Femoral Shaft Fracture during Bungee Jump: A Case Report and Literature Review.

Bungee jumping is a recreational sport that is accepted to carry a level of risk. We present the case of a femoral fracture sustained during bungee jumping and examine the published literature on bungee jumping-related injuries. A previously well 31-year old female performed a 200ft bungee jump from a crane. The apparatus was performed as expected and documented on the bystander video footage. As the bungee-cord became taut for the second time, there was an audible crack with accompanying scream. A closed, neurovascularly-intact injury was sustained to her right thigh. Radiographs revealed a c

[Osteosynthesis-associated infections : Epidemiology, definition and diagnosis].

Osteosynthesis-associated infections occur in 1-5% after closed and in up to 30% after open fractures. There are three different descriptions of implant-associated infections after fracture fixation, which are crucial for the selection of the adequate treatment strategy; temporal appearance from the index surgery (early versus late), pathogenesis of the infection (exogenous, hematogenous and contiguous from an adjacent focus), duration of infection symptoms (acute versus chronic). Diagnosis of osteosynthesis-associated infection is challenging, as chronic low-grade infections often present onl

Two-Year Follow-Up Shows Gentamicin-Coated Tibial Nails Reduce Infection Rates in Open Tibial Fractures.

Introduction: Open tibial fractures carry a high risk of fracture-related infection (FRI), and prevention typically relies on early antibiotics and debridement. However, achieving optimum local antibiotic concentration remains challenging. Gentamicin-coated intramedullary nails (GCN) have been developed to prevent biofilm formation, showing short-term efficacy without interfering with fracture healing. Medium- and long-term data on GCN use are limited. This study aimed to assess the effectiveness and safety of GCN in medium-term follow-up. Methods: A prospective cohort study of patients with o

Distal femoral replacement for the treatment of periprosthetic distal femoral fractures around a total knee arthroplasty: a meta-analysis.

BACKGROUND: Periprosthetic fracture is a rare complication of arthroplasty but can have devastating consequences for the patient and presents a complex surgical challenge. Locking compression plate and retrograde intramedullary nail are both widely accepted surgical fixation techniques for distal femoral periprosthetic fractures around a total knee arthroplasty. Although there is still a need for further high-quality research into both techniques, there is even less literature concerning the use of distal femoral replacement to treat distal femoral periprosthetic fractures. Interest has been p

Is the osteosynthesis of lateral malleolar fractures by pin/cerclage effective? A retrospective study.

INTRODUCTION: Lateral malleolar fractures are present in about 90% of ankle fractures. The standard treatment is plate osteosynthesis. Although the plates are thin, the risk of cicatricial complications is not insignificant and can lead to cutaneous or even infectious complications and the latter can cause functional repercussions. We therefore aimed to evaluate a technique consisting of osteosynthesis of the lateral malleolus using two cerclages and two intramedullary pins. HYPOTHESIS: Our hypothesis is that the use of osteosynthesis by pin/cerclage of the lateral malleolus makes it possible

Locked intramedullary nailing of symptomatic metastases in the humerus.

The humerus is a common site for skeletal metastases in the adult. Surgical stabilisation of such lesions is often necessary to relieve pain and restore function. These procedures are essentially palliative and should therefore provide effective relief from pain for the remainder of the patient's life without the need for further surgical intervention. We report a retrospective analysis of 35 patients (37 nails) with symptomatic metastases in the shaft of the humerus which were treated by locked, antegrade nailing. There were 27 true fractures (73.0%) and ten painful deposits (27.0%). Relief f

How comparable is so-called standard fracture fixation with an identical implant? A prospective experience with the antegrade femoral nail in South Africa and Europe.

BACKGROUND: The utilisation and consequences of standardised operative procedures may importantly differ between different healthcare systems. This is the first investigation comparing the treatment and outcome of femoral shaft fractures stabilised with an identical implant between trauma centres in 2 continents (Europe, EU and South Africa, SA). METHODS: Following standardised introduction of the technique, the prospective, observational multicentre study enrolled 175 patients who underwent intramedullary fracture fixation using the antegrade femoral nail (AFN) for femoral shaft fractures. Focal osteolysis at the junctions of a modular stainless-steel femoral intramedullary nail.

BACKGROUND: During routine follow-up of patients treated with a three-piece stainless-steel modular femoral nail, osteolysis and periosteal reaction around the modular junctions of some of the nails were noted on radiographs. The purpose of this study was to evaluate the prevalence, etiology, and clinical relevance of these radiographic findings. METHODS: Forty-four femoral fractures or nonunions in forty-two patients were treated with a modular stainless-steel femoral intramedullary nail. Seventeen nails were excluded, leaving twenty-seven intramedullary nails in twenty-seven patients for th

A Prospective Comparative Study of the Outcome of Diaphyseal Humeral Shaft Fractures with Anterior BridgePlating versus Open Reduction Internal Fixation by Dynamic Compression Plating.

INTRODUCTION: Plate osteosynthesis has traditionally been the preferred treatment for humeral shaft fractures requiring surgical intervention. However, plating involves an open approach, necessitating careful and meticulous handling of soft tissue. As a result, the anterior bridge plating (ABP) as minimally invasive plate osteosynthesis (MIPO) is emerging as a promising technique for managing humeral shaft fractures. We performed an analysis on both groups (40 patients each), and our primary objectives compared were radiological outcome, functional outcome using UCLA shoulder score Mayo elbow s

High Spatial Resolution Chemical Imaging of Implant-Associated Infections with X-ray Excited Luminescence Chemical Imaging Through Tissue.

Microbial infections associated with implantable medical devices are a major concern in fracture fixation failure. Early diagnosis of such infection will allow successful eradication with antibiotics without an extra cost for a second surgery. Herein, we describe XELCI as a technique with high X-ray resolution, implant specificity, and chemical sensitivity to noninvasively image chemical concentrations near the surface of implanted medical devices. The devices are coated with chemically reporting surfaces. This chemically responsive surface consists of two layers coated on an implantable medic

Patient-specific anatomical alignment relative to the contralateral collodiaphyseal angle as an independent predictor of screw cut-out after proximal femoral nailing.

BACKGROUND: Screw cut-out remains a major mechanical complication after proximal femoral nailing (PFN) for intertrochanteric fractures. Traditional predictors such as tip-apex distance (TAD) and reduction quality do not account for individual femoral anatomy. This study aimed to determine whether deviation from the contralateral collodiaphyseal (CCD) angle (|Δ angle|) independently predicts screw cut-out after PFN. METHODS: A total of 354 patients (mean age 77.6 ± 12.0 years; 58% female) treated with PFN between 2015 and 2020 were retrospectively analyzed. Radiographic parameters included TAD

Factors influencing implant failure in cephalomedullary nails for proximal femoral fractures: A retrospective review.

AIMS: The use of cephalomedullary nailing (CMN) to treat extracapsular hip fractures is a well-established fixation method. Nationally, the use of CMN has increased from 67.7% in 2012 to 90.9% in 2025 for subtrochanteric fractures (National Hip Fracture Database). Although rare, implant failure is a recognised complication which poses risk to patients who are otherwise already burdened with high morbidity and mortality. The purpose of this study is to review the outcomes of CMN fixations for proximal femoral fractures, with particular focus on implant failure. We analysed patient demographics,

Cephalomedullary nailing of trochanteric fractures in geriatric patients restores natural hip geometry - a retrospective radiological analysis.

BACKGROUND: Trochanteric fractures are common in geriatric patients, and cephalomedullary nailing is widely used. However, its ability to restore and maintain native hip geometry over time remains unclear. METHODS: In this retrospective single-center study, patients aged ≥ 75 years treated with cephalomedullary nailing between June 2022 and August 2024 were included. Centrum-collum-diaphyseal (CCD) angle, femoral offset, and leg length discrepancy (LLD) were assessed on the operated and contralateral sides postoperatively and at final follow-up (minimum 6 weeks). Gait kinematics and hip disability after intertrochanteric fracture surgery: Is there a relationship?

BACKGROUND/AIM: Walking ability is key for independence in daily life in older adults following intertrochanteric fractures (ITF), yet gait impairments in this population remain poorly described. This study aimed to evaluate gait impairments in patients after ITF surgery and investigate the relationship between gait kinematics and hip-related disability. METHODS: Gait analysis was performed in 21 ITF patients at 3 and 6 months (M) post-surgery with intramedullary nails, and in 21 controls. Lower-limb kinematics and spatiotemporal gait parameters were computed. Hip disability was assessed thro
