Shoulder dislocations are one of the most frequent joint dislocations, with recurrent dislocations being a common complication. Common practice is to immobilize the shoulder in internal rotation for two to three weeks following closed reduction. However, recent literature suggests immobilization in external rotation can be an alternative management strategy. External rotation braces have been manufactured over the years to support and maintain this position. This literature review aims to gather current evidence on the conservative management of primary traumatic anterior shoulder dislocations

BACKGROUND: Postmenopausal osteoporosis (PMO) results from a reduction in bone mass and microarchitectural deterioration in bone tissue due to estrogen deficiency, which may increase the incidence of fragility fractures. In recent years, the "gut-immune response-bone" axis has been proposed as a novel potential approach in the prevention and treatment of PMO. Studies on ovariectomized murine model indicated the reciprocal role of Th17 cells and Treg cells in the aetiology of osteoporosis. However, the relationship among gut microbiota, immune cells and bone metabolic indexes remains unknown in

BACKGROUND: Dislocation of the elbow joint is the second most common dislocation in the upper extremity, dislocation of the shoulder being the most common. It has been reported that uncomplicated dislocation of the elbow joint may be associated with a decreased range of motion, degenerative changes in the elbow joint, ectopic calcification, or neurological deficits. As the medial collateral ligament complex can be completely disrupted during dislocation, we evaluated the association between the long-term results of treatment of simple posterolateral dislocation of the elbow and the presence of

BACKGROUND: Currently, the use of radiotherapy alone for people with multiple myeloma is limited to palliation of pain, pending fracture, and control of spinal-cord compression. Single immune-checkpoint inhibitors, such as anti-programmed death-1 (anti-PD1), have not been successful. We aimed to evaluate the activity and safety of the combination of pembrolizumab and low-dose, single-fraction, hypofractionated radiotherapy to treat patients with relapsed or refractory multiple myeloma.
METHODS: For this prospective, single-centre, single-group, open-label, phase 2 trial, we recruited patients

Background and aim Distal radius fractures (DFRs) are among the most common orthopedic injuries, particularly in elderly and postmenopausal women due to osteoporosis and low-energy trauma, such as falls on an outstretched hand. Restoration of anatomical alignment is essential for optimal functional outcomes, as residual deformity can lead to stiffness, reduced grip strength, malunion, and early degenerative changes. With evolving treatment strategies, open reduction and internal fixation (ORIF) using volar locking plates has become a preferred modality due to its biomechanical stability and fa

IMPORTANCE: Medicare provides near-universal health insurance to US residents aged 65 years or older. How eligibility for Medicare coverage affects racial and ethnic disparities in operative management after orthopedic trauma is poorly understood.
OBJECTIVE: To assess the association of Medicare eligibility with racial and ethnic disparities in open reduction and internal fixation (ORIF) after distal radius fracture (DRF).
DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study with a regression discontinuity design obtained data from the Healthcare Cost and Utilization Project al

Osteomyelitis is a progressively destructive invasive infection of the bone that can result in both localized and systemic illness. This includes an acute suppurative infection, generalized weakness, a failure to thrive, a pathological fracture, and non-healing ulcers. When chronic osteomyelitis develops, therapeutic options are limited, as antimicrobial agents cannot penetrate the necrotic bone, and repeated surgical debridement may be needed. Re-establishing full thickness coverage of the wounds and ulcers associated with osteomyelitis is challenging due to factors such as ongoing pressure i

In this retrospective study we present the long-term results in 89 patients who underwent anterior interbody fusion of acute cervical spine injuries in our institution between 1972 and 1983. There were 23 dislocations, 50 fracture-dislocations and 16 burst fractures without significant posterior lesions. Only 20 patients showed no neurologic symptoms. In all 79 patients who were treated initially in our trauma department, closed reduction was performed as soon as possible. No aggravation of the neurologic conditions by this manoeuvre was observed. The mean time between injury and operation was

AIMS: Dislocation after total hip arthroplasty (THA) is a frequent cause of revision. Patients with intrinsic risk factors have been identified. The use of a dual-mobility (DM) acetabular component has shown great effectiveness in preventing dislocation, with questions regarding selective or absolute use. The aim of this study was to compare the outcome of conventional THA (C-THA) and DM-THA, when used for selected patients.
METHODS: This retrospective continuous cohort study evaluated 531 patients, of whom 313 received C-THA and 218 DM-THA. There were 354 primary osteoarthritis (67%), 121 fe

BACKGROUND: Closed reduction and percutaneous pinning of unstable proximal humeral fractures is a well-described technique with some theoretical advantages over open techniques. To our knowledge, the risk of injury to neurovascular structures from percutaneous pinning of the proximal part of the humerus has not been studied. We sought to quantify this risk using a cadaveric model.
METHODS: In ten fresh-frozen cadaveric shoulders, the intact proximal part of the humerus was pinned under fluoroscopic guidance with use of an identical published technique. A total of five 2.5-mm terminally thread

BACKGROUND: Monitored anesthesia care (MAC) combined with regional anesthetic techniques is commonly used for open reduction and internal fixation (ORIF) of unstable ankle fractures. When compared to general anesthesia (GA), MAC with regional anesthesia has been associated with reduced systemic adverse effects and shorter recovery times, although regional anesthesia carries additional costs and potential complications of its own. A new technique involving the infiltration of high-volume, low-concentration (VL) local anesthetic was therefore developed. When used in conjunction with MAC, we refe

PURPOSE: To quantitatively evaluate anterior cortical defect remodelling and bone healing following infra-tubercle anterior closing-wedge high tibial osteotomy (ACW-ITO) and to identify clinical and surgical factors associated with remodelling. We hypothesised that substantial spontaneous remodelling would occur, that greater slope correction would result in larger initial defects with greater regrowth, and that patient-related factors would influence remodelling.
METHODS: A retrospective cohort of 100 consecutive patients undergoing infra-tubercle slope-reducing osteotomy was analysed. Stand

Hip joint implants are among the most prevalent types of medical implants utilized for the replacement of damaged joints. The utilization of modern implant materials, such as cobalt-chromium alloys, stainless steel, titanium, and other titanium alloys, is accompanied by challenges, including the toxicity of certain elements (e.g., aluminum, vanadium, nickel) and excessive Young's modulus, which adversely impact biomechanical compatibility. A mismatch between the stiffness of the implant material and the bone tissue, known as stress shielding, can lead to adverse outcomes such as bone resorptio

PURPOSE: Several minimally invasive procedures were used to treat displaced intra-articular calcaneal fractures (DIACFs). No agreement among different authors about either the ideal fixation method or which technique is minimally invasive. The aim of this study was to compare functional and radiographic outcomes of two minimally invasive techniques in treatment of Sanders type II and III DIACFs by using K-wires or cannulated screws without bone grafts.
METHODS: A prospective randomized controlled study was conducted on 28 patients (34 feet) with Sanders type II or III DIACFs, treated by close

It is generally believed that earthquakes occur when faults weaken with increasing slip rates. An important factor contributing to this phenomenon is the faults' dynamic friction, which may be reduced during earthquakes with high slip rates, a process known as slip-rate weakening. It has been hypothesized that the weakening phenomenon during fault slip may be activated by thermal pressurization of pores' fluid and flash heating, a microscopic phenomenon in which heat is generated at asperity contacts due to high shear slip rates. Due to low thermal conductivity of rock, the heat generated at t
