Same-day Discharge Following Holmium Laser Enucleation of the Prostate Under Spinal Anesthesia: A Propensity Score Matched Comparison With General Anesthesia.

OBJECTIVE: To determine if using spinal anesthesia (SA) for holmium laser enucleation of the prostate (HoLEP) impacted the ability to perform same-day discharge (SDD) compared to a prostate volume-matched cohort undergoing HoLEP under general anesthesia (GA). METHODS: From January 1, 2021 to March 28, 2024, 995 men underwent HoLEP by a single surgeon. Three hundred eleven were identified who had SA and a recorded preoperative prostate volume. Propensity score matching based on prostate volume was performed with the remaining cohort who received GA in a 1:1 ratio. The primary outcome was rate

Preservation of motor evoked potentials under anesthesia in children with spinal muscular atrophy type II undergoing spinal deformity surgery.

Spinal muscular atrophy is a progressive condition in which movement is gradually lost as a result of the loss of spinal motor neurons. Individuals with this condition may require surgical correction of a secondary scoliosis. Motor evoked potentials were recorded using transcranial electrical stimulation in four such individuals undergoing surgery. All the patients were nonambulatory and in wheelchairs. Motor evoked potentials were recordable in both upper and lower limb muscles, with similar stimulation parameters to control subjects undergoing surgery for idiopathic scoliosis. The amplitudes

Good outcome with conservative treatment of delayed spinal epidural hematoma following combined spinal-epidural anesthesia: a rare case report.

BACKGROUND: Delayed spinal epidural hematoma (SEH) following central neuraxial block (CNB) is a rare but serious complication. The underlying causes of SEH associated with neuraxial anesthesia are still unclear. Furthermore, the decision between surgical intervention and conservative management for SEH remains a complex and unresolved issue. CASE PRESENTATION: We report a case of delayed SEH in a 73-year-old woman who underwent vaginal hysterectomy under combined spinal-epidural anesthesia, with the administration of postoperative anticoagulants to prevent deep vein thrombosis on the 1st post

Comparative Outcomes of Hemorrhoidectomy under Spinal Anesthesia between Older and Super-old Patients.

OBJECTIVES: With increasing life expectancy, the number of older and very old patients undergoing anorectal surgery is rising. However, concerns remain regarding the safety of hemorrhoidectomy among those aged ≥80 years. In this study, we compared postoperative outcomes between older (age 65-79 years) and very old (age ≥80 years) patients undergoing open hemorrhoidectomy under spinal anesthesia. METHODS: This single-center retrospective cohort study included consecutive adult patients who underwent excisional hemorrhoidectomy between April 2024 and May 2025. Patients were stratified into olde

Regional anesthesia after spinal cord injury attenuates behavioral loss, hemorrhage, and cell death.

OBJECTIVE: Prior research has shown that regional application of an anesthetic (lidocaine) via a lumbar puncture can attenuate pain-induced secondary tissue loss after spinal cord injury (SCI). Applying lidocaine rostral to injury (at T2) also had a protective effect, suggesting neural signaling to/from rostral systems contributes to pain-induced tissue loss after SCI. The present study examined whether regional anesthesia protects the injured spinal cord when applied immediately after injury in the absence of additional pain. We hypothesized that inhibiting cellular activity in the region of

Rising Above Lumbar Scars: Thoracic Spinal Anesthesia With Hypobaric Bupivacaine for Hip Replacement.

Neuraxial anesthesia is widely practiced for lower limb, abdominal, and pelvic surgeries due to its favorable risk-benefit profile compared with general anesthesia (GA). However, patients with previous lumbar spine surgery and instrumentation present significant challenges for conventional lumbar spinal anesthesia. Altered anatomy, epidural scarring, and the presence of metallic implants often preclude successful needle placement or result in patchy anesthesia. In such patients, GA may be considered, but it carries substantial risks in elderly individuals with significant comorbidities. Thorac

Feasibility of continuous high-resolution bioreactance monitoring during cesarean delivery under spinal anesthesia.

PURPOSE: This study is the first to apply bioreactance-based hemodynamic monitoring with a 4-s interval during cesarean delivery under spinal anesthesia. We evaluated the feasibility of continuous, high-resolution perioperative monitoring in a routine clinical setting, with a particular focus on the temporal relationship between hypotension and changes in advanced hemodynamic parameters. METHODS: This prospective observational study conducted between December 2023 and April 2024 included 51 healthy parturients scheduled for elective cesarean delivery under spinal anesthesia were included. When the Pupils Lie: Unmasking an Unnoticed High Spinal Anesthesia During Orthopedic Surgery.

High spinal anesthesia is a rare but potentially serious complication of neuraxial techniques. Unlike total spinal anesthesia, which typically presents dramatically with profound cardiovascular collapse, high spinal anesthesia may evolve more insidiously and manifest primarily with neurological signs. We report the case of a 23-year-old polytrauma patient undergoing femur and tibia fixation under combined spinal-epidural anesthesia (CSEA) who developed sudden unresponsiveness with fixed, dilated pupils shortly after femoral canal reaming. An epidural top-up of 10 mL 0.25% bupivacaine was admin

Volume status evaluation by IVC diameter and pleth variability index in spinal anesthesia.

OBJECTIVE: We aimed to predict the risk of hypotension due to spinal anesthesia (HSA) by measuring the diameter and area of the inferior vena cava (IVC-D, IVC-A) through ultrasound and pleth variability index (PVI) in patients who underwent endovenous radiofrequency ablation under spinal anesthesia (SA). METHODS: This cross-sectional, observational, and prospective study was completed with 64 patients. Before SA, PVI IVC-D, and IVC-A were measured. Patients were divided into two groups as hypotension group (Group 1) and patients who did not developed hypotension after SA (Group 2). Shapiro-Wi

Effects on ondansetron of postdural puncture headache after cesarean section under spinal anesthesia.

BACKGROUND: Spinal anesthesia is commonly performed for cesarean section, however, postdural puncture headache (PDPH) is one of its most common adverse effects. Ondansetron is an antiemetic for cancer treatment and analgesia-induced nausea and vomiting. In this study, the authors aim to evaluate the effect of postoperative ondansetron on PDPH. METHODS: In this randomized controlled clinical trial study, 120 pregnant patients are ASA ll, undergoing elective cesarean section, were randomized into two groups (placebo or study). The patients in the study group, immediately after the birth of a ba

Effectiveness and safety of spinal anesthesia in patients undergoing open radical retropubic prostatectomy.

OBJECTIVE: Prostate cancer is one of the most widespread neoplasms affecting the male gender. The most commonly used procedures in various urological centers are laparoscopic and robotic surgery because they are considered minimally invasive techniques. We present our experience in traditional open radical prostatectomy performed under spinal anesthesia. MATERIALS AND METHODS: We reviewed the clinical courses of 88 consecutive patients who underwent open radical prostatectomy performed under spinal anesthesia at our Institution. RESULTS: Median age: 67.7 years. Median follow up duration: 48

Exacerbation of Spinal Stenosis Symptoms Following Neuraxial Anesthesia in an Achondroplastic Cesarean Section.

We report the case of an achondroplastic female who presented with acute neurologic decline following epidural anesthesia for an elective cesarean section. Achondroplasia presents unique anatomical challenges to anesthesiologists in perioperative management, and cesarean sections are standard for achondroplastic pregnancies. High rates of spinal stenosis and lumbar radiculopathy in this patient population make administration of epidural analgesia technically challenging and may increase the risk of neurologic injury. Ultrasound is an effective means of administering epidural anesthesia for mos

Hypotension after spinal anesthesia for cesarean section: how to approach the iatrogenic sympathectomy.

PURPOSE OF REVIEW: Hypotension during cesarean section remains a frequent complication of spinal anesthesia and is associated with adverse maternal and fetal events. RECENT FINDINGS: Despite ongoing research, no single measure for sufficient treatment of spinal-induced hypotension was identified so far. Current literature discusses the efficacy of low-dose spinal anesthesia, timing and solutions for adequate fluid therapy and various vasopressor regimens. Present guidelines favor the use of phenylephrine over ephedrine because of decreased umbilical cord pH values, while norepinephrine is dis
