Flexible laryngoscopy in neonates and infants: insertion through a median opening in the face mask.

The most common reasons for laryngoscopy in newborns and infants are inspiratory stridor and/or dyspnea. With the help of flexible endoscopes, especially with instruments of diminished diameter, laryngoscopy can be carried out even in premature newborns. We report on a technique of flexible laryngoscopy without the need for restraint and with improved safety. A modified face mask is used for endoscopy. An additional hole is made into the mask with a 4.5 mm drill. It is located in the midline just above the nostrils, about 2.5 cm above the ventilation opening. The flexible endoscope is passed i

Performance and Hallucination Analysis of Large Language Models on European Anesthesiology Examinations: Cross-Sectional Comparative Study.

BACKGROUND: Large language models (LLMs) have shown promising performance on medical examinations across specialties. However, comparative evaluations of current-generation LLMs across multiple European anesthesiology examinations, alongside structured assessment of hallucinations vs question-related confusion, remain lacking. OBJECTIVE: This study aimed to compare the performance of 4 state-of-the-art LLMs on anesthesiology and intensive medicine examination questions and assess their hallucination rates. METHODS: This computational comparative study analyzed 437 multiple-choice questions (

Evaluating the Safety of a Ketamine Sedation Protocol In a Pediatric Oncology Population in Tanzania: A Quality Improvement Project.

This project aimed to reintroduce and evaluate a pediatric ketamine sedation protocol at a Zonal Referral Center in Mwanza, Tanzania, where nonanesthesia clinicians provide sedation when trained anesthesia personnel are unavailable. The objectives were to assess knowledge retention, confidence, and protocol compliance. The study design used a pre- and postintervention design. Nonanesthesia clinicians at a > 1,000-bed hospital were surveyed to assess their knowledge and confidence about ketamine sedation. Over 10 days, 50 clinicians received training that included lectures and cognitive aid res

Association of Anaesthetists guidelines: safe vascular access 2025.

INTRODUCTION: Safe vascular access is integral to anaesthetic and critical care practice. However, despite technological and procedural advances, it remains a frequent source of adverse events and patient harm. Ensuring a safe and effective approach to the selection, insertion and care of vascular access devices should be a priority for all practitioners. METHODS: This updated consensus statement builds upon previous iterations of safe vascular access guidelines. An expert, multidisciplinary, multi-society working party agreed on major themes and conducted a review of literature and best prac

Dexmedetomidine for Postoperative Delirium Prevention in the Older Adult: An Integrative Review.

The brain and cognition are particularly vulnerable to anesthetic and surgical insults, with postoperative delirium being the most common po … chnique influences the process, necessitating the importance of educated certified registered nurse anesthetists. Dexmedetomidine, a nonspecific α2-adrenergic receptor agonist, exhibits anti-inflammatory properties that counteract the proinflammatory mechanisms initiated by surgical insult. Additionally, dexmedetomidine mimics natural sleep pathways and reduce …

The Surgical and Anesthesia Workforce and Provision of Surgical Services in Rural Communities: A Mixed-Methods Examination.

PURPOSE: Rural-urban disparities in the surgical and anesthesia workforce exist. This mixed-methods study describes the distribution of the surgical and anesthesia workforce and qualitatively explores how such workforce and other factors influence rural hospitals' provision of surgical services. METHODS: We calculated provider counts by county from the Area Health Resource File. Using American Hospital Association survey data, we sampled rural hospitals, stratified by critical access status and state policies. We conducted qualitative semistructured interviews with administrators at 16 hospit

The intraoperative use of non-opioid adjuvant analgesic agents: a survey of anaesthetists in Australia and New Zealand.

BACKGROUND: Opioids have long been the mainstay of drugs used for intra-operative analgesia. Due to their well-known short and long term side effects, the use of non-opioid analgesics has often been encouraged to decrease the dose of opioid required and minimise these side effects. The trends in using non-opioid adjuvants among Australian Anaesthetists have not been examined before. This study has attempted to determine the use of non-opioid analgesics as part of an opioid sparing practice among anaesthetists across Australia and New Zealand. METHODS: A survey was distributed to 985 anaesthet

A Narrative, Qualitative Research Study of CRNA Perspectives of Practice Within the Anesthesia Care Team Model.

This research focused on the perspectives of certified registered nurse anesthetists on practice within the anesthesia care team (ACT) model. This narrative, qualitative study examined participants' perspectives on the strengths, challenges, conflict resolutions, and any other information they provided about working in the ACT model. The participants drew sharp distinctions between the supervision requirement of the ACT and that of collaboration with a team approach. The participants reported that they enjoyed and supported collaboration and found that the team approach led to efficient patien…

Anaesthetist Preparedness for Transhepatic Approach to Ostium Secundum Atrial Septal Defect Device Closure: A Case Report.

Routes for transcatheter closure of ostium secundum atrial septal defect (OS ASD) by device implantation include hepatic vein & jugular vein besides standard transfemoral approach. We report the case of a female child with OS ASD, who presented with an anomalous systemic venous drainage. Only the hepatic vein was draining directly to RA hence the only option left was transhepatic. Multidisciplinary consensus was taken and the procedure proceeded under general anaesthesia. To avoid cathlab misadventures, tracing systemic venous drainage besides pulmonary venous drainage in a case of OS ASD is i

Balancing Risks in Obstetrics: Anesthesia Management in Facioscapulohumeral Muscular Dystrophy with Scoliosis.

Facioscapulohumeral muscular dystrophy (FSHD), the third most common form of muscular dystrophy, is associated with a normal life expectancy, resulting in more pregnant women presenting with the condition. Pregnancy often exacerbates muscle weakness in patients with FSHD, posing unique perioperative challenges. Careful evaluation of systemic involvement is essential to minimize maternal and neonatal risks. Perioperative management is often challenging because both general and neuraxial anesthesia carry significant risks. Spinal deformities further complicate anesthesia management, requiring tr

Dexmedetomidine Dosing to Prevent Pediatric Emergence Delirium.

Because of the high incidence and untoward effects of emergence delirium in the pediatric population, investigating pharmacologic measures for preventing this phenomenon is important to the anesthesia provider. Dexmedetomidine, a highly selective α2 agonist, has been shown to prevent emergence delirium in the perioperative setting; however, recommendations for best practice regarding use of this medication are not widely available. Barriers to the use of dexmedetomidine may include side effects such as bradycardia and delayed emergence, as well as limited evidence for the best practice of timi

Methylene Blue as an Adjunct to Treat Vasoplegia in Patients Undergoing Cardiac Surgery Requiring Cardiopulmonary Bypass: A Literature Review.

The term vasoplegia describes hypotension refractory to vasopressor therapy, a common finding related to cardiac surgery requiring cardiopulmonary bypass. High doses of vasoactive agents are associated with adverse effects such as peripheral and mesenteric ischemia. Databases were systematically searched for literature on methylene blue as an adjunct therapy to treat vasoplegia. Fifteen articles were selected. The quality of the studies was evaluated using the US Preventive Services Task Force (USPSTF) grading tool, and a chart was created to present the components of each study. Preoperative,

Aspirations to become an anaesthetist: longitudinal study of historical trends and trajectories of UK-qualified doctors' early career choices and of factors that have influenced their choices.

BACKGROUND: It is important to inform medical educators and workforce planners in Anaesthesia about early career choices for the specialty, factors that influence them and to elucidate how recent choices of men and women doctors relate to the overall historical trends in the specialty's popularity. METHODS: We analysed longitudinal data on career choice, based on self-completed questionnaires, from national year-of-qualification cohorts of UK-trained doctors from 1974 to 2012 surveyed one, three and 5 years post-qualification. Career destination data 10 years post-qualification were used for

Unilateral hypoglossal neurapraxia following endotracheal intubation for total shoulder arthroplasty.

A case is described of postoperative right hypoglossal neurapraxia after general anesthesia and interscalene block with endotracheal intubation for left total shoulder arthroplasty. Postoperative hypoglossal neurapraxia has been reported in cases, yet it remains a rare complication of anesthesia-related interventions. In this case report, postulated causes of hypoglossal neurapraxia are presented. A review of the literature pertaining to anesthesia-related causes of hypoglossal nerve injury is included. Anesthesia providers should be aware of the course of cranial nerve XII as it relates to th

Placenta percreta: report of a case.

Placenta accreta, increta, or percreta are rare but potentially lethal obstetric emergencies. Removal of abnormal growth of the placenta into the uterine wall is difficult or impossible and results in massive blood loss. Hysterectomy may be necessary to save the mother's life. The common predisposing factors in development of placenta percreta are repeat cesarean and placenta previa. The diagnosis of placenta percreta may remain undiagnosed until delivery. The case presented describes a scenario involving placenta percreta with bladder involvement in which the diagnosis was known in advance. T

Endoscopic thymectomy: a neurologist's perspective.

Myasthenia gravis (MG) is an autoimmune neuromuscular disease characterized by the presence of antibodies interacting at the neuromuscular junction (NMJ), resulting in loss of strength and severe exhaustibility of striated muscles. The abnormal production of these antibodies is triggered mainly in the thymus, and hence thymectomy in MG is considered a universally recommended treatment in order to improve the symptomatologic condition of this pathology. Currently, minimally invasive thymectomy using the Da Vinci robot system is certainly one of the most innovative techniques, performed in Pisa
