Minimally invasive osteosynthesis techniques are very suitable for treatment of unstable fractures in children. Because of little additional soft tissue trauma, these techniques are also called biological osteosynthesis. They allow fracture treatment in children and adolescents without any harm to the growth plate. Angular deformities can be corrected due to further growth. Kirschner wires, threaded or unthreaded, elastic intramedullary rods or external fixators are most commonly used for osteosynthesis. After successful closed reduction of the fragments under x-ray amplifier control, the impl

INTRODUCTION: Only few case series of capitellar and trochlear fractures have been reported. Some of them assume that the presence of a posterior comminution (type B according to Dubberley classification) can represent a negative risk factor for treatment and prognosis respect to the type A cases (without posterior comminution). Nevertheless, how this parameter impacts the treatment and the prognosis has never been quantified before.
MATERIALS AND METHODS: All the capitellar and trochlear fractures treated from 2007 to 2015 have been retrospectively reviewed. The presence of posterior comminu

BACKGROUND: Shoulder replacement surgery is an established treatment for patients with end-stage glenohumeral osteoarthritis or rotator cuff tear arthropathy who have not improved with non-operative treatment. Different types of shoulder replacement are commonly used, but their relative benefits and risks compared versus one another and versus other treatments are uncertain. This expanded scope review is an update of a Cochrane Review first published in 2010.
OBJECTIVES: To determine the benefits and harms of shoulder replacement surgery in adults with osteoarthritis (OA) of the shoulder, inc

INTRODUCTION: complete joint fractures of the lower end of the humerus are rare and severe injuries in adults, affecting the functional prognosis of a narrow and complex joint such as the elbow. Multiple therapeutic options are available, but osteosynthesis is widely used provided that it can recover stable, mobile, and painless joint. The purpose of our study was to describe the epidemiological, clinical, radiological and therapeutic features of these fractures, as well as to evaluate functional outcomes in our patients.
METHODS: we conducted a 3-year prospective study (January 2014-December

BACKGROUND: Mobile imaging, such as viewing radiographs as text messages, is increasingly prevalent in clinical settings. The purpose of this study was to determine whether remote diagnosis of pediatric elbow fractures using smartphone technology is reliable. In addition, this study aimed to determine whether the assessment regarding the decision for operative treatment is affected by evaluation of images on a mobile device as opposed to standard picture archiving and communication system (PACS).
METHODS: Standard anteroposterior and lateral radiographs of 50 pediatric elbow trauma cases were

Posterior glenohumeral joint dislocation is an uncommon injury and is associated with bony and ligamentous disruption. It requires prompt diagnosis and early treatment to prevent acute or recurrent instability and subsequent dysfunction. Reverse Hill-Sachs lesions associated with this injury are challenging to treat, and optimal treatment is controversial. Treatment methods can be divided into those that achieve stability through muscle transfers, osteotomies, or posterior bone-block procedures (glenoid augmentation) and those that restore the sphericity of the humeral head. Joint replacement

Muscle disability is a common sequel after fracture management. Previous research has shown divergent results concerning muscle-power recovery after bone healing. This study has investigated the muscle function of wrist extensors after lateral condylar fracture in children, as evaluated by a hand-held dynamometer and compared with sex- and age-matched children. From 1999 to 2004, 20 patients (13 boys and seven girls; mean age: 9 years and 4 months) with displaced lateral condylar fracture of the humerus were treated by open reduction and internal fixation with Kirschner wires (K-wire). The dur

We present the outcome of treating complex injuries of the elbow with the "Orthofix" external fixator in patients where the condition of the soft tissues did not permit extensile surgical approach or where the internal fixation would not be stable enough to permit safe early joint mobilization postoperatively. Eight patients were treated, three with supra-intercondylar fracture in the presence of marked osteoporosis and five with fracture-dislocation of the elbow and excessive soft tissue impairment. The fixator was applied for a mean of 8.5 weeks (5-13 weeks). Great care was taken to use the

The authors describe a patient who presented from birth on a severe involvement of connective tissues with pathological fractures, lack of auricular cartilage, hyperlaxity of fingers and cutis laxa with deep folds, all suggestive of derangements of collagen and elastin. Hypothermia at 24 hours of age should have already indicated the possibility of Menkes' syndrome. From the 3rd month on, the patient presents a neurological deterioration and a myoclonic epilepsy which is resistant to treatment. Craniocerebral tomodensitometry revealed, with time, a cerebral atrophy and subdural hematomas. Angi

From 1972 to 1983 105 patients were admitted with a dislocated supracondylar fracture of the humerus. The average follow-up was five years. In general the functional results of treatment were good. The anatomical results were less satisfactory, and most times concerned varus deformities of the elbow, which did not influence the elbow function. Lesions of arteries and nerves only occurred in seriously dislocated fractures. The isolated nerve injuries were reversible. Two patients developed a Volkmann's ischemic contracture; their case history is discussed. In 24 cases a second reduction was per

BACKGROUND: High-energy traumatic fractures represent a challenge for orthopaedic surgeons because there are a great variety of morphologic patterns and associated injuries1. Although the incidence is higher in developing countries, these fractures pose a major financial burden all over the world because of their considerable hospital length of stay, time away from work, rate of failure to return to work, complications, and cost of treatment2-4. Since the fracture patterns are so variable, some cases may have a lack of available specific osteosynthesis implants, despite recent advancements in

BACKGROUND: Previous studies have demonstrated that varus alignment of the elbow has been associated with lateral condyle fractures. However, for other pediatric and adolescent elbow pathologies, the influence of upper extremity alignment has not been investigated. This study aimed to compare elbow alignment (quantified using Baumann's angle) among patients with medial epicondyle humeral fractures and capitellum osteochondritis dissecans (OCD) and age- and sex-matched control patients.
METHODS: Pediatric and adolescent patients (ages: 5-18 years, inclusive) treated at a tertiary care hospital…
