Rapid intraoperative construction of autologous small caliber blood vessels.

Autologous pericardium, briefly tanned in glutaraldehyde, is an excellent biomaterial when used in various cardiac procedures, including repair and replacement of heart valves. A generalized lack of calcification and an absence of inflammatory response in these combined experiences has led the authors to consider the use of treated autologous pericardium for the construction of substitute small caliber blood vessels. Rapid, intraoperative construction of a vascular graft from autologous pericardium, briefly treated in 0.62% glutaraldehyde, is accomplished through the use of two concentric, mat

Dealing With Calcium in STEMI: A Unique Case of Intracoronary Fracture and Rescue of a Guide Catheter Extension.

Guide catheter extensions (GCE) represent a useful tool in percutaneous coronary interventions (PCI), allowing to increase support and successfully deliver material through calcific lesions or tortuous anatomy. We present the first case of fracture of the metallic collar braid of a Guidezilla TM II (Boston Scientific, Natick, MA, USA) GCE, remaining trapped in a severely calcified left anterior descending artery of a patient presenting with ST-segment elevation myocardial infarction. The fragment was retrieved from the coronary artery with snaring technique, and the PCI was competed with conve

Peri-coronary inflammation and coronary microcalcification activity in subjects with diabetes mellitus; a cross-sectional analysis.

INTRODUCTION: Coronary microcalcification activity (CMA) independently prognosticates cardiovascular disease risk. The relationship between dysfunctional peri-coronary adipose tissue (PCAT), a measure of local inflammation, and CMA has not been characterised in people with diabetes mellitus. METHODS: Subjects with diabetes mellitus underwent 18F-Sodium Fluoride (18F-NaF) Positron Emission Tomography (PET) imaging and Computed Tomography (CT) coronary calcium score (CCS) imaging. Non-contrast PCAT (NC-PCAT) was quantified using the CT-value (Hounsfield Units (HU)) of the tissue adjacent to the

Procedural and Clinical Outcomes After Orbital Atherectomy Versus Intravascular Lithotripsy in Patients With Calcified Nodules.

BACKGROUND: Calcified nodules (CNs) remain a major challenge in percutaneous coronary intervention (PCI). We sought to compare procedural and clinical outcomes after orbital atherectomy (OA) versus intravascular lithotripsy (IVL)-facilitated PCI in patients with CNs. METHODS: We identified patients with optical coherence tomography (OCT)-defined CNs who underwent PCI with either OA or IVL between 2012 and 2022 and had both pre- and post-PCI OCT images available. The imaging end point was the minimal stent area on final post-PCI OCT. The clinical end point was 2-year target-lesion failure, a c

Prospective Assessment of Outcomes in Left Main Coronary Artery Calcium Modification and Angioplasty Using Different Modalities in an Indian Population (PROLEMCA) Study.

BACKGROUND: Calcification of the left main coronary artery (LMCA) is a critical factor influencing outcomes after percutaneous coronary intervention (PCI) in both LMCA and other coronary arteries. Inadequate lesion preparation can lead to complications like stent thrombosis or restenosis, which result from poor stent expansion and apposition due to reduced vessel compliance. AIMS: This study aims to assess and monitor the short- and long-term outcomes of recent calcium modification techniques, specifically intravascular lithotripsy (IVL), Wolverine cutting balloon (WCB) and rotational atherec

Clinical and Coronary Artery Lesion Characteristics and Outcomes in Acute Coronary Syndrome Patients with High Bleeding Risk.

INTRODUCTION: Although it is well-known that patients with high bleeding risk (HBR) who undergo percutaneous coronary intervention (PCI) have poor clinical outcomes, the details have not been fully clarified for acute coronary syndrome (ACS) population. The aim of this study was to describe patient and lesion characteristics in patients with ACS and HBR as defined by the Academic Research Consortium (ARC). METHODS: Patients with ACS (n = 961) who underwent optical coherence tomography (OCT)-guided PCI were investigated. They were divided into HBR and non-HBR groups according to the ARC HBR cr

Efficacy of intravascular lithotripsy (IVL) in coronary stenosis with severe calcification: A multicenter systematic review and meta-analysis.

BACKGROUND: With heavily calcified coronary and peripheral artery lesions, lesion preparation is crucial before stent placement to avoid underexpansion, associated with stent thrombosis or restenosis and patency failure in the long-term. Intravascular lithotripsy (IVL) technology disrupts superficial and deep calcium by using localized pulsative sonic pressure waves, making it to a promising tool for patients with severe calcification in coronary bed. AIMS: The aim of the study is to systematically review and summarize available data regarding the safety and efficacy of IVL for lesion prepara

Denosumab Decreases Epicardial Adipose Tissue Attenuation in Dialysis Patients with Secondary Hyperparathyroidism and Low Bone Mass.

INTRODUCTION: Denosumab preceding elective surgery is an alternative option when parathyroidectomy is not immediately possible. Denosumab (an osteoprotegerin mimic) may play a role in the cardiovascular system, which is reflected in the features of epicardial adipose tissue (EAT) and coronary artery calcification (CAC). METHODS: We investigated the effects of denosumab on EAT attenuation (EATat) and CAC in dialysis patients with secondary hyperparathyroidism (SHPT). This cohort study included patients on dialysis with SHPT. The baseline characteristics of dialysis patients and propensity scor

Association of Estimated Long-term Exposure to Air Pollution and Traffic Proximity With a Marker for Coronary Atherosclerosis in a Nationwide Study in China.

IMPORTANCE: Epidemiologic evidence of the mechanisms of the association between long-term exposure to air pollution and coronary heart disease (CHD) is limited and relies heavily on studies performed in Europe and the United States, where air pollution levels are relatively low. In particular, the association between air pollution and CHD in patients with underlying risks for CHD is understudied. OBJECTIVE: To determine whether air pollution and proximity to traffic are associated with the coronary artery calcium (CAC) score, a key atherosclerotic marker. DESIGN, SETTING, AND PARTICIPANTS: I

Coronary Artery Calcification.

Coronary artery calcification (CAC) is an established marker of subclinical atherosclerosis and an independent predictor of future coronary heart disease in the asymptomatic primary prevention population, particularly in the intermediate risk cohort. CAC also helps in reclassifying those patients and their risk of cardiovascular events into higher or lower risk categories. MESA (Multi-Ethnic Study of Atherosclerosis) is a National Heart, Lung, and Blood Institute-sponsored population-based medical research study involving 6,814 men and women from 6 U.S. communities without a medical history of

HDL cholesterol, leptin and interleukin-6 predict high risk coronary anatomy assessed by CT angiography in patients with stable chest pain.

OBJECTIVES: Coronary computed tomography angiography (CTA) describes several features of coronary plaques, i.e. location, severity, and composition. Integrated CTA scores are able to identify individual patterns of higher risk. We sought to test whether circulating biomarkers related with metabolism and inflammation could predict high risk coronary anatomy at CTA in patients with stable chest pain. METHODS: We evaluated a panel of 17 biomarkers in 429 patients (60.3 ± 0.4 years, 268 males) with stable chest pain who underwent coronary CTA having been enrolled in the Evaluation of Integrated C

A histopathological analysis of the epidemiology of coronary atherosclerosis: an autopsy study.

Источник пометил публикацию как отозванную

BACKGROUND: Atherosclerosis accounts for a large proportion of cardiovascular system associated morbidity and mortality. We studied the possible association between the histopathological changes of the coronary atherosclerotic lesions and the risk of sudden cardiac death (SCD) using autopsy cases. METHODS: We performed an autopsy analysis (n = 13, 4 women, 9 men mean age 67.5 years; age range 56-93 years) of SCD which occurred in patients aged over 50 years during March 2010 to December 2013. The following variables were considered: sex, age, medical history, autopsy findings to macroscopic a

Association of SNPs in the UGT1A gene cluster with total bilirubin and mortality in the Diabetes Heart Study.

OBJECTIVE: A negative relationship between total bilirubin concentration (TBili) and CVD risk has been documented in a series of epidemiological studies. In addition, TBili is thought to be under strong genetic regulation via the UGT1A gene family, suggesting it may be a heritable CVD risk factor. However, few studies directly relate TBili-associated UGT1A variants to CVD severity or outcome. This study replicated the genetic association for TBili in the Diabetes Heart Study (DHS), and examined the relationships of TBili-associated SNPs with measures of subclinical CVD and mortality. METHODS:

Deep learning-based plaque characterization in hybrid IVUS-OCT images is superior to single-modality deep learning analysis and human experts: head-to-head comparison against histology.

AIMS: Hybrid intravascular ultrasound-optical coherence tomography (IVUS-OCT) can enable more accurate plaque characterization than single-modality intravascular imaging, enhancing treatment planning and vulnerable plaque detection. However, image interpretation in IVUS-OCT is challenging and time-consuming. To overcome this limitation, we introduce a novel histology-trained deep learning (DL)-classifier for plaque component classification in IVUS-OCT images and compare its performance against single-modality DL and expert analysts. METHODS AND RESULTS: IVUS-OCT frames and matched histologica

Enhanced Efficacy of Rotational Atherectomy for Calcified Nodules With Contralateral Calcification: Insights From a Multicenter Intravascular Ultrasound Imaging Study.

BACKGROUND: Calcified nodules (CNs) represent a high-risk coronary lesion phenotype associated with target lesion revascularization (TLR). Although rotational atherectomy (RA) is an established treatment for calcified lesions, its benefit for CNs remains unclear. This study aimed to evaluate the impact of RA on TLR and to identify specific morphological features on intravascular ultrasound that may influence its therapeutic effect for CNs. METHODS: In a substudy of the U-SCAN registry (Coronary Intravascular Ultrasound for Calcified Nodule), 348 patients with CNs identified by intravascular u

Associations of segment-level coronary calcium scoring metrics with major and minor cardiovascular events.

BACKGROUND: Coronary artery calcium (CAC) scoring is a well-established method for the non-invasive assessment of coronary artery calcification. Although its potential for clinical risk prediction has been discussed numerous times, there is limited evidence on the assessment of calcification at the coronary artery segment level. METHODS: This prespecified analysis of the DISCHARGE Trial (NCT02400229) included patients who received a calcium scoring CT and showed presence of coronary calcification. The diffusivity index and risk groups derived from that index, and the number of calcified segme
