Painful Acute Corneal Ulceration and Anterior Stromal Keratitis in an Adolescent With Reactive Arthritis.

Reactive arthritis is occasionally associated with conjunctivitis and uveitis, but rarely keratitis. The authors describe a patient with paracentral anterior stromal keratitis with overlying epithelial erosion and multiple satellite subepithelial infiltrates. The initial appearance and the severe pain were suggestive of an infectious process. Recovery was rapid following introduction of topical steroid, with features suggesting an immune process in the recovery phase. The authors suggest that a history of arthritis and/or recent enteritis should be sought in the work-up of an atypical keratiti

Rheumatoid Arthritis: Trends in Antirheumatic Drug Use, C-reactive Protein Levels, and Surgical Burden.

OBJECTIVE: Over the past decade, the therapeutic approach used to treat patients with rheumatoid arthritis (RA) has considerably changed. It remains unclear whether these changes have been accompanied by decreased disease severity and surgical treatment burden at the population level. Therefore, we investigated time trends in antirheumatic drug consumption, C-reactive protein (CRP) levels, and use of orthopedic surgery among Danish patients with RA. METHODS: Using medical databases, we identified all patients with RA living in Northern Denmark during 1996-2012. For each calendar year, we comp

Disease flare of ankylosing spondylitis presenting as reactive arthritis with seropositivity: a case report.

INTRODUCTION: Concurrent rheumatoid factor seropositivity is occasionally detected in ankylosing spondylitis and often causes confusion in clinical routine. Overlap between various seronegative arthritides is a known but uncommon association. Differentiation of spondyloarthropathy from rheumatoid arthritis is important, since the natural history, complications, treatments and prognosis of the two diseases differ significantly. CASE PRESENTATION: Here, we report the case of a 47-year-old Sri Lankan man who had a long history of intermittent joint pains worsening following a recent episode of s

Heat shock protein 60 reactive T cells in juvenile idiopathic arthritis: what is new?

Juvenile idiopathic arthritis (JIA) is a disease characterized by chronic joint inflammation, caused by a deregulated immune response. In patients with JIA, heat shock proteins (HSPs) are highly expressed in the synovial lining tissues of inflamed joints. HSPs are endogenous proteins that are expressed upon cellular stress and are able to modulate immune responses. In this review, we concentrate on the role of HSPs, especially HSP60, in modulating immune responses in both experimental and human arthritis, with a focus on JIA. We will mainly discuss the tolerogenic immune responses induced by H

Antiinflammatory effects of etoricoxib alone and combined with NSAIDs in LPS-induced reactive arthritis.

OBJECTIVE: Nonsteroidal anti-inflammatory drugs constitute the primary therapeutic approach in reactive arthritis. Here, we compared etoricoxib, a specific COX-2 inhibitor, with other cyclooxygenase inhibitors on articular incapacitation, edema, leukocyte migration, and gastric damage, in a model of LPS-induced reactive arthritis in rats. METHODS: E. coli Lipopolysaccharide was injected into a carrageenan-primed knee-joint of rats. The effects of etoricoxib, piroxicam, indomethacin, as well the combination of etoricoxib either with piroxicam or indomethacin, were evaluated on articular incapa

Emergence of Salmonella Blockley, possible association with long-term reactive arthritis, and antimicrobial resistance.

We previously reported the case of a patient who became ill with Salmonella serotype Blockley after eating a chicken meal and who was still suffering symptoms of salmonella-reactive arthritis 10 years after the initial infection. The immunological consequences of infection were of concern and prompted a literature review because of the possibility that Salmonella Blockley may be more likely to cause long-term illness in certain patients than more common serovars. The review revealed the additional cause for concern of plasmid-borne extended spectrum beta-lactamases and the high frequency of in

Spontaneous pneumoperitoneum: an unusual complication of systemic reactive AA amyloidosis secondary to rheumatoid arthritis.

We report a 71-year-old man with reactive AA amyloidosis secondary to rheumatoid arthritis who developed spontaneous pneumoperitoneum with intestinal pseudo-obstruction as an initial symptom. Severe deposition of amyloid in the intestinal wall was considered to play an important role in the pathogenesis of this unusual symptom. The patient has been successfully treated with total parenteral alimentation and intermediate-dose prednisolone (30 mg/day). Although pneumoperitoneum usually suggests gastrointestinal perforation requiring emergency surgery, conservative therapy should be seriously con

Reactive arthritis induced by Clostridium difficile enteritis as a complication of Helicobacter pylori eradication.

Clostridium difficile has recently been established as a cause of reactive arthritis (ReA). We present a case of Clostridium difficile-induced ReA as a complication of Helicobacter pylori eradication, which, to the best of our knowledge, is the first such case reported.

Synovial membrane cytokine profiles in reactive arthritis secondary to intravesical bacillus Calmette-Guérin therapy.

We describe the cellular infiltrate and cytokine profile in sequential synovial membrane biopsies from a patient with acute followed by chronic synovitis after intravesical bacillus Calmette-Guérin (BCG) therapy for an in situ transitional cell carcinoma of the bladder. Histological and immunohistochemical analysis of 3 synovial biopsies were done sequentially over a 9 month period. The patient was HLA-B27 positive, but HLA-DR4 negative, and did not have the "shared epitope." Unlike other cases, this patient's arthritis did not respond initially to nonsteroidal antiinflammatory drugs and was e

Antibodies against cell envelope antigens of Yersinia enterocolitica in reactive arthritis and ankylosing spondylitis.

Yersinia enterocolitica is suspected of involvement in the aetiology of reactive arthritis and ankylosing spondylitis. Its two major outer membrane proteins are important targets for IgA and other protein A reactive immunoglobulins in the sera of some patients with these diseases.

The clinical features and HLA associations of reactive arthritis associated with non-gonococcal urethritis.

Fifty-seven patients with arthritis associated with non-gonococcal genital infection have been studied. Synovitis characteristically affected one or a few joints, expecially the knee, ankle or metatarsophalangeal joints and was accompanied by tenosynovitis and enthesopathies--each in about one third of the patients. A quarter of the patients had ocular, cutaneous, or mucous membrane lesions (Reiter's syndrome). Although six patients developed a chronic or relapsing course, average duration of the acute episode in the majority was three to five months. Available evidence strongly suggests that

Serum antiimmunoglobulins reactive with human and rabbit IgG in rheumatoid arthritis and other conditions.

IgG, IgA, and IgM antiimmunoglobulins reactive with human and rabbit IgG were measured in patients with rheumatoid arthritis (RA), patients with rheumatic fever or osteoarthritis, and normal individuals. Values of all antiimmunoglobulins were significantly evaluated in RA patients as compared with other groups and depended upon activity and stage of the disease. IgM antibodies with specificity for human IgG predominated quantitatively over others in sera of RA patients with high titers of RF, whereas most of those reactive with rabbit IgG in latex negative or positive RA patients belonged to I

Familial reactive arthritis of Reiter's and ankylosing spondylitis types in the HLA-27 genotype.

The close relationship between Reiter's disease and ankylosing spondylitis, which had previously been suspected on the grounds of family and population studies, has been re-emphasised following the recent demonstration of their common close association with the HLA-27 genotype. Nevertheless there are few case reports of both conditions occurring simultaneously in first degree relatives; and documentation of HLA genotype in such families was not generally considered before 1973. Both varieties of reactive arthritis are reported here as occurring in first degree relatives sharing HLA-27 genotype

Bilateral Knee Infectious Arthritis Caused by Proteus mirabilis Mimicking Reactive Arthritis in an Elderly Patient with Metastatic Cancer: A Case Report.

BACKGROUND: Infectious arthritis is an uncommon but serious condition in older adults and may be easily misdiagnosed due to atypical clinical presentations. Proteus mirabilis is a rare causative pathogen of infectious arthritis, and its manifestations can resemble reactive arthritis, particularly in elderly patients with malignancy or immunocompromised status. CASE PRESENTATION: A 74-year-old woman with advanced endometrial cancer was admitted with fever and urinary tract infection caused by Proteus mirabilis, complicated by septic shock. After initial improvement with intravenous antibiotic

On the difficulties of establishing a consensus on the definition of and diagnostic investigations for reactive arthritis. Results and discussion of a questionnaire prepared for the 4th International Workshop on Reactive Arthritis, Berlin, Germany, July 3-6, 1999.

OBJECTIVE: There is no agreement on how to classify and diagnose reactive arthritis (ReA) and it is also unclear what kind of specific clinical and laboratory investigations are appropriate. We define relevant points of agreement and identify points of disagreement among an international group of experts in the field. METHODS: Prior to the 4th International Workshop on Reactive Arthritis, Berlin, July 1999, we sent questionnaires to 42 experts identified by personal knowledge and recent publications. RESULTS: The response rate was 81% (n = 34). There was agreement on the nomenclature and rec

Expanding the spectrum of reactive arthritis (ReA): classic ReA and infection-related arthritis including poststreptococcal ReA, Poncet's disease, and iBCG-induced ReA.

Reactive arthritis (ReA) is a form of sterile arthritis that occurs secondary to an extra-articular infection in genetically predisposed individuals. The extra-articular infection is typically an infection of the gastrointestinal tract or genitourinary tract. Infection-related arthritis is a sterile arthritis associated with streptococcal tonsillitis, extra-articular tuberculosis, or intravesical instillation of bacillus Calmette-Guérin (iBCG) therapy for bladder cancer. These infection-related arthritis diagnoses are often grouped with ReA based on the pathogenic mechanism. However, the uniqu

Synovial fluid and serum antibodies against Chlamydia in different forms of arthritis: intra-articular IgA production in Chlamydia sexually acquired reactive arthritis.

Since the presence of Chlamydia has been shown in synovial fluid (SF) from some patients with Chlamydia reactive arthritis, we investigated whether anti-Chlamydia antibodies present in the joint are derived from the circulation or are locally produced. We compared titres of IgG, IgM and IgA antibodies against Chlamydia, and against a control antigen (tetanus toxoid), by an enzyme-linked immunosorbent assay (ELISA), in paired samples of serum and SF from Chlamydia trachomatis sexually acquired reactive arthritis (CT-SARA) patients and from patients with other forms of arthritis. Incidence and prevalence of psoriatic arthritis, ankylosing spondylitis, and reactive arthritis in the first descriptive population-based study in the Czech Republic.

OBJECTIVE: To estimate the annual incidence and prevalence of psoriatic arthritis (PsA), ankylosing spondylitis (AS), and reactive arthritis (ReA) in a sample of the Czech population. METHODS: The population-based study was conducted in two regions of the Czech Republic (with a total population of 186000 inhabitants) in 2002-2003. Incident cases were registered on condition of confirming a definite diagnosis according to existing classification criteria during the study period (1 March 2002 to 1 March 2003). Prevalence was studied on the basis of identification of established diagnoses (befor
