Cerebral volume and diffusion MRI changes in children with sensorineural hearing loss.

PURPOSE: Sensorineural hearing loss (SNHL) is the most prevalent congenital sensory deficit in children. Information regarding underlying brain microstructure could offer insight into neural development in deaf children and potentially guide therapies that optimize language development. We sought to quantitatively evaluate MRI-based cerebral volume and gray matter microstructure children with SNHL. METHODS & MATERIALS: We conducted a retrospective study of children with SNHL who obtained brain MRI at 3 T. The study cohort comprised 63 children with congenital SNHL without known focal brain le

The vestibular implant: Opinion statement on implantation criteria for research.

This opinion statement proposes a set of candidacy criteria for vestibular implantation of adult patients with bilateral vestibulopathy (BVP) in a research setting. The criteria include disabling chronic symptoms like postural imbalance, unsteadiness of gait and/or head movement-induced oscillopsia, combined with objective signs of reduced or absent vestibular function in both ears. These signs include abnormal test results recorded during head impulses (video head impulse test or scleral coil technique), bithermal caloric testing and rotatory chair testing (sinusoidal stimulation of 0.1 Hz).

Electrical stimulation induces synaptic changes in the peripheral auditory system.

Since a rapidly increasing number of neurostimulation devices are used clinically to modulate specific neural functions, the impact of electrical stimulation on targeted neural structure and function has become a key issue. In particular, the specific effect of electrical stimulation via a cochlear implant (CI) on inner hair cell (IHC) synapses remains unclear. Importantly, CI candidacy has recently expanded to include patients with partial hearing loss. Unfortunately, some CI recipients experience progressive hearing loss after activation of electrical stimulation. The mechanism(s) accounting

The effect of interaural fluctuation rate on correlation change discrimination.

While bilateral cochlear implants (CIs) provide some binaural benefits, these benefits are limited compared to those observed in normal-hearing (NH) listeners. The large frequency-to-electrode allocation bandwidths (BWs) in CIs compared to auditory filter BWs in NH listeners increases the interaural fluctuation rate available for binaural unmasking, which may limit binaural benefits. The purpose of this work was to investigate the effect of interaural fluctuation rate on correlation change discrimination and binaural masking-level differences in NH listeners presented a CI simulation using a p

Auditory response to intracochlear electric stimuli following furosemide treatment.

The influence of functional hair cells on electrical stimulation of the auditory nerve is an important issue as individuals with significant residual hearing are now cochlear implant candidates. Previous work has shown that chemical deafening during the course of acute experiments changes the auditory nerve's responses to electrical stimulation [Third Quarterly Progress Report, NIH contract N01-DC-9-2106 (2000), Final Report, NIH Contract N01-DC-9-2106 (2002)]. This study extended that work by investigating the changes and subsequent recovery following furosemide injections which reversibly im

[Use of auditory brain stem implant in a patient with bilateral vestibular schwannoma (type 2 neurofibromatosis].

Surgical technique and functional results of implantation of the acoustic implant of the brain stem (BS) are described for a 16-year-old patient with bilateral vestibular shvannoma (neurofibromatosis of type 2). Initially, a large (IV degree) tumor was removed from the left cerebellopontile angle using the translabyrinthine-transcochlear approach. The function of the facial nerve recovered completely. The BS implant was implanted a year later in the course of removing shvannoma from the right cerebellopontile angle using the above approach. The patient received 22-channel implant Nucleus-CI22M

Mechanical and acoustic analysis of middle ear reconstruction.

Existing mechanical and acoustic models of the human middle ear are used to investigate the expected performance of various middle ear reconstructive procedures. Although the results of these analyses generally are consistent with observed surgical results, such an analytic approach enables the formation of some simple rules for maximizing middle ear function after stapedectomy and tympanomastoid surgery. Model analyses predict the following: (1) stapes prostheses of 0.6 to 0.8 mm in diameter produce air-bone gaps of less than 12 dB; smaller diameter prostheses produce larger air-bone gaps; (2

[A hearing implant: indications, technic and results].

The authors report four cases of implantation of a new device via an osseous approach involving implantation of an intra-osseous vibrator and of an external box, transmission being by magnetic induction using an external aerial, i.e. with no transcutaneous portion. The Xomed Audiant is an osseous rehabilitation system designed essentially for use in transmission deafness which cannot be corrected by conventional surgery or cannot be managed by an air conduction device, e.g. agenesis of the ear or sequelae of evacuation cavities in otorrhea. The authors report their experience of four cases of

Revision stapes surgery: problems with some solutions.

A review of 217 consecutive revision stapes operations performed during a ten-year period revealed that the surgeon encounters more pathological variables than he does during primary operations. Hence, the technical solutions are less stereotyped and the net results less predictable. Prosthesis displacement, with or without incus tip erosion, was the most common primary cause of failure (82%). However, oval window problems such as footplate refixation, perilymph fistula, otosclerotic regrowth, and lateralized oval window membrane in 60% of cases often complicated prosthesis displacement. First

Pharmacological protection of auditory function against noise and hypoxia with MK 801.

INTRODUCTION: Pharmacological protection of cochlear function is one of the most exciting goals of inner ear medicine. The selective glutamate antagonist MK 801 may be neuro-protective to the cochlea.1 This study aims to investigate whether round window administration of MK 801 protects cochlear function against a combination of noise and atmostpheric hypoxia. METHODS: Eight guinea-pigs were studied. Round Window E. Catheters (Durect Inc., USA) were implanted through a trans-bulla approach and EcoG thresholds determined at 8, 16, 24 and 30 kHz. Artificial perilymph (n = 4) or 10 mM MK 801 (n =

Pitch matching psychometrics in electric acoustic stimulation.

OBJECTIVE: Combined electric-acoustic stimulation (EAS) is a therapeutic option for patients with severe to profound mid- and high-frequency hearing loss while low-frequency hearing is mostly unaffected. The present study investigates bimodal pitch matching in EAS users as a function of the angular placement of electrodes. Results are compared with data obtained from previous pitch matching studies. Knowledge of electric and acoustic pitch mapping may be important for effective fitting to control the frequency range of acoustic and electric processing. DESIGN: Pitch adjustment experiments wer
