The authors report a case of arachnoid cyst of cauda equina with severe chronic coccygodynia. The value of radioculography in the diagnosis of this condition is pointed out. Resection of the cyst abolished coccygodynia completely and permanently. No case of arachnoid cyst causing coccygodynia has been found in the available literature.

An unusual case is reported of an intradural lumbar arachnoid diverticulum causing mild compression of the cauda equina and producing debilitating symptoms which were relieved by inadvertent drainage. A survey of the literature indicates that lumbar arachnoid diverticula are very rare. Only one other report of an intradural lesion of this type has been found. These lesions cause back pain or radicular pain and sensory disturbances when the patient is upright of bending forward. These symptoms are relieved by lying down. Percussion tenderness over the level of the diverticulum is a consistent f

The authors discuss reabsorption of cerebrospinal fluid in relation to the post-mortem findings in two children, one with total agenesis of the Pacchionian system accompanied by hydrocephalus, and the other with a subtotal agenesis and no hydrocephalus. Case 1 is the only known documented case of total agenesis of the Pacchionian system and gives credence to the idea that an impaired reabsorption of cerebrospinal fluid at the level of the Pacchionian system is a cause of hydrocephalus. The patient in Case 2 showed only two small areas of arachnoid granulations containing a few flattened, micro

BACKGROUND: Neuroenteric cysts (NC) are caused by an error in embryonic development, in which there is a failure in the separation between the notochord and the anterior intestine, leading to the formation of cysts, usually located in the cervicothoracic spine region.
CASE DESCRIPTION: A 10-year-old male patient reported 20 days of neck pain. In the magnetic resonance imaging study, the lesion C2-C3 was homogeneously hyperintense on T1- and T2-weighted images. There was no restriction on diffusion-weighted imaging, with severe spinal cord compression. Surgery was suggested to remove the cyst

Spinal arachnoid webs are rare intradural extramedullary lesions that most commonly occur in the thoracic spine and may cause focal dorsal spinal cord compression, cerebrospinal fluid flow obstruction, syringomyelia, and progressive myelopathy. Diagnosis can be delayed because symptoms are often nonspecific and the arachnoid web itself may be difficult to directly visualize on standard magnetic resonance imaging. The characteristic "scalpel sign," defined by focal dorsal indentation of the spinal cord, is a key imaging feature. We present a case of a 47-year-old male patient with progressive m

BACKGROUND AND PURPOSE: Neurosurgical approaches in Sylvian arachnoid cysts include microsurgery, endoscopy, and shunting. Yet, their relative safety and efficacy is still under debate. This retrospective study evaluated 36 pediatric patients with Sylvian arachnoid cysts and treated with different surgical types to contribute to global data.
METHODS: The study included 24, 8, and 4 patients receiving endoscopic, microsurgical, and shunt surgeries, respectively. Preoperative and postoperative assessments included the patients' demographics and symptoms, cyst size and type, psychomotor status,

Spinal extradural arachnoid cyst is a rare condition, commonly located in the thoracic spine. It can be asymptomatic or present with symptoms of myelopathy due to spinal cord or nerve root compression. MRI is the initial imaging modality to diagnose this condition. CT myelography is the imaging modality of choice for detecting the actual site of communication between the subarachnoid space and arachnoid cysts; hence, it plays a pivotal role in surgical planning. We describe the imaging features of a symptomatic large spinal extradural arachnoid cyst and cord changes in a 12-year-old boy, who p

Arachnoid Cysts (AC) are benign lesions containing cerebrospinal fluid, and although most of them are asymptomatic, they can cause neurological symptoms like headaches, seizures, and neuropsychiatric problems. The aim of this study was to asses and document co-morbid psychiatric disorders in children with AC aged between 6 and 17. Wechsler Abbreviated Scale of Intelligence-Second Edition (WASI-II), a clinical measure used to assess the intelligence quotient (IQ) scores of the patients, Kiddie Schedule for Affective Disorders and Schizophrenia Present and Lifetime Version (K-SADS-PL; semi-struc

OBJECTIVE: In pediatric patients, middle cranial fossa (MCF) arachnoid cysts are often discovered incidentally on imaging in asymptomatic patients during workup for other indications. This study aims to describe current management gestalt and threshold for surgical intervention by surveying an international cohort of neurosurgeons.
METHODS: A web-based survey was circulated via email list of attendants of the 2019 Canadian Pediatric Neurosurgery Study Group (CPNSG) and International Society of Pediatric Neurosurgery (ISPN) mailing list. The survey consisted of 8 clinical scenarios involving p

This case demonstrates an endoscopic fenestration of an enlarging giant occipital arachnoid cyst. The patient is a 42-year-old woman presenting with headache, progressive vision loss, and nausea and vomiting. MRI demonstrates a large, nonenhancing cystic lesion in the right occipital lobe measuring up to 8.3 cm, consistent with an arachnoid cyst. This surgical video illustrates the technique for an endoscopic fenestration into the native ventricular system utilizing stereotactic MRI-guided stealth navigation. Postoperatively, the patient had full recovery with improvement of headaches and visi

BACKGROUND: Moyamoya disease (MMD) is a very specific disorder in terms of spontaneous development of extracranial-to-intracranial collateral circulation through the dura mater, but the underlying mechanisms are unclear. This study aimed to investigate the role of the arachnoid membrane in this unique angiogenesis in MMD.
METHODS: A piece of arachnoid membrane and 1- to 2-mL cerebrospinal fluid were simultaneously harvested during surgery from 26 patients with MMD. The specimens were also collected during surgery as the controls from 6 patients with atherosclerotic carotid artery diseases. Th

Quadrigeminal cistern arachnoid cysts (ACs) are usually asymptomatic, may be accidental findings during radiological evaluation, and are rare (5%-10% of all intracranial ACs). We report a case of type I quadrigeminal cistern AC managed via navigation-guided cystoventriculostomy followed by endoscopic third ventriculostomy (Video 1). A 0° rod-lens endoscope was used. Different types of cysts may require different endoscopic approaches, and our procedure was facilitated by the presence of significant ventriculomegaly.1 The endoscopic procedure was completed uneventfully; navigation was used to l

Arachnoid cysts (AC) occur in different intracranial locations. Management and prognosis depend on the clinical presentation and treatment guidelines do not exist. With this study, we want to demonstrate the clinical variety of arachnoid cysts in children and place a focus on outcome factors in operated cases. This retrospective study of a consecutive single unit series of children, who underwent AC surgery between January 2010 and September 2019, provides demographic, clinical, imaging data, and information about surgical treatment and outcome. Overall, 63 patients (71.4 male) underwent surge

BACKGROUND: Incontinentia pigmenti (IP) is a rare X-linked dominant genetic disorder that can be fatal in male infants. It is a disease that affects many systems of the human body. In addition to characteristic skin changes, patients may also have pathological features of the eyes, teeth, and central nervous system. Therefore, the lesions in these systems may be the first symptoms for which patients seek treatment. To date, no cases of IP complicated by intracranial arachnoid cyst (IAC) have been reported. This paper aims to report a case of IP with IAC in order to share the diagnosis and trea

BACKGROUND: Meningioma growing into an arachnoid cyst is an extremely rare event. Only 3 cases are reported in the literature. In 2 of them, an operative procedure in or near the arachnoid cyst preceded tumor growth.
CASE DESCRIPTION: We report a case of a patient requiring marsupialization of an arachnoid cyst of the middle cranial fossa. On follow-up, 3 years postoperatively he showed no signs of recurrence or tumor growth. One year later, the fourth year after surgery on the cyst, he presented with large tumor growth into the former cyst's cavity. Pathologic workup after resection revealed
