surgery to shorten the tip or remove the hardware. The purpose of this study was to investigate the influence of the bending of the tip of the nail in this procedure. METHODS: We retrospectively reviewed 72 children operated on using this technique with the tip bent either at 90° or at 180°. In both groups we record

Extracorporeal shockwave therapy (ESWT) in the treatment of atrophic non-unions of femoral shaft fractures.

INTRODUCTION: As of now, the role of extracorporeal shockwave therapy (ESWT) in the treatment of atrophic non-unions is inconclusive. The purpose of the study was to investigate the effectiveness of ESWT and the role of this technology in the treatment of atrophic non-union of femurs. METHODS: We retrospectively analyzed 22 patients treated between July 1999 and September 2007 in our hospital with ESWT for atrophic non-unions of isthmic femoral shaft fractures that were initially treated surgically using closed reamed nailing procedure. Patients with open fractures, skeletal immaturity, on an

The AO/ASIF proximal femoral nail antirotation (PFNA): a new design for the treatment of unstable proximal femoral fractures.

INTRODUCTION: PFNA design compacts the cancellous bone to provide increased stability and has been bio-mechanically proven to retard rotation and varus collapse. METHODS: Between 2006 and 2007, 62 consecutive patients with unstable proximal femoral fractures were treated with the PFNA. RESULTS: Twenty males and 42 females with a mean age of 78 years (44-94) were reviewed. In 48 patients, the fracture resulted from a low energy injury. The majority of the fractures belonged to AO/ASIF types 31A2.3 (22) and 31A3.2 (29). Twelve patients required open reduction. The PFNA blade position was centr

Expandable nail system for osteoporotic humeral shaft fractures: preliminary results.

BACKGROUND: Conventional nails rely on interlocking screws for axial and rotational stability. Such screws have poor fixation in patients with poor bone quality (osteopenia). The Fixion nail does not depend on interlocking screws-axial and rotational stability is instead achieved by nail expansion. Therefore, this nail may be better suited for patients with poor bone quality who require humeral stabilization. METHODS: The system was used to manage 25 unstable humerus shaft fractures in osteoporotic bone. An antegrade approach was used in 18 patients and a retrograde approach was used in 7 pat

The unreamed AO femoral intramedullary nail, advantages and disadvantages of a new modular interlocking system. A prospective study of 67 cases.

Compared with the Reamed Femoral Nail (RFN), the Unreamed Femoral Nail (UFN) has some theoretical advantages. It can also be used for subtrochanteric fractures and ipsilateral fractures of the femoral neck, there is less iatrogenic damage to the vascularisation of the bone, the risk of fat embolism syndrome and ARDS is reduced and the operation is a less time consuming procedure, associated with less blood loss. From April 1995 to July 1996, 66 patients with 67 fractures have been treated with the UFN: 16 patients with metastatic disease, 18 patients with a subtrochanteric fracture, 32 patient

Can "Aseptic" Looking TIBIA Non-union be Result of an Unrecognized Subclinical Infection?

INTRODUCTION: The usual cause of a distal tibial fracture is a high-energy trauma. Although multiple options are available for their treatment such as intramedullary nailing, open plating, and external fixator, each of these options might result in a non-union. Knowing the type of non-union not only allows us to guess the cause but also directs us toward the best possible treatment. Despite this, we might still get surprises on the operating table due to pre-operative misdiagnosis. CASE REPORT: Reporting a case of a 42-year-old male with a 15-month-old left distal tibia non-union. The index i

Congenital pseudarthrosis of the tibia: Management and complications.

Congenital pseudarthrosis of the tibia (CPT) is a rare pathology, which is usually associated with neurofibromatosis type I. The natural history of the disease is extremely unfavorable and once a fracture occurs, there is a little or no tendency for the lesion to heal spontaneously. It is challenging to treat effectively this difficult condition and its possible complications. Treatment is mainly surgical and it aims to obtain a long term bone union, to prevent limb length discrepancies, to avoid mechanical axis deviation, soft tissue lesions, nearby joint stiffness, and pathological fracture.

Ninety-day mortality after intertrochanteric hip fracture: does provider volume matter?

BACKGROUND: Research on the relationship between orthopaedic volume and outcomes has focused almost exclusively on elective arthroplasty procedures. Geriatric patients who have sustained an intertrochanteric hip fracture are older and have a heavier comorbidity burden in comparison with patients undergoing elective arthroplasty; therefore, any advantage of provider volume in terms of mortality could be overwhelmed by the severity of the hip fracture condition itself. This study examined the association between surgeon and hospital volumes of procedures performed for the treatment of intertroch

Effectiveness of antibiotic-impregnated bone cement in treating postoperative infections after femoral shaft fracture treatment with intramedullary nailing.

Postoperative infections are a significant challenge in orthopedic surgeries, particularly after procedures involving femoral shaft fractures treated with intramedullary nailing. Antibiotic-impregnated bone cement (AIBC) offers a promising solution by providing targeted antimicrobial delivery to the surgical site, potentially enhancing infection control and bone healing. A retrospective analysis was conducted from January 2020 to January 2023, involving 40 patients who developed postoperative infections after femoral shaft fracture treatment with intramedullary nailing. All patients underwent

Does a gentamicin-coated intramedullary nail prevent postoperative infection in Gustilo type I and II tibial open fractures? A comparative study and retrospective analysis.

PURPOSE: Tibial open shaft fractures are very common and susceptible to infection, which can lead to significant morbidity especially infection and non-union. Antibiotic coated nail is one option for fixing open shaft tibial fractures to minimise infection. This study aimed to compare the clinical outcome of Gentamicin-coated tibial nails versus regular unreamed interlocking tibial nails in the treatment of type I and II tibial open fractures. METHODS: Between 2013 and 2020, in a retrospective study of 124 patients with Gustilo type I and II tibial fractures compared non-antibiotic-coated nai

[Impact of buried versus exposed flexible intramedullary nails osteosynthesis on pediatric forearm fractures].

UNLABELLED: In elastic stable intramedullary nailing (ESIN), there are different opinions among surgeons on whether to leave the nail buried in the same arm or to leave it exposed. OBJECTIVE: To determine the risk of re-fracture in patients with a nail buried directly into the amr or left exposed as a treatment for forearm fractures, and to investigate postoperative complications. PATIENTS AND METHOD: The study included 113 pediatric patients with a forearm fracture of both diaphyses. Two groups were formed according to whether the nail was buried (Group B, n: 53) in the same arm or left exp

Improving agreement in assessing subtrochanteric fracture healing among orthopedic surgeons using the Radiographic Union Score for Hip (RUSH).

BACKGROUND: Prompt identification of fracture healing is crucial for medical decision-making and clinical research. This study aims to assess the effectiveness of the Radiographic Union Score for Hip (RUSH) in subtrochanteric fractures and determine the optimal scoring threshold for fracture healing. METHODS: We obtained 94 sets of X-ray films from the postoperative follow-up of 35 patients who underwent intramedullary nail fixation for subtrochanteric fractures. Six orthopedic trauma surgeons evaluated the imaging data and determined fracture healing based on subjective judgment. They then s

Intramedullary nails in the treatment of the distal radius fractures.

The distal radius fracture is a common injury in old persons. Its treatment remains a critical challenge because of number of cases, the final cost of the procedure, the level of X-ray irradiation, and the demand of technique of fixation in osteoporotic bone. The entire closed procedure requires a strict percutaneous nailing. This offers advantages in terms of postoperative pain, per-operative irradiation, and cost. The main problem was so far the ability to insure a stable reduction in time. This point is discussed with the introduction of the "Nail-o-Flex®" nail. A continuous series of 83 pa
