Are benzodiazepines still the medication of choice for patients with panic disorder with or without agoraphobia?

OBJECTIVE: Recently, pharmacological treatment guidelines for panic disorder have changed as newer treatment options have become available. The authors examined how the use of psychotropic drugs has shifted over the course of 10 years to determine if prescribing patterns have changed to reflect these revised treatment guidelines. METHOD: A total of 443 patients with panic disorder were enrolled in the Harvard/Brown Anxiety Research Project, a prospective longitudinal study of anxiety disorders. These patients were interviewed over the course of 10 years to examine their use of psychotropic me

Relationship between perfectionism, personality disorders and agoraphobia in patients with panic disorder.

OBJECTIVE: In earlier reports, we found that perfectionism might be involved in the development and/or maintenance of agoraphobia in panic disorder. The present report extends this work by examining the relationship between perfectionism and comorbidity with personality disorders in panic disorder patients with agoraphobia (PDA) and those without agoraphobia (PD). METHOD: We examined comorbidity of personality disorders by Structured Clinical Interview for DSM-III-R Personality Disorders (SCID-II) and assessed perfectionism using multidimensional perfectionism scale in 56 PDA and 42 PD patien…

Prefrontal enlargement of CSF spaces in agoraphobia: a qualitative CT-scan study.

1. This CT study was designed to assess brain morphology in 21 patients with agoraphobia and 21 normal control subjects matched for age and sex. 2. Internal and external CSF spaces were evaluated by qualitative assessment on a 3-point scale (normal, questionable, abnormal). 3. Patients showed bilateral enlargement of prefrontal CSF spaces (p < .05). The rating abnormal" was given in the left hemisphere to 6 (28.6%) of the patients, to 4 (19%) of the patients in the right hemisphere, but to none (0%) of the normal controls. 4. These findings suggest that alterations in brain morphology are invo…

Long-term maintenance and discontinuation of imipramine therapy in panic disorder with agoraphobia.

BACKGROUND: There has been little systematic work done regarding the long-term treatment of panic disorders. The aim of the present study was to assess the 12-month cumulative risk of relapse specifically due to discontinuation of imipramine and to test the hypothesis that maintenance treatment with imipramine protects patients with panic disorder and agoraphobia from such reversals. METHOD: Following an acute-phase open trial with imipramine (2.25 mg/kg per day) involving 110 patients for 6 months, the 56 patients who were in stable remission, did not require additional treatment, and consen…

Life events and panic disorder/agoraphobia: a comparison at two time periods.

Semistructured interviews of 187 patients with a DSM-III diagnosis of panic disorder or agoraphobia with panic attacks were conducted an average of 12 years after onset of the disorder. Interviews solicited life events that occurred in the year before onset and in the year preceding index evaluation. The number and type of life events were found to be similar at onset and at the later evaluation. However, the results showed that a tendency for increased reporting of life events before onset in a subgroup of patients was counterbalanced by a tendency for decreased reporting in another subgroup.…

Clinical experiments in maintenance and discontinuation of imipramine therapy in panic disorder with agoraphobia.

Several issues remain to be ascertained beyond the acute response to imipramine hydrochloride in patients with panic disorder. Study 1 consisted of a prospective, systematic characterization of half-dose 12-month maintenance in patients with panic disorder with agoraphobia who had shown marked and stable response to 6 months of acute-phase treatment with imipramine. Study 2 assessed the 6-month cumulative relapse rate following discontinuation of acute-phase imipramine treatment in a comparable sample of patients. The same assessment battery was used in both studies, and the integrity of exper…

The possible function of positive reinforcement in home-bound agoraphobia: a case study.

We conducted an uncontrolled case study (ABA design) based upon the hypothesis that the behavior of a home-bound agoraphobic is at least partially maintained by positive reinforcement in the home and that a disruption of access to home-based reinforcement would lead to an increased frequency of out-of-the-home behavior. Data concerning the types and amounts of behavior engaged in by the subject within the confines of her home and yard were gathered during a 30-day base line period. In addition, potential reinforcers in the home were identified by a survey schedule and by self-report of time al

Mental health locus of control in agoraphobia and depression: a longitudinal study of inpatients.

This study represents a longitudinal extension for 75 inpatients of a previous cross-sectional study, wherein agoraphobic inpatients, both d … t. Both a vulnerability model and a symptom model of the relation between externality to chance and agoraphobia received some support. On the other hand, reductions in internality during treatment predicted posttreatment relapse. Locus of control variables were also associated with effectiveness of treatment and posttreatment course, both with respect to fear …

Is agoraphobia harder to treat? A comparison of agoraphobics' and simple phobics' response to treatment.

Agoraphobia is reputed to be more difficult to treat than simple phobia. In a test of this supposition, 38 agoraphobics and 19 simple phobics were each given 10 sessions of graduated in vivo exposure. They were assessed before and after therapy using a behavioral avoidance test, behavioral diaries, and self-report measures. Analysis of covariance revealed unequivocal posttest differences only on self-assessed disability level; agoraphobics had changed less in their report of global disability immediately following treatment but not at follow-up. Sixty-eight percent of the simple phobics showed…

Breathing retraining, exposure and a combination of both, in the treatment of panic disorder with agoraphobia.

The present study investigates the differential effectiveness of three treatment packages for agoraphobia. Patients suffering from panic disorder with agoraphobia (DSM-III-R) received one of three treatments: Breathing Retraining with Cognitive Restructuring (BRCR), graded self-exposure in vivo (EXP), or a combination of BRCR and EXP. Treatments consisted of 8 sessions. Assessment consisted of self-report measures for panic, phobic anxiety and avoidance, depression, general anxiety, somatic complaints and fear of body sensations, and of two respiratory measures (respiratory rate and alveolar p…

Drugs and psychological treatments for agoraphobia/panic and obsessive-compulsive disorders: a review.

In the short term, both antidepressants and exposure therapy usually improve agoraphobia/panic (AP) and obsessive-compulsive (OC) disorders and are accepted by most patients; psychological methods omitting exposure are not consistently helpful. Antidepressants have a broad-spectrum rather than specific anti-agoraphobia/panic or anti-obsessive-compulsive action. For long-term efficacy, there is good evidence for the value of exposure, but none for drugs. Because of relapse on ceasing drugs, and their side-effects, medication is less useful as the first line of treatment for chronic agoraphobia/…

Agoraphobia: relative and combined effectiveness of therapist-assisted in vivo exposure and imipramine.

Sixty-two chronically agoraphobic patients completed a controlled study to assess the effects of 1) imipramine up to 200 mg/day (mean = 130 mg/day), 2) 12 weekly therapist-assisted in vivo exposure sessions (flooding), and 3) imipramine plus flooding. The control group received systematic therapeutic instructions for self-directed in vivo exposure (programmed practice). Clinical measures of global severity, phobia, panic, anxiety, depression, and behavioral performance tests were administered before treatment and at Weeks 4, 8, and 12 of treatment. Results revealed significant improvement in a

Clonazepam in the treatment of panic disorder and agoraphobia: a one-year follow-up.

Fifty patients with panic disorder or agoraphobia with panic attacks with treated with clonazepam, a high potency benzodiazepine. The authors report on the outcome of these patients after an average follow-up interval of 1 year, suggesting that clonazepam is a safe, effective, and easy to administer medication in a relatively treatment-refractory population of nondepressed patients with panic disorder and agoraphobia.

A comparison of subliminal, supraliminal and faded phobic cine-films in the treatment of agoraphobia.

Thirty-two agoraphobic patients were randomly allocated to four groups and treated by repeated exposure to cine-films at twice weekly intervals for three weeks. Three of the groups saw the same cine-film, comprising a range of agoraphobic scenes, and a control group saw a potter working on his wheel. The three groups seeing the phobic cine-film included one who viewed it at an illumination level below the visual threshold (subliminal group), one seeing it under normal conditions (supraliminal group) and one which underwent graduated exposure from subliminal to supraliminal viewing levels as th

Frontal cortex absolute beta power measurement in Panic Disorder with Agoraphobia patients.

UNLABELLED: Panic disorder patients are hypervigilant to danger cues and highly sensitive to unpredictable aversive events, what leads to anticipatory anxiety, that is one key component of the disorder maintenance. Prefrontal cortex seems to be involved in these processes and beta band activity may be related to the involvement of top-down processing, whose function is supposed to be disrupted in pathological anxiety. The objective of this study was to measure frontal absolute beta-power (ABP) with qEEG in panic disorder and agoraphobia (PDA) patients compared to healthy controls. METHODS: qE…
