Polymyalgia rheumatica is a relatively common syndrome of older patients, mostly white, manifested by aching and morning stiffness of the pelvic and shoulder girdles and accompanied by a rapid erythrocyte sedimentation rate. The symptoms are due to a synovitis, which is mild, nondestructive and very responsive to low-dose steroid treatment, which may need to be continued for several years. Among 47 patients with polymyalgia rheumatica there were 30 (63.8%) with temporal giant-cell arteritis. Rapidly progressing visual reduction occurred in 2, acute blindness in 10. Four forms of polymyalgia rheumatica were identified by their different clinical courses, with differing risk of blindness. Those patients with polymyalgia rheumatica and giant-cell arteritis are most endangered who have headaches and neck and shoulder pains, but no or only mild general symptoms.

A patient is described in which features of both giant cell arteritis and polyarteritis nodosa were present simultaneously. The case emphasises our lack of an aetiologic classification for arteritis and that on occasions typical clinical presentations may be misleading.

A 62-year-old white woman presented with severe facial swelling, glossitis, and odynophagia. Giant cell arteritis (GCA) was diagnosed on the basis of temporal artery biopsy, and the patient responded to corticosteroids. GCA may rarely involve vessels of the mouth and face, resulting in unusual clinical manifestations. Early recognition and treatment are important.

Temporal headache, blindness, and polymyalgia rheumatica are well-recognized manifestions of giant cell arteritis. However, the disease may present in less evident fashion as shown by 30 of 74 patients with biopsy-proven giant cell arteritis whose predominant complaint was not one ot these cardinal symptoms. For this group of 30, the main problem was fever in 12 patients, anorexia, weight loss, and elevated serum alkaline phosphatase level suggesting an occult malignancy in 7, and unexplained anemia in 3. Two cases of giant-cell arteritis with cerebral arteritis are presented. The clinical situation and the suggestive but nonspecific angiographic features make the antemortem diagnosis possible. Greater awareness of this entity will facilitate its diagnosis and the institution of effective steroid therapy.

Cardiac conditions were examined in 48 patients with nonspecific aorto-arteritis. The authors assume that nonspecific aorto-arteritis may be accompanied by lesions, although usually moderate, of the valvular apparatus.
