Firearm legislation, gun violence, and mortality in children and young adults: A retrospective cohort study of 27,566 children in the USA.

BACKGROUND: Firearm violence results in the death of thousands of children in the US annually. The effects of firearm legislation on gun vio … study. Most were adolescents aged 15-19 (87.3%), male (89.7%), and black (53.7%). The proportion of accidental injuries increased relative to state law leniency (R2 = 0.90), with highest percentage in lenient states (33.2%) compared to strict (16.7%). The proportion of suicide attempts were higher in states with lenient laws (4.4%) compared to strict (1.3%). Accident …

School Nurses on the Front Lines of Healthcare: Red Flags and Red Herrings: Improving the Recognition of Bruises and Burns Associated With Physical Abuse in School-Age Children.

Through a series of case scenarios, this article describes the assessment and management of suspected physical child abuse presenting as bruises and burns. Although it is not uncommon for school-age children to have accidental injuries, recognizing patterns associated with physical child abuse and understanding red flags for abuse is vital. Failure to recognize injury patterns suspicious for physical child abuse, and thus failure to intervene appropriately, may leave children at risk for more serious injury or death.

[Adequate management of stab and gunshot wounds].

BACKGROUND: It is difficult to verify the treatment of stab and gunshot wounds with prospective randomized studies. That is why the results of observational studies are so important. MATERIAL: From 1 January 1989 to 31 December 1998, we saw 74 patients because of stab (64) and gunshot (ten) wounds. RESULTS: Most of the patients, whose injuries were caused mainly for criminal reasons (criminal 54, autoaggressive 14, accidental 4, unknown 2), came to hospital on weekends. We saw 38 abdominal, 23 thoracic, and 13 mixed injuries. On average, 3.8 h passed between the time of injury and getting fi

Perforating eye injuries in KwaZulu.

Data on 100 consecutive perforating eye injuries treated at Edendale Hospital, Pietermaritzburg, were collected in order to evaluate some of the features of this condition seen in KwaZulu, and also to assess the outcome of current management. Fifty-six of the injuries were due to assault and 44 were accidental. Fifty injuries involved the anterior segment only, and 50 involved the posterior segment. Only 6 eyes achieved a final visual acuity of 6/12 or better, and 44 eyes ended up with no light perception. Factors contributing to the poor visual results in this Third-World situation include th

Relationships among plasma cortisol, adrenocorticotrophin, and severity of injury in recently injured patients.

When samples were taken within 2 hr of accidental injury, plasma cortisol levels increased with injury severity score (ISS) in patients with minor and moderate injuries (ISS, 1-12) but decreased at higher ISS. Radioimmunoassay of adrenocorticotrophin (ACTH) showed very variable plasma concentrations at all ISS ranges. Like cortisol, ACTH increased with ISS up to a score of about 13, but it thereafter plateaued, and in patients with severe injuries plasma cortisol fell in relation to ACTH as well as in absolute terms. This suggested that although ACTH secretion was generally far from maximal th

Dental and maxillofacial skeletal injuries seen at the University of Otago School of Dentistry, New Zealand 2000-2004.

An epidemiological study of dental and facial trauma injuries was performed on patient presentations to the University of Otago School of Dentistry during the period 2000-2004. A total of 1287 patients were seen for dental injuries with 3473 tooth injuries. The mean age was 17.48 +/- 13.13 years (range: 2-86), the highest number of injuries occurred in the 16-25 year group with a male to female ratio of 2.01:1. Uncomplicated crown fractures were the most common injury that required treatment and the variables of age, gender, tooth type, type of injury, cause of injury, location where injury oc

Estimating the Relevance of Historical Red Flags in the Diagnosis of Abusive Head Trauma.

OBJECTIVE: To replicate the previously published finding that the absence of a history of trauma in a child with obvious traumatic head injuries demonstrates high specificity and high positive predictive value (PPV) for abusive head trauma. STUDY DESIGN: This was a secondary analysis of a deidentified, cross-sectional dataset containing prospective data on 346 young children with acute head injury hospitalized for intensive care across 18 sites between 2010 and 2013, to estimate the diagnostic relevance of a caregiver's specific denial of any trauma, changing history of accidental trauma, or

Outcome of survivors of accidental deep hypothermia and circulatory arrest treated with extracorporeal blood warming.

BACKGROUND: Cardiopulmonary bypass has been used to rewarm victims of accidental deep hypothermia. Unlike other rewarming techniques, it restores organ perfusion immediately in patients with inadequate circulation. This study evaluated the long-term outcome of survivors of accidental deep hypothermia with circulatory arrest who had been rewarmed with cardiopulmonary bypass. METHODS: Deep hypothermia (core temperature, <28 degrees C) with circulatory arrest was found in 46 of 234 patients with accidental hypothermia. In 32 of the 46 patients, rewarming with cardiopulmonary bypass was attempted

Patterns of structural head injury in children younger than 3 years: a ten-year review of 519 patients.

BACKGROUND: Abusive head injury is a major contributor to morbidity and mortality in infants and toddlers, but data comparing patterns of injury in corroborated accidental trauma and confessed child abuse are scarce. METHODS: This is a retrospective review of head injuries with abnormal neuroimaging in children younger than 3 years during a 10-year period in Auckland, New Zealand. Histories were assumed to be true. Results were analyzed for incongruity then compared with data on confessed abuse and corroborated accidental injury. RESULTS: Five hundred nineteen cases were analyzed. Most cases

Anomalous innominate artery complicating tracheal surgical procedures.

OBJECTIVE: To highlight the importance of imaging in reducing an accidental injury to the anomalous brachiocephalic trunk and its branches during tracheal surgery. CASE REPORT: This paper reports two cases of accidental injury to the great vessels in the neck during tracheal surgery. The first incident occurred during a repeat tracheostomy, when the right common carotid artery was injured. On reviewing the computed tomography images, the bifurcation of the brachiocephalic artery was seen to the left of the midline, and the right common carotid artery was adherent just below the tracheostomy s

A Practical Diagnostic Approach to Non-Drowning Asphyxia in Animals: Forensic Pathology and Biomarkers.

The term asphyxia refers to a disruption in brain function due to rapid and persistent cerebral hypoxia or anoxia as a consequence of accidental or non-accidental injury. Considering the different mechanisms that may determine asphyxiation, such injuries can be referred to different categories: strangulation (death by hanging, ligature or manual strangulation), suffocation (smothering, choking, confined spaces and vitiated atmosphere), mechanical asphyxia (positional and traumatic asphyxia) and drowning (submersion or immersion in liquid). In both human and veterinary forensic practice, fatal

Widening of the pericerebral space in infants - consultative problems.

The study was an attempt to characterize symptoms and factors suggesting a traumatic cause of pericerebral space widening in infants. This heterogeneous group of disorders, rarely observed in forensic pathological practice, poses consultative problems, as it may be difficult to distinguish between spontaneous disease-related changes and traumatic complications, and differentiate accidental from non-accidental trauma. The study is based on the records of two cases submitted for examination in order to determine the degree of health impairment. In both cases, a female infant was indicated as the

Intentional and unintentional abuse of infants and children.

Intentional abuse of an infant is usually apparent. External injuries such as burns, bites, and bruises, as well as the skeletal changes of fractures, frequently multiple and in various stages of healing, are obviously intentionally inflicted, and are impossible accidental injuries for the nonambulatory young infant. Similar injuries in young children may be more difficult to evaluate because of their fear of and intimidation by the person who inflicted the injury. Fortunately, the high degree of suspicion regarding the nature of the injury permits the physician to make his accusations with le…

A review of chemical eye burns referred to the Blodgett Regional Poison Center.

We reviewed a random sampling of 500 cases of chemical eye exposures referred to the Blodgett Regional Poison Center from July 1, 1986 to December 31, 1986. While some injuries were encountered in laboratories and industrial plants, the great majority originated in the home (84.4%) and involved commonly used household products. Most exposures were acute and reported within minutes to 1 hr. Cases predominantly involved children suffering accidental exposure. The most common symptoms were eye discomfort (56.2%) and redness (26.4%), but many patients were completely asymptomatic (11.4%). The repo

[Topical mitomycin C treatment of a benign oesophageal stricture following caustic ingestion in a four-year-old boy].

Accidental caustic ingestion is a common problem in young children at the age of 1 to 4 years. In cases of circular injuries of the oesophagus subsequent strictures can arise. Endoscopic balloon dilatation is the commonly used intervention but does not always lead to permanent improvement. We report the successful use of mitomycin C in a young boy in whom we could achieve ongoing relief of dysphagia after the unsuccessful long-term use of frequent endoscopic dilatations.

Survival following accidental scarf strangulation.

Injury or death by strangulation, unless otherwise explained, is almost always homicidal. Accidental strangulation may occur but only very rarely. We present such a case of accidental strangulation and survival in a motorbike pillion rider. A long scarf (dupatta) clad woman, sitting at the back of a two wheeler motorbike, fell after her long scarf got caught in the back wheel. The lady was first taken to a local clinic and then later was referred to a hospital for a suspected spine injury where she made an uneventful recovery. This case report exposes the precarious position of women pillion r

Photoacoustic-Guided Laparoscopic and Open Hysterectomy Procedures Demonstrated With Human Cadavers.

Hysterectomy (i.e., surgical removal of the uterus) requires severing the main blood supply to the uterus (i.e., the uterine arteries) while preserving the nearby, often overlapping, ureters. In this paper, we investigate dual-wavelength and audiovisual photoacoustic imaging-based approaches to visualize and differentiate the ureter from the uterine artery and to provide the real-time information needed to avoid accidental ureteral injuries during hysterectomies. Dual-wavelength 690/750 nm photoacoustic imaging was implemented during laparoscopic and open hysterectomies performed on human cada
