Buccal versus intranasal midazolam sedation for pediatric dental patients.

PURPOSE: The purpose of this randomized, controlled, crossover clinical trial was to evaluate and compare the behavioral differences resulting from the sedative effects of atomized buccal (transmucosal) and atomized intranasal (parenteral) midazolam (0.3 mg/kg) as a method for sedation of pediatric dental patients. METHODS: Twenty-five 36- to 72-month-old patients were randomly selected and divided into two groups that were sedated with either atomized buccal in the first visit or intranasal midazolam in the second visit (0.3 mg/kg). Patient be- havior was rated, and drug acceptance by method

Combination of clonidine sedation and spontaneous breathing-pressure support upon acute respiratory distress syndrome: a feasibility study in four patients.

INTRODUCTION: As alpha-2 agonists preserve ventilator drive, patients presenting with acute respiratory distress syndrome (ARDS, Pa02/FiO2 < 200) were managed using sedation with an alpha-2 agonist, clonidine, combined to spontaneous ventilation (SV) + pressure support ventilation (PS). METHODS: Sedation was provided by an alpha-2 agonist, clonidine 1-2 microg x kg(-1( x h(-1), without bolus administration, and supplemented with a neuroleptic, loxapine, if needed. Four patients presenting with ARDS were managed with pressure support ventilation (PS = 8 cm H20,rarely 10-12 cm H20) and high PEE

Effects of sedation on echocardiographic variables of left atrial and left ventricular function in healthy cats.

Although sedation is frequently used to facilitate patient compliance in feline echocardiography, the effects of sedative drugs on echocardiographic variables have been poorly documented. This study investigated the effects of two sedation protocols on echocardiographic indices in healthy cats, with special emphasis on the assessment of left atrial size and function, as well as left ventricular diastolic performance. Seven cats underwent echocardiography (transthoracic two-dimensional, spectral Doppler, color flow Doppler and tissue Doppler imaging) before and after sedation with both aceproma

The efficacy and safety of oral pentobarbital sedation in pediatric echocardiography.

BACKGROUND: Because patient movement during echocardiography interferes with diagnostic quality, many institutions sedate children who are unable to cooperate. The purpose of this review was to determine the efficacy and safety of oral pentobarbital for sedation during pediatric transthoracic echocardiography. METHODS: Echocardiography laboratory quality assurance data were recorded for 12 years. Sedation data included adverse events, dosing, and failed sedation. The study population was grouped by age: neonates (<1 month), infants (1-12 months), and young children (1-4 years). RESULTS: A to

A national survey into the peri-operative anaesthetic management of patients presenting for surgical correction of a fractured neck of femur.

We conducted a national postal survey of trauma anaesthetists in the UK to ascertain current practice for the peri-operative anaesthetic management in patients with fractured necks of femur. We received 155 replies from 218 questionnaires sent (71.1% response rate). Regional anaesthesia was preferred by 75.8% of respondents, with 95.5% of these employing a spinal technique. This was generally performed bad side down (45.7%) using ketamine (37.3%) and/or midazolam (41.2%) to aid positioning. In all, 31.4% used fentanyl in the intrathecal injectate, whereas only 5.9% used morphine. Paracetamol a

Autonomic dysfunction predicts mortality in patients with multiple organ dysfunction syndrome of different age groups.

OBJECTIVE: Multiple organ dysfunction syndrome (MODS) is the sequential failure of several organ systems after a trigger event, like sepsis or cardiogenic shock. Mortality rate is high, up to 70%. Autonomic dysfunction may substantially contribute to the development of MODS. Our study aimed to characterize a) the spectrum of autonomic dysfunction of critically ill MODS patients; b) whether autonomic dysfunction is different in patients receiving sedation, mechanical ventilation, or catecholamines; c) the age dependency of autonomic dysfunction in MODS; and d) whether autonomic dysfunction pred

Costs of sedation using oral midazolam: money, time, and parental attitudes.

OBJECTIVE: Many agents suitable for pediatric outpatient sedation have been identified and compared, but less data have appeared on the effect of sedation use on Emergency Department (ED) length of stay (LOS) or visit costs. We sought to discover the relationship between one commonly used method of sedation, orally administered midazolam, and ED LOS and visit costs. Parents were then surveyed to determine their attitudes toward sedation given knowledge of these costs. METHODS: All ED patients under 10 years of age seen in a pediatric ED during April and May of 1996 for repair of lacerations <

Our Ultrasound Guided Brachial Plexus Block Experiences for Upper Extremity Surgeries in Pediatric Patients.

OBJECTIVES: Brachial plexus block is the most effective analgesia and anesthesia procedure for the upper extremity surgeries in pediatric patients. In recent years, ultrasound guidance for this procedure has reduced the fail and complications like pneumothorax, intravascular injection and nerve damage. However, the number of studies about brachial plexus block is not enough, particularly in pediatric patients, which remained under-researched. In our study, we aimed to discuss the efficacy and safety of the ultrasound-guided brachial plexus block in pediatric patients by retrospectively examini

[Expert consensus on the clinical application of single-use (disposable) flexible bronchoscopes].

Single-use (disposable) flexible bronchoscope is widely used clinically both in China and abroad and has become an important tool for diagnostic and therapeutic procedures in respiratory diseases. The lightweight, portability, and non-reusability of the single-use flexible bronchoscope makes its use not only similar to that of conventional flexible bronchoscope, but with additional applications and indications. Currently, single-use flexible bronchoscopes are used in pulmonology, critical care medicine, infectious diseases, anesthesia, emergency departments and other clinical settings. Chinese

Clinical predictors of apnoea-hypopnoea during propofol sedation in patients undergoing spinal anaesthesia.

This study assessed the relationship between the occurrence of apnoea-hypopnoea during propofol sedation for spinal anaesthesia and two different predictive tests of sleep apnoea: the STOP-Bang score (snoring while sleeping, daytime tiredness, observed breathing stoppages, high blood pressure-body mass index, age, neck circumference, gender); and the obstructive sleep apnoea (OSA) score. Thirty-four middle-aged men not diagnosed with obstructive sleep apnoea received propofol infusions adjusted to produce a bispectral index of 70-75. ApnoeaLink(TM) was used to estimate the incidence of apnoea-
