Clinical relevance of homocysteine levels in patients receiving coronary stenting for unstable angina.

BACKGROUND: We prospectively investigated whether plasma homocysteine (HCY) concentrations are related to target lesion revascularization (TLR) rates in patients with unstable angina undergoing stenting. METHODS: We enrolled 196 consecutive patients with at least one successful coronary stent implantation for unstable angina. RESULTS: The mean vessel diameter was 3.1 +/- 0.5 mm. At follow-up (17.8 +/- 7.5 months), patients with higher HCY levels (> 17 micromol/l, 4th quartile) had similar TLR rates to the rest of the sample (11.1 vs 13.2%, p = 0.90). On the other hand, high HCY levels did se

Circulating endothelial progenitor cells in patients with unstable angina: association with systemic inflammation.

BACKGROUND: Endothelial progenitor cells (EPC) are present in peripheral blood and can develop a functional endothelial phenotype. The number and function of circulating EPCs are altered in atherosclerosis, diabetes, and after myocardial infarction and EPCs have been shown to promote postnatal angiogenesis and vasculogenesis. We investigated the number and adhesive properties of EPCs from patients with unstable angina and no evidence of cardiac necrosis. METHODS AND RESULTS: Patients were selected with unstable angina (n=29) and no evidence of cardiac necrosis, and controls with stable angina

Differentiation of dendritic cells in monocyte cultures isolated from patients with unstable angina.

OBJECTIVE: Dendritic cells (DC) stimulate T-cell proliferation and activation in the course of adaptive immunity. Its role in unstable coronary plaque in humans is unknown. So we investigated the functional role of DC in patients with unstable angina pectoris (US) using human monocyte-derived DC. METHODS: Twenty milliliters of blood was drawn from the femoral artery of 20 patients, who underwent coronary angiography. Ten patients with a diagnosis of US and 10 patients with normal CAG were included in the observation and control groups, respectively. The mononuclear cells were separated from t

Interleukin-7-mediated inflammation in unstable angina: possible role of chemokines and platelets.

BACKGROUND: Atherogenesis and plaque destabilization involve immune-mediated mechanisms, but the actual mediators have not been fully clarified. Interleukin (IL)-7 is a regulator of T-cell homeostasis but also may be involved in inflammation. We hypothesized that IL-7 could be involved in the inflammatory processes observed in atherosclerosis and acute coronary syndromes. METHODS AND RESULTS: To study the role of IL-7 in coronary artery disease, we analyzed IL-7 levels and the effect of this cytokine on inflammatory mediators in patients with stable and unstable angina and in healthy control

Clopidogrel in acute coronary syndromes (unstable angina and non-Q-wave myocardial infarction).

Given the importance of thrombosis in acute coronary syndromes, antithrombotic therapy has become standard treatment for these conditions. This article reviews the mechanism of action and the major evidence supporting the clinical use of clopidogrel, a potent antiplatelet agent of the thienopyridines class, focusing on its role in the setting of acute coronary syndromes without persistent ST segment elevation (unstable angina and non-Q wave myocardial infarction). Some unanswered questions relating to this medication are also highlighted. Finally, current updates on clinical guidelines for the

QT interval in patients with unstable angina and non-Q wave myocardial infarction.

BACKGROUND: Non-Q wave myocardial infarction (NQMI) and unstable angina (UAP) have similar clinical presentations and similar ST-T changes on the electrocardiogram. The purpose of this study was to assess whether changes in QT interval might help differentiating between these entities. METHODS: The QT intervals of 52 patients hospitalized with NQMI were compared to those of 52 patients hospitalized for UAP. All patients had repeated ECG for at least 4 days. RESULTS: Maximal QTc in patients with NQMI was significantly longer than in patients with UAP (475 vs 439 ms, P < 0.0001). QTc on the ad

[Evaluation of C3c, C4 and C1-esterase inhibitor (C1-INH) during unstable angina].

BACKGROUND: The complement system plays an important role in the physiopathology of acute myocardial infarction (AMI) taking part in myocardial damage and reperfusion injury. The aim of this study is to investigate the plasmatic levels of some complement components (C3c, C4 and C1-INH) during unstable angina (C1-INH) and their different concentrations in relation to the different myocardial areas affected by ischemia. METHODS: The plasmatic levels of C1-INH, C3c and c4 in 30 patients affected by unstable angina, and those of 22 clinically healthy subjects (control group) were evaluated (Nefel

[Early intervention in acute coronary syndromes (unstable angina and non-Q wave infarction].

The timing for coronary angiography and revascularization in unstable angina and infarction without Q-wave in patients without high-risk characteristics after arrival to the hospital is uncertain. If high-risk patients are not considered, there are several therapeutical strategies for the diagnosis and treatment of the patients. The first is a conservative approach, in which coronary angiography and revascularization is done only in patients presenting angina or electrocardiographic changes of ischemia after arrival to the hospital. The other one is an invasive approach, in which coronary angi

Role of prolonged intravenous heparin in unstable angina patients prior to coronary angioplasty.

BACKGROUND: The optimal timing of coronary angioplasty in unstable angina patients is controversial. Early reports suggested using 3-5 days of intravenous heparin and aspirin for plaque stabilization before angioplasty. There is no clearcut data in this regard from the published literature. The purpose of this study was to evaluate whether delaying the angioplasty in order to stabilize the plaque affected the outcome. METHODS: We reviewed the hospital course of patients who were admitted with unstable angina through the emergency room and ruled out for myocardial infarction, and who required

[Which medical treatment can minimize late complications after pharmacologic treatment of unstable angina?].

UNLABELLED: Current pharmacological treatment of unstable coronary diseases has significantly reduced the rate of early and late complications. Aim of this study was to investigate the impact of in-hospital and ambulatory treatment on the occurrence of late complications in patients with unstable angina treated conservatively. The study group consisted of consecutive 79 patients hospitalized in our Department on the turn of years 1998-1999 (48 men--age 62 +/- 11 years), followed up by at least 1 year. We analyzed the administered agents on the first day of hospitalization: acetylsalicylic acid

Debate: When should we intervene in unstable angina - Time for an old look?

Current treatment modalities for patients with acute coronary syndromes center on early diagnosis, risk stratification and, increasingly, early treatment including invasive approaches. The appropriate timing of these invasive modalities in the context of the overall treatment program remains an area of controversy. Specifically, studies in the past recommended a period of medical 'stabilization' while current approaches are considerably more aggressive. The potential hazard of early intervention, in particular, has not properly been weighed against the benefit. This article hopes to provide a

Tissue plasminogen activator using a rapid-infusion low-dose regimen for unstable angina.

The clinical effect of a low-dose rapid-infusion intravenous regimen was assessed using human recombinant tissue-type plasminogen activator (rt-PA) in unstable angina. Fifty patients with unstable angina pectoris were randomly assigned to blinded treatment with either placebo (24 patients) or low-dose (20 mg bolus, 30 mg infusion over 1 hour) intravenous rt-PA (26 patients). Before randomization, all patients were treated with aspirin, twice-daily subcutaneous heparin, and maximally tolerated antianginal therapy. Of the 50 patients assigned, 26 received rt-PA and the outcome was successful in

Cooling down unstable angina with high dosage of isosorbide dinitrate (ISDN) continuously infused.

The aim of this study was to explore the capability of isosorbide dinitrate (ISDN) infusion to quench the 'hot phase' of unstable angina, a real cardiological emergency. Fifteen patients consecutively admitted to CCU because of angina at rest, with at least 4 ischaemic attacks per day, resistant to the usual therapy with oral and/or topical nitrates and calcium-antagonists, were included in the study. During ischaemia the electrocardiogram showed ST-segment elevation in 7 patients, ST-segment depression in 4, an alternation of ST-segment elevation and depression in 3 and pseudonormalization of
