Outcomes of Endovascular Treatment for Infectious Thoracic Aortic Diseases.

PURPOSE: To investigate the effectiveness and safety of thoracic endovascular aortic repair (TEVAR) for infectious aortic diseases. MATERIALS AND METHODS: Patients who underwent TEVAR for infectious aortic diseases including infected thoracic aortic aneurysm, aortobronchial fistula (ABF), and aortoenteric fistula from December 2011 to October 2022 at 4 institutions were retrospectively studied. The primary outcome of the study was overall survival, whereas the secondary outcome was comprehensive adverse events. Comprehensive adverse events were defined as a combination of deaths, aortic event

VFA/TAMAI predicts aggravated organ damage after central line removal due to catheter-related bloodstream infection in patients with enterocutaneous fistula.

BACKGROUND: Enterocutaneous fistula (ECF) patients face complex metabolic and immunological perturbations, creating "nutri-inflammatory exhaustion" that increases vulnerability to catheter-related bloodstream infections (CRBSI). Body composition analysis may predict organ damage following CRBSI treatment in these high-risk patients. METHODS: This retrospective cohort study analyzed 146 ECF patients with confirmed CRBSI from two tertiary hospitals (November 2019-May 2024). CT-based body composition analysis calculated visceral fat area to total abdominal muscle area index ratio (VFA/TAMAI). Th

Hypoxia-pretreated exosomes promote enterocutaneous fistula healing via regulating inflammation and tissue repair.

BACKGROUND: Enterocutaneous fistula (ECF) is an abnormal pathological passage. This study aims to investigate the role and mechanism of action of gelatin sponges loaded with hypoxic exosomes (GS-Hypo-Exos) in the treatment of enterocutaneous fistulas. METHODS: A rat ECF model was established and subjected to intervention therapy, with the following groups: control group, gelatin sponge loaded with PBS group (GS-PBS), gelatin sponge loaded with hypoxia-exosomes group (GS-Hypo-Exos), and gelatin sponge loaded with normoxic exosomes group (GS-Nor-Exos). After 21 days of intervention, rat serum a

A case of fistulating disease involving the terminal ileum and sigmoid colon-A video vignette.

AIMS: This video presents a case involving complex inflammatory bowel disease (IBD) with fistulation of the terminal ileum and sigmoid colon. We aim to discuss an overview of the decision-making involved in the pre-operative, intra-operative and post-operative stages, while highlighting the benefits of utilising a robotic platform. METHODS: A 29-year-old patient with Crohn's disease underwent a robotic ileo-colic resection without anastomosis and double barrel ileostomy formation. A multidisciplinary approach was taken with regard to surgical planning. Pre-operative dietician and gastroentero

How to isolate a high output enteroatmospheric fistula in the open abdomen with negative pressure therapy: an institution's step by step guide to the VAC donut.

There is multiple evidence to suggest that isolation techniques of high output enteroatmospheric fistulas (EAF) in open abdomens can be advantageous in controlling fistula effluent while allowing time for abdominal wall to granulate. The large loss of proteins, electrolytes and fluid, and the distressing nature of the open abdomen for both patients and doctors, make managing these EAFs a clinical challenge. We present our experience with a high output mucosal protruding EAF and the creation of a 'VAC donut' allowing a successful diversion of the enteric content whilst promoting granulation of

Acute diverticulitis: Key features for guiding clinical management.

Diverticular disease of the colon and small bowel is an important cause of pathology leading to emergency department visits and urgent gastrointestinal surgery. CT is a highly sensitive and specific modality for the diagnosis of acute diverticulitis and its complications as well as for the exclusion of alternate causes of pathology. Ultrasound, MRI and virtual CT colonoscopy have important adjunct roles for screening and workup of complications in specific patient populations. While diverticular disease most commonly involves the descending and sigmoid colon, it can also affect the proximal co

Cholecystocolonic fistula.

Cholecystoenteric fistulas are a complication of biliary disease, having an estimated incidence in the range of 0.1 - 0.5 percent. Roughly 8-27 percent of these are of the cholecystocolonic type,1 occurring most commonly at the hepatic flexure. An 82-year-old male presented to our tertiary hospital emergency department and was diagnosed with a cholecystocolonic fistula (CCF) by CT imaging. Surgical evaluation confirmed the CT diagnosis with subsequent removal of the fistula and gallbladder, which was curative.

Incidental vesicocolic fistula on routine bone scintigraphy: Value of additional delayed images and direct radionuclide cystography.

An unexpected vesicocolic fistula can be detected incidentally on routine bone scintigraphy. A 55-year-old man who had a radical colectomy for carcinoma of the sigmoid colon 1 year previously underwent bone scintigraphy to evaluate bone metastasis. Whole-body images showed an abnormal accumulation of radioactivity in the right lower quadrant of the abdomen, but the radioactivity did not precisely define a structure. Additional delayed images obtained after 15 and 24 hours of the initial image localized a vesicocolic fistula. Subsequent radionuclide cystography confirmed leakage of the radioact

Direct radionuclide cystography imaging in colovesical fistula due to inguinal hernia operation complication.

Colovesical fistula is an abnormal connection between the enteric and urinary systems, usually sigmoid colon, caused by various conditions. One cause of colovesical fistula is iatrogenic injury, such as induced by inguinal hernia surgery. We present a case of colovesical fistula. A 57-year-old male was admitted to a local hospital with complaints of dysuria and pneumaturia. He had a past history of total extraperitoneal laparoscopic inguinal hernia repair operation 7 years previously for bilateral inguinal hernia. The case was assessed with radiologic and scintigraphic techniques. Radiologic t

Complications of laparoscopic hysterectomy: the Monash experience.

A retrospective review of medical records was performed to assess the incidence and type of significant complications encountered during laparoscopic hysterectomy Two hundred and sixty-five consecutive patients were reviewed between the years 1994 and August 2001. Two hundred and thirty-two laparoscopic vaginal hysterectomies and 33 total laparoscopic hysterectomies were performed. The operations were performed at Monash Medical Centre, a Melbourne tertiary public hospital, and two Melbourne private hospitals, by three surgeons. Ten significant complications occurred. There were two cases of u
