Forty-nine patients with disseminated gonococcal infection (DGI) hospitalized at Boston City and University Hospitals over a 7-year period were studied. Patients with clinical manifestations of DGI and with cervical, urethral, rectal, pharyngeal, synovial or blood cultures positive for Neisseria gonorrhoeae were separated into two groups based on the presence or absence of suppurative arthritis. There were 19 cases of suppurative arthritis (Group II) and 30 cases with only tenosynovitis, skin lesions, or both (Group I). Blood cultures were positive only in Group I patients (43%) and synovial f

Septic arthritis of the sternoclavicular joint is a rare infectious arthritis in which the risk factors are reported to be such as diabetes, immunosuppression, and intravenous drug use. Due to a lack of prominent symptoms, delayed diagnosis can lead to severe complications such as mediastinitis and empyema. Advanced sternoclavicular septic arthritis can be a hidden etiology masked by severe symptoms. Therefore, awareness of this condition is crucial for timely and appropriate treatment. We report a rare case of sternoclavicular septic arthritis progressing to empyema with bronchial fistulae. T

Dogs with canine rheumatoid arthritis had significantly elevated levels of antibodies to canine distemper virus. This increase was particularly seen in the synovial fluids, compared with paired sera, and was not found in dogs with infective arthropathies, osteoarthritis or in osteoarthritis secondary to rupture of the cranial cruciate ligament. Analysis of the immune complexes precipitated from synovial fluids showed immunoglobulins in all types of arthropathy. Western blotting analyses showed reactivity with anti-distemper antisera in immune complexes from dogs with rheumatoid arthritis, but

Patients with rheumatoid arthritis and other immune-mediated inflammatory diseases are at higher risk for infectious morbidity and mortality, partially due to the therapies used to treat these conditions. Both prednisone and targeted biologic therapies such as tumor necrosis factor antagonists have been implicated to various degrees, although in some cases firm data are lacking with regard to certain types of infections. To date, there is a paucity of information regarding the infectious risks associated with the newer biologic agents. As new biologic agents become available for use, their pot

In two recent articles published in Molecular Immunology, Steere and colleagues continue their search for an autoimmune mechanism of arthritis in patients who have failed short-course antibiotic therapy for Borrelia burgdorferi, the spirochetal agent of Lyme disease. As in previous attempts, the authors fail to elucidate a molecular mimicry mechanism for the putative autoimmune process, leading to the conclusion that there is no credible scientific evidence for a post-infectious autoimmune mechanism of arthritis in chronic Lyme disease.

Juvenile idiopathic arthritis (JIA) constitutes a group of arthritis of unknown origin that begins before the age of 16 years. Still´s disease is the systemic form of this condition. Its clinical presentation is marked by fever, rash and sometimes joint pain, in the absence of evidence of another aetiology of the fever. We present the cases of two boys aged 4 and 10 years admitted for fever, with a cerebral origin for the first and no infectious site for the second. Fever persisted after antimalarial treatment and adequate antibiotics. Ferritinaemia, elevated sedimentation rate, lactate dehydr

INTRODUCTION: Reactive arthritis (ReA) is a joint inflammation that follows an infection at a distant site, often in the gastrointestinal or urogenital tract. Since the emergence of COVID-19 in January 2020, several case reports have suggested a relation between reactive arthritis and severe acute respiratory syndrome coronavirus 2 (SARS-COV-2), due to the novelty of the disease, most findings were reported in the form of case reports or case series, and a comprehensive overview is still lacking.
METHODS: We searched PubMed/Medline and Embase to identify studies addressing the association bet

Several infectious agents may cause arthritis or arthropathy. For example, infection with Borrelia burgdorferi, the etiologic agent of Lyme disease, may in the late phase manifest as arthropathy. Infections with Campylobacter, Salmonella, or Yersinia may result in a postinfectious reactive arthritis. Acute infection with parvovirus B19 (B19V) may likewise initiate transient or chronic arthropathy. All these conditions may be clinically indistinguishable from rheumatoid arthritis. Here, we present evidence that acute B19V infection may elicit IgM antibodies that are polyspecific or cross-reacti

The systemic juvenile idiopathic arthritis (SJIA) is a rare, auto-inflammatory and chronic childhood disease. Arthritis of at least one joint is associated with a daily fever lasting more than two weeks and with one of the following signs: lymphadenopathy, hepatomegaly, serositis, or skin rash. Systemic symptoms are often initially preponderant, in the absence of arthritis than can occur weeks or months later. The typical rash is maculopapular discrete, fleeting and dew. We present an unusual SJIA case, where the rash, like urticaria, presents itself as persistent, itchy hives. An arthritis of

Using the ELISA technique, antibodies against two different basement proteins, laminin and nidogen (ALNA), were determined in 226 children suffering from one of 37 different inflammatory or infectious diseases. These included 80 patients with streptococcal infection and 40 with juvenile rheumatoid arthritis. Forty-eight percent of the streptococcus-infected patients (or 75% of those in the acute phase) and 60% of juvenile rheumatoid arthritis patients had significantly elevated ALNA levels compared with healthy controls. Interestingly 10 adult rheumatoid arthritis patients displayed normal ALN

Report of a 30 years old man story pustulosis palmaris et plantaris, spondylodiscitis L5-S1, right sterno-clavicular arthritis, right sacro-iliitis, and pubic symphysitis. Multiple bacteriologic investigations did not discover any infectious articular process. Histologic articular findings were inflammatory pattern, non steroid anti-inflammatory drugs corrected the arthritis. Anterior thoracic arthropathies are quasi-constant. Inflammatory spondylodiscitis are frequent, while sacro-iliac involvement is infrequent, this pattern distinguishing pustular arthro-osteitis from classic pelvispondylit

Tuberculosis is a communicable disease that is occasionally complicated by arthritis, either as part of a disseminated primary infection or through late reactivation, often in immunocompromised hosts such as elderly people, HIV-positive patients or patients under immunosuppressive therapy. Even though musculoskeletal manifestations occur in only about 2% of tuberculosis cases, they can be very heterogeneous and present as spondylitis, osteomyelitis or mono-oligo-polyarthritis. Because of the potential for disastrous outcomes in cases where the diagnosis is delayed, it is important to recognize

Thirty-two knee and 23 sternoclavicular joints from 35 subjects with acquired immunodeficiency syndrome were examined prospectively at necropsy. There were two instances of opportunistic infectious arthritis: one caused by Staphylococcus aureus, the other by Sporothrix schenckii. In five other subjects, para-articular bone was infiltrated by granulomatous or neoplastic sequelae of the human immunodeficiency virus infection. There was no immunohistochemical (p24 antigen) or other evidence for existence of a specific acquired immunodeficiency syndrome arthritis.

One patient with psoriatic arthritis and a second with rheumatoid arthritis were each treated with low-dose methotrexate (10 to 15 mg per week, orally) for more than a year before pulmonary cryptococcosis developed. These are the first case reports of opportunistic fungal disease during this treatment. Low-dose methotrexate may be more immunosuppressive than has been appreciated. Patients demonstrating pneumonitis while receiving this therapy should be carefully evaluated to rule out an infectious cause.

Understanding of the genetic basis of autoimmune diseases is currently incomplete. Cytokine gene polymorphisms warrant consideration as factors explaining variation in the human immune and inflammatory responses and as candidate susceptibility genes for related pathological states. Interleukin 12 (IL-12) is a key regulator of the polarisation of immune responses to T helper 1 or 2 categories and plays a role in autoimmune and infectious diseases. Using a bioinformatic strategy, we aligned cDNA and expressed sequence tag sequences to identify putative polymorphic regions of the IL-12 p40 gene.

BACKGROUND: Immune-mediated conditions associated to Corona Virus Disease-19 (COVID-19) have been reported, including vasculitis, antiphospholipid antibody syndrome, myositis, and lupus. Emerging studies have reported the potential occurrence of reactive arthritis in patients previously infected with COVID-19. This systematic review summarised the current evidence on the occurrence of reactive arthritis in patients previously infected by COVID-19.
METHODS: This study was conducted according to the 2020 PRISMA guidelines. All the clinical investigations describing the occurrence of reactive ar

Reiter's disease (RD) is a reactive arthritis syndrome in which arthritis, conjunctivitis, dermatitis, and urethritis are the main features. It may be an acute and self-limited disease or it may cause severe debilitation. It generally follows an infectious episode, such as Shigella dysentery or nonspecific urethritis. Recent evidence has shown that patients in whom RD develops have a genetic disposition since the presence of HLA-B27 correlates highly with RD. Therapy of RD involves supportive care.

We report on pediatric patient with Nijmegen breakage syndrome (NBS), a rare DNA repair disorder characterized by microcephaly, immunodeficiency and predisposition to malignant lymphomas, who developed juvenile idiopathic arthritis (JIA)-like polyarthritis. In patients with primary immunodeficiencies (PID), septic arthritis due to pyogenic bacteria or mycoplasmal arthritis are the most common osteoarticular manifestations. In certain PID, chronic, non-infectious arthritis resembling rheumatoid arthritis may occur. In our patient microbiologic cultures of synovial fluid including Mycoplasma spp
