Diffusion-weighted MRI in transient global amnesia and its diagnostic implications.

OBJECTIVE: To analyze how the evidence of hippocampal diffusion-weighted imaging (DWI) lesions may support the clinical diagnosis of transient global amnesia (TGA). METHODS: In this retrospective observational study, 390 consecutive patients with isolated TGA were analyzed, who were evaluated at our institution between July 1999 and August 2018. The size, location, and number of lesions and time-dependent lesion detectability were examined. The incidence of DWI lesions was reviewed with regard to different levels of clinical diagnostic certainty upon presentation to the emergency department.

Bilateral hippocampal abnormalities in magnetic resonance imaging in transient global amnesia.

Transient global amnesia (TGA) is an isolated amnesic syndrome with normal neurologic examination. Patients remain alert and communicative with no loss of personal identity; however, they experience striking loss of memory for recent events and an impaired ability to retain new information. We report a case of TGA in a patient with acute ischemia in bilateral hippocampal abnormalities. This provides evidence in support of an ischemic hypothesis as the possible etiology of TGA.

An acute tiny left putamenal lesion presenting with transient global amnesia.

INTRODUCTION: Transient global amnesia (TGA) is one of the most striking syndromes in clinical neurology. Brain lesions producing TGA have been reported in the hippocampus, amygdala, thalamus, caudate nucleus, and neocortex. CASE REPORT: We report a 63-year-old woman presenting with TGA associated with a localized lesion in the left putamen. CONCLUSIONS: This case suggests a potential role of the putamen in human memory processing.

Transient global amnesia after breathing hyperoxic mixtures in otherwise regular dives.

The diagnosis of decompression sickness may be difficult, particularly when patients present with atypical clinical manifestations after dives going on uneventfully and with full adherence to safety parameters. The case reports involve two divers who presented with transient global amnesia (TGA) immediately after surfacing from otherwise regular dives during which they had been breathing hyperoxic mixtures (Nitrox). A wide spectrum of symptoms of neural dysfunction following hyperoxic mixtures have been reported, including cases of memory impairment related to previous generalized convulsions.

[Transient global amnesia: indications for a syndrome involving cerebral venous stasis].

In 3 patients, a woman aged 58 and 2 men aged 61 and 58, respectively, who presented to the Emergency Clinic with sudden antegrade and retrograde amnesia, the diagnosis 'transient global amnesia' (TGA) was made. In the first and the last patient the TGA was preceded by a Valsalva-like manoeuvre, i.e. vomiting and tying the shoelaces while bending over and holding his breath, respectively. Until recently, TGA was assumed to have three possible causes: arterial ischaemia (transient ischaemic attack; TIA), migraine and epilepsy. A fourth and more recent hypothesis is cerebral venous stasis. This

Right frontal cortex hypometabolism in transient global amnesia. A PET study.

A 60-year-old lady with previous hypertension was studied with PET in the acute (early recovery) phase of an otherwise typical episode of transient global amnesia (TGA). Follow-up over > 1 year was uneventful, and delayed CT scans and MRI showed no brain damage. No medical cause was disclosed despite extensive work-up. The PET study revealed a matched reduction in cerebral blood flow and oxygen consumption over the entire lateral frontal cortex on the right side, with an associated, less significant reduction in ipsilateral thalamic and lentiform nucleus metabolism, but sparing the hippocampal

Transient global amnesia following cerebral angiography with non-ionic contrast medium.

Transient global amnesia (TGA) is an uncommon syndrome of recent memory deficit and inability to learn new data, usually resolving within 24 h. Two cases following use of non-ionic contrast media in cerebral angiography are described. The neuroanatomy of memory is reviewed. Possible aetiologies of TGA in relation to cerebral angiography include ischaemia (embolic, arterial spasm), epilepsy that may be primary or tumour-related and direct toxic effects of contrast media.

Prognosis of transient global amnesia: a long-term follow-up study.

A long-term follow-up study was performed on patients with transient global amnesia (TGA) in order to evaluate the prognosis, the recurrence rate and the occurrence of stroke and dementia. 102 patients (57 women, 45 men; mean age 62.8 +/- 9.4 years) were prospectively included and followed up. The follow-up duration ranged between 12 and 241 months with an average value of 82.2 +/- 51.1 (mean +/- SD). The death rate showed no difference from that of sex- and age-matched subjects. TGA recurred in 19 cases (18.63%). Only 4 patients suffered subsequent stroke, and only 3 showed intellectual deter

Five-Year Prognosis of Transient Ischemic Attack and Transient Global Amnesia in a Finnish Population.

BACKGROUND: Transient ischemic attack (TIA) often precedes the ischemic stroke and, therefore, requires urgent treatment, whereas transient global amnesia (TGA) is a benign condition but often causes concern to patients and their relatives. The longitudinal prognosis of TIA or TGA has not recently been reported in Finland. In this study, we studied the 5-year prognosis of TIA and TGA concerning especially stroke, TIA, and mortality. METHODS: The original study population includes 293 patients with TIA and 56 patients with TGA diagnosed in Kuopio University Hospital (KUH) in 2017. Original dat

Transient global amnesia and hippocampal diffusion restriction: an overlooked radiological finding.

Transient global amnesia (TGA) is characterized by isolated sudden anterograde amnesia. Diffusion restriction can be observed in the hippocampus on DWI-MRI at varying rates in TGA patients. This study analyzes the prevalence and characteristics of the hippocampal diffusion restriction (HDR), its relationship with vascular risk factors, and the prevalence of lesions overlooked in routine reports. 91 patients diagnosed with TGA at a tertiary hospital between 2011 and 2022 were evaluated retrospectively. The mean age was 64.8 ± 7.3 years, and 63.7% of patients were female. 75.8% of the patients h

The link between reversible cerebral vasoconstriction syndrome and transient global amnesia.

Reversible cerebral vasoconstriction syndrome (RCVS) and transient global amnesia (TGA) are acute and self-limiting intra-cerebral conditions. Although previously studied as independent phenomena, there are increasing reports of co-occurrence of these two pathologies. We report a 55-year-old male who presented to the hospital with recurrent thunderclap headaches over the course of 1 week with sudden onset of anterograde memory loss. His medications included a selective serotonin reuptake inhibitor and intermittent use of pseudoephedrine. On examination he was amnestic to recent events and nota

Forgetting the Unforgettable: Transient Global Amnesia Part I: Pathophysiology and Etiology.

Transient global amnesia (TGA) is a clinical syndrome characterized by the sudden onset of a temporary memory disorder with a profound anterograde amnesia and a variable impairment of the past memory. Since the first description, dating back over 60 years, several cases have beenreported in the literature. Nevertheless, TGA remains one of the most mysterious diseases in clinical neurology. The debate regarding the etiology of this disease has focused mainly on three different mechanisms: vascular (due to venous flow changes or focal arterial ischemia), epileptic, and migraine related. However,

Transient global amnesia associated with sildenafil use. Report of one case.

A 69 years old male with erectile dysfunction lasting 2 years, took 50 mg of sildenafil for having sex with his wife at about 6 o'clock in the morning. One hour later his wife detected that he had an anterograde memory impairment: this was interpreted as a confusional state. The neurological examination suggested a transient global amnesia (TGA). EEG and cerebral magnetic resonance imaging were non-informative and memory deficits resolved within 24 h. Therefore, a TGA was diagnosed. Since no other trigger was detectable, sildenafil was deemed responsible for its occurrence.

Simultaneous transient global amnesia and right MCA stroke after Valsalva manoeuvre.

A 61-year-old man suffered an episode of transient confusion and anterograde amnesia after a Valsalva-related manoeuvre. The MRI diffusion weighted imaging (DWI) sequences showed a left hippocampal and two right parietal lesions that were deemed as acute. The MR angiography disclosed a high-grade stenosis in the right middle cerebral artery as was described by a transcranial colour-coded ultrasound as well. Ultrasound investigation of the jugular veins showed a right jugular venous reflux after a Valsalva manoeuvre. The patient was diagnosed with transient global amnesia based on clinical grou
