Mycobacterium marinum wrist arthritis: local and systematic dissemination caused by concomitant immunosuppressive therapy.

The diagnostic difficulties and the potential serious course of Mycobacterium marinum synovitis are illustrated in a patient in whom treatment with local steroid injections and immunosuppressive therapy resulted in local destruction of the wrist and systemic reactions. Healing was achieved after correct medical and surgical treatment was initiated.

[Percutaneous management of osseous avulsions of extensor tendons and lateral ligaments of the fingers with the hooked wire].

A new method of percutaneous treatment of some fractures of the fingers is being described. With the use of a magnification table, fitted onto the X-ray intensifier, evulsion fractures of extensor tendons or collateral ligaments can be reduced and fixed by means of a K-wire. After insertion, the end of the wire is being bent in the shape of a hook ("wire hook"), into which a pull-out wire is inserted. By anchoring the wire-hook at the opposite side of the finger under tension, the fragment is held in position by the hook. After four weeks the wire-hook is cut below its anchorage and can easily

Functional cast immobilization of thumb metacarpophalangeal joint injuries.

A prospective study was performed on 40 patients who were treated with a modified thumb spica cast (MTSC) for injuries involving the region of the metacarpophalangeal joint of the thumb. Twenty-seven of these injuries occurred during sports participation. The immobilization consisted of a fiberglass cast which incorporated the thumb in a balanced position and allowed full flexion and extension of the wrist. Twenty patients had injuries involving the ulnar collateral ligament (five surgical). Three patients had radial collateral ligament injuries (two surgical). Fourteen patients had fractures

Assessment of the ligamentous stress distribution in the pathomechanics of the boutonniere deformity through a computational 3D model.

The boutonniere deformity of the digit, resulting from a flexion deformity of the proximal interphalangeal joint (PIPJ) and an extension deformity at the distal interphalangeal joint, is a well-documented condition. However, the association between alterations in forces and damage to the extensor mechanism that causes progression to a boutonniere deformity remains a gap in our understanding. To bridge this gap, we took a unique approach. We used a custom-built specialized apparatus to evaluate digital extension in eight cadaveric hands before and after transecting the central slip, oblique fib

A new surgical approach to treatment of bilateral syngnathia in a patient with popliteal pterygium-syndrome.

This case describes the treatment of syngnathia with a popliteal-pterygium syndrome. Although short-term successful surgical treatment has been described in literature, long-term follow up is essential, as the mouth opening limitations can be repetitively reoccurring. This requires new, individual treatment strategies as demonstrated in this case. A young patient is shown, who is successfully undergoing an alloplastic temporomandibular joint replacement to improve mouth opening, increasing the posterior airway space and facilitating the food intake.

Median nerve movement in the carpal tunnel before and after carpal tunnel release using transverse ultrasound.

PURPOSE: We aimed to compare the movement of the median nerve within the carpal tunnel during wrist and finger motions between before and after carpal tunnel release (CTR) using transverse ultrasound in carpal tunnel syndrome (CTS) patients and to evaluate the biomechanical efficacy of CTR for CTS. METHODS: Twenty-four patients with CTS were examined by transverse ultrasound. The location of the median nerve within the carpal tunnel was examined quantitatively as a coordinate at varied wrist positions with finger extension and flexion, respectively, before and after CTR. RESULTS: We found th

Biomechanical risk factors and flexor tendon frictional work in the cadaveric carpal tunnel.

Pathological changes in carpal tunnel syndrome patients include fibrosis and thickening of the subsynovial connective tissue (SSCT) adjacent to the flexor tendons in the carpal tunnel. These clinical findings suggest an etiology of excessive shear-strain force between the tendon and SSCT, underscoring the need to assess tendon gliding characteristics representative of repetitive and forceful work. A mechanical actuator moved the middle finger flexor digitorum superficialis tendon proximally and distally in eight fresh frozen cadaver arms. Eighteen experimental conditions tested the effects of

[Cutis verticis gyrata as an early rare presenting sign of acromegaly].

Cutis verticis gyrata (CVG) is a descriptive term for a dermal sign in which deep furrows and convoluted ridges are seen upon the scalp. They are formed due to thickening of the skin folds of the scalp and produce an appearance that resembles the gyri of the brain. The condition is classified primary when the etiology is unknown or neurologically based. CVG will be considered secondary when a definite cause, systemic or localized, is responsible for the sign. We describe a 34 year-old male who was presented with a one-month history of arthralgia in his interphalangeal joints. His physical exam

[Early arthritis: action desired - treatment required].

Rheumatoid Arthritis (RA) is the most prevalent inflammatory joint disease in adults and shows a destructive course in most cases. The outcome of the disease - functional decline and invalidity - necessitates an early therapy. Recent studies demonstrate that the initiation of the treatment with a disease modifying antirheumatic drug (DMARD) treatment within the first three months after the onset of symptoms is crucial for sustained improvement of prognosis as well as therapeutic success and outcome. In the early stage of the disease, the criteria for the classification of Rheumatoid Arthritis

Clinical presentation of psoriatic arthritis.

Psoriatic arthritis is a spondyloarthropathy which occurs in patients with skin and/or nail psoriasis. Basing its characterization on morphological purposes, several types of arthritis have been described. Alternatively, we propose a simplified classification into three subsets, focusing on the levels of expression of cutaneous and articular elements which devise this syndrome. The first is established psoriatic arthritis which occurs in patients with evident or remittent skin and/or nail psoriasis. Its clinical spectrum consists of the five subsets classically described by Moll and Wright in

Predictors of neurovascular displacement in hands with Dupuytren's contracture.

A prospective study of hands with Dupuytren's contracture was designed to test the association of three variables, the presence of an interdigital soft tissue mass, the presence of flexion contractures at each digital joint, and the duration of contracture, with the formation of spiral nerves. 66 digits in 37 hands affected by Dupuytren's disease were examined intra-operatively. Of the 34 digits (52%) with spiral nerves, 28 had soft tissue masses (42%). The sensitivity of a soft tissue mass alone as a predictor of a spiral nerve was 59% and the specificity 75%. The presence of a flexion contra
