Mulitcenter study on antibiotic prophylaxis, infectious complications and risk assessment in TUR-P.

INTRODUCTION: Transurethral resection of the prostate is one of the most frequent urological procedures. Urinary tract infections represent major sequelae, but data about antibiotic prophylaxis in TUR-P are controversial and outdated. MATERIAL AND METHODS: We conducted a retrospective multicentre study of TUR-P in ten German hospitals. Primary endpoints were epidemiological and outcome data of TUR-P. Secondary endpoints were the identification of factors associated with febrile UTIs and sepsis after TUR-P. RESULTS: We included 444 patients with a median age of 71.0 years. Nearly every patien…

Antibiotic prophylaxis for urinary tract infection after midurethral sling: a randomized controlled trial.

OBJECTIVE: Because of the reported high percentage of bladder infections after placement of a midurethral sling (8.9%-34%), this study was undertaken to determine if a 3-day postoperative course of a urinary antibiotic would significantly lower the incidence of urinary tract infection (UTI) among treated women. METHODS: Women who were planning outpatient vaginal surgery for stress incontinence were enrolled, after informed consent, and randomized to receive placebo or nitrofurantoin (100 mg 2 times a day) for 3 days after surgery. Research subjects and investigators were blinded to the random

Post-operative wound infection in salvage laryngectomy: does antibiotic prophylaxis have an impact?

Salvage laryngectomy carries a high risk of post-operative infection with reported rates of 40-61%. The purpose of this study was to analyse infections in our own patients and review the potential impact of our current antibiotic prophylaxis (AP). A retrospective analysis of infection in 26 consecutive patients between 2000 and 2010 undergoing salvage total laryngectomy (SL) following recurrent laryngeal cancer after failed radiotherapy or chemo-radiation was undertaken. The antibiotic prophylaxis was intravenous teicoplanin, cefuroxime and metronidazole at induction and for the following 24 h…

Late-onset group B streptococcal meningitis in a neonate with early antibiotic prophylaxis.

An otherwise healthy female baby who had received early postnatal antibiotic prophylaxis because of maternal vaginal group B streptococcal (GBS) colonization died of late-onset GBS meningitis. GBS was isolated from blood and cerebrospinal fluid. Genotyping using pulsed-field gel electrophoresis confirmed that the GBS isolates from the patient were identical to those from her mother's vagina. Susceptibility tests demonstrated no antibiotic resistance. This report provides additional microbiologic evidence indicating that early postnatal antibiotic prophylaxis is not effective in preventing late…

Randomized clinical trial of single-dose antibiotic prophylaxis for non-reconstructive breast surgery.

BACKGROUND: The aim of this randomized clinical trial was to determine whether a single intravenous dose of 2 g flucloxacillin could prevent wound infection after primary non-reconstructive breast surgery. METHODS: The study included 618 patients undergoing local excision (n = 490), mastectomy (n = 107) or microdochectomy (n = 21). Patients were randomized to receive either a single dose of flucloxacillin immediately after the induction of anaesthesia or no intervention. Wound morbidity was monitored by an independent research nurse for 42 days after surgery. RESULTS: The incidence of wound

Surgical infections after laparoscopic cholecystectomy: ceftriaxone vs ceftazidime antibiotic prophylaxis. A prospective study.

The incidence of surgical infections after laparoscopic cholecystectomy is reported to be <2%, because of the minimal trauma due to this approach. We report the results of a prospective study of antibiotic prophylaxis in laparoscopic cholecystectomy, comparing ceftriaxone vs ceftazidime. From Jan 1 to Dec 31 2002 a consecutive series of 242 cholecystectomies were performed, consisting in 18 open cholecystectomies and 224 laparoscopic cholecystectomies, 7 of which (3.1%) were converted to open cholecystectomies for technical and/or anatomical reasons. One hundred and eleven patients received 1 …

Best evidence topic report. Antibiotic prophylaxis for pretibial haematomas in the elderly population.

A short cut review was carried out to establish whether prophylactic antibiotics reduce infections and other complications in elderly patients with pretibial haematomas. Altogether 65 papers were found using the reported search, of which none presented any evidence to answer the clinical question. It is concluded that there is no evidence available to answer this question. Further research is needed.

Infective endocarditis: dental implications and new guidelines for antibiotic prophylaxis. American Heart Association.

The American Heart Association (AHA) has developed new guidelines for antibiotic prophylaxis for patients at risk of infective endocarditis (IE). These guidelines are based on the on-going study of endocarditis cases and continued experience with antibiotic prophylaxis, and are an update of the 1990 recommendations. The major changes from the 1990 guidelines are: the recognition that most cases of IE are not attributable to an invasive procedure; that cardiac conditions may present different levels of risk categories; increased delineation of when prophylaxis should be given in cases of mitral…

Antibiotic prophylaxis in pulmonary surgery: a double-blind study of penicillin versus placebo.

A prospective, randomized double-blind study comparing high-dose short-term penicillin-G prophylaxis with placebo was conducted on patients referred for elective pulmonary surgery. The major advantages of penicillin prophylaxis over placebo were observed for wound infections (2/45 vs 9/47, respectively, p = 0.03), postoperative antibiotic use (13/45 vs 23/47, respectively, p = 0.049), and postoperative hospital stay (median 10 days vs 13 days, respectively, p = 0.02). The prophylactic penicillin regimen had no effect on the incidence of empyema or lower respiratory tract infections. Staphyloco

Prevalence of antibiotic resistance in fecal flora before transrectal ultrasound-guided prostate biopsy and the clinical impact of targeted antibiotic prophylaxis.

OBJECTIVES: In this study, we aimed to evaluate ciprofloxacin resistance and the presence of extended-spectrum beta-lactamase producing organisms (ESBLs) in the population of patients with indication to undergo prostate biopsy. Our additional aim was to compare the targeted antibiotic prophylaxis and sepsis rates after transrectal ultrasound guided prostate biopsy in patients who received routine ciprofloxacin prophylaxis and to evaluate the predictive factors of antibiotic resistance. METHODS: Between September 2012 and January 2014, 300 patients were randomized to two groups. The first grou…

Does Antibiotic Choice for Prophylaxis Affect the Outcome of Wound Infections in Earthquake Victims?

OBJECTIVE: Wound infections represent a major concern among earthquake survivors. Antibacterial prophylaxis is standard care for this population. However, there is no clear consensus guidelines regarding the choice of prophylactic agents. This study aimed to examine whether the spectrum of antibacterial prophylaxis affects wound infection rates in hospitalized survivors of the February 2023 earthquake in south-eastern Türkiye who were transferred to a tertiary care center outside the disaster zone. MATERIALS AND METHODS: This retrospective study included patients with uninfected wounds upon h
