Reappraisal of alcoholic myopathy. Clinical and biopsy study on chronic alcoholics without muscle weakness or wasting.

To determine if alcoholic neuropathy which causes denervation of the distal muscles of chronic alcoholics also produces a subclinical myopathy of their proximal muscles, we studied 11 chronic alcoholics who had no muscular weakness or wasting. Six patients demonstrated distal hyporeflexic (ankle jerks) sensory neuropathy on clinical examination. Four patients, one of whom was asymptomatic, had slow peroneal motor nerve conduction velocities. Patterns of neuropathy were present in the electromyograms of the proximal muscles of two patients. Muscle biopsy studies with enzyme histochemistry indic…

Alcoholics anonymous and recovery in Türkiye: A qualitative study in the context of Social Identity Theory.

Alcoholics anonymous (AA) groups play a central role in facilitating the transition from an 'addicted' identity to a 'recovering' identity; however, empirical research on how this identity transformation is socially constructed, maintained and questioned within group contexts is limited. Drawing on Social Identity Theory (SIT), this qualitative study examines how AA group dynamics shape members' sense of belonging, processes of identity reconstruction and experiences of recovery while also illuminating the vulnerabilities that may destabilize recovery identity. The study involved in-depth inte…

Forgiveness and Adult Children of Alcoholics-Related Problems Among College Students as Mediated by Psychological Distress.

Adult Children of Alcoholics (ACOAs) face myriad challenges related to experiencing negative life events resulting from their parents' problematic drinking. ACOAs are at an elevated risk of developing alcohol-related problems and psychological distress, which may influence their overall well-being. The current, cross-sectional study examined the association of forgiveness with both ACOA-related problems and alcohol-related problems among college students raised by parents with a history of problematic drinking (N = 348), as mediated by psychological distress (i.e., depression, anxiety, and str…

Subacute encephalopathy with seizures in alcoholics (SESA) syndrome: a subtype of focal non-convulsive status epilepticus.

Focal status epilepticus is often associated with intracranial infections, neoplasms or vascular aetiologies. Subacute encephalopathy with seizures in alcoholics (SESA) syndrome is a clinically under-recognised disorder characterised by encephalopathy in the context of seizure and specific focal EEG abnormalities. Our case involved a male in his 50s with a history of alcohol use disorder, who presented with left-sided twitching and encephalopathy, found to be in a focal status. An EEG revealed abnormal discharges in the right frontoparietal region. MRI of the brain displayed signal changes in …

Resting state synchrony in long-term abstinent alcoholics: Effects of a current major depressive disorder diagnosis.

Alcoholism is characterized by a lack of control over an impulsive and compulsive drive toward excessive alcohol consumption despite significant negative consequences; our previous work demonstrated that successful abstinence is characterized by decreased resting-state synchrony (RSS) as measured with functional magnetic resonance imaging (fMRI), within appetitive drive networks and increased RSS in emotion regulation and inhibitory executive control networks. Our hypothesis is that LTAA (Long-Term Abstinent Alcoholics) with a current major depressive disorder (MDD) drank primarily to deal wit…

The Distribution of Liver Steatosis, Fibrosis, Steatohepatitis and Inflammation Activity in Alcoholics According to FibroMax Test.

BACKGROUND: The diagnosis of alcoholic liver diseases is based on the history of alcohol abuse, clinical evidence of liver disease and laboratory abnormalities. The new non-invasive biomarkers have higher sensitivity to quantify and predict steatosis and fibrosis than ultrasonography. OBJECTIVES: The aim of this study was to evaluate the prevalence of liver diseases in alcoholics by means of FibroMax. MATERIAL AND METHODS: A total of 142 consecutive alcoholics were enrolled in the study. The prevalence of liver diseases was assayed by means of non-invasive biomarkers: fibrosis by FibroTest, …

The prevalence of sleep disturbance in alcoholics admitted for treatment: a target for chronic disease management.

Prolonged and heavy use of alcohol is associated with persistent sleep disturbances. Objective and subjective measures of sleep quantity and quality were collected on 164 individuals undergoing detoxification. A high prevalence of sleep disturbance was found in this sample. Sleep quality improved by week 4 but continued to be altered, signaling a target area for recovery management. This study supports the high prevalence of sleep disturbance in individuals undergoing alcohol treatment. Health promotion strategies in an addiction recovery model should address quality-of-life enhancements for i

Long-term γ-hydroxybutyric acid (GHB) and disulfiram combination therapy in GHB treatment-resistant chronic alcoholics.

Leading Italian studies support the use of γ-hydroxybutyric acid (GHB), not only in the treatment of the alcohol withdrawal syndrome, but also in maintaining alcohol abstinence. GHB gives a better result than naltrexone and disulfiram in maintaining abstinence, and it has a better effect on craving than placebo or disulfiram. The problem is that about 30-40% of alcoholics are non-responders to GHB therapy. In our clinical practice, we speculate that by combining disulfiram with GHB treatment we may be able to achieve a kind of 'antagonist' effect by using the 'psychological threat' of disulfir…

CONCLUSION: Patient follow-up is problematic due to the large number of dropouts among alcoholics. Early screening in search for factors predictive of unsuccessful detoxification (long duration of alcohol consumption, multiple addiction) would be helpful in elaborating appropriate pluridisciplinary management.

Regional cerebral blood flow measured by the resting and vascular reserve (RVR) method in chronic alcoholics.

It has been reported that cerebral blood flow is decreased in chronic alcoholics without neurological diseases. We performed SPECT by the resting and vascular reserve (RVR) method in 11 chronic alcoholics (mean age 59.1 +/- 10.4, all males) and 11 age-matched healthy controls (mean age 58.8 +/- 10.1, all males). The resting and vascular reserve (RVR) method is a consecutive (99m)Tc-ECD SPECT study before and after acetazolamide administration with adjunctive radionuclide angiography, which is easy to perform for a routine clinical study and quite useful for evaluating regional cerebral blood f…

The relationship of cognitive functioning to amount of recent and lifetime alcohol consumption in outpatient alcoholics.

Previous investigations of the relationship between drinking patterns and cognitive functioning have generally studied severely alcoholic patients, in whom the neurocognitive effects of alcohol consumption can be obscured by other medical or psychosocial factors. In the present study, cognitive functioning was examined after a minimum of 4 days of abstinence in 69 mildly to moderately alcohol-dependent outpatients without comorbid psychiatric, neurologic, or systemic medical illness. Circumscribed decrements in reaction time and verbal memory were associated with higher amounts of alcohol cons

Help-seeking for alcohol-related problems: social contexts surrounding entry into alcoholism treatment or Alcoholics Anonymous.

OBJECTIVE: The role of problem drinkers' social contexts in help-seeking patterns was investigated using recent entrants into outpatient alcoholism treatment or Alcoholics Anonymous (AA). Untreated, active problem drinkers served as controls. METHOD: Subjects (N = 45, with 15 per group: 62% male) were recruited from the community. Habitual drinking practices, social network characteristics, social support, help-seeking barriers and incentives, and event occurrences during the 2 years prior to help-seeking were assessed during structured interviews. Collaterals verified subject reports. RESUL…

Previous studies suggest that the presence of liver disease may exacerbate thiamine deficiency in alcoholics. A sustained loss of transketolase activity in brain could result in disruption of pentose shunt activity and concomitant reductions in reducing equivalents and lipid metabolism within the cell. [Use of the F wave for detection of preclinical changes in the peripheral nerves in alcoholics].

Orthodromic conduction in motor fibres and F wave were analysed in the median, ulnar, peroneal and tibial nerves in groups of 30 subjects: one control and one comprising patients with chronic alcoholism without detectable clinically damage to the peripheral nervous system. Abnormalities were demonstrated in both these analysed parameters, but they were more pronounced and were present in a greater proportion of cases in the assessment of the F wave. In the light of these results the determination of this response may be a useful supplementary method in the investigation of peripheral nerves, e

Morphologic spectrum of liver diseases among chronic alcoholics. A comparison between Tokyo, Japan and Cincinnati, U.S.A.

To deliniate the histopathological features of liver diseases seen in Japanese alcoholics, 130 Japanese alcoholic patients were studied in comparison with 238 American alcoholic patients. In Japan female alcoholic patients were extremely rare. The male to female ratio was 127 to 3 in Japan and 152 to 89 in U.S.A. Although all aspects of alcoholic liver disease did exist in Japan, typical cases of acute alcoholic hepatitis with alcholic hyalins were remarkably smaller in number (9.2% in Japan versus 39.5% in the U.S.A.). The severity based on histopathological findings was also less in the Japa…
