Predictors of mortality among newborns admitted with perinatal asphyxia at public hospitals in Ethiopia: a prospective cohort study.

INTRODUCTION: Perinatal asphyxia is a complicated newborn health problem and applies a high contribution to the increased proportion of newborn mortality. It occurs in newborns due to altered breathing or inadequate inhalation and exhalation resulting in reduced oxygen perfusion to certain body tissues and organs. Irrespective of the increased progress in health care towards newborns and implementations in reductions in under-five, infant, and neonatal mortality in the past 10 years, perinatal asphyxia remained as the most common severe newborn health challenge that causes a high number of mor

Comparison of intravenous and intraosseous administration of vasopressin and epinephrine during cardiopulmonary resuscitation of asphyxiated neonatal piglets.

BACKGROUND: Epinephrine is currently the only vasopressor recommended during neonatal cardiopulmonary resuscitation (CPR). Previous neonatal animal studies suggested intravenous (IV) vasopressin as a potential alternative; however, no neonatal study has compared the efficacy of intraosseous (IO) with IV vasopressin to epinephrine administration during CPR. DESIGN/METHODS: Newborn piglets (n = 8/group) were anesthetized, intubated, surgically instrumented, and exposed to normocapnic hypoxia followed by asphyxia and asystole. Piglets were resuscitated following randomization to 0.4IU/kg IV or I

A comparative analysis of APGAR score and the gold standard in the diagnosis of birth asphyxia at a tertiary health facility in Kenya.

BACKGROUND: Birth asphyxia is a consistent key contributor to neonatal morbidity and mortality, notably in sub-Saharan Africa. The APGAR score, though a globally used diagnostic tool for birth asphyxia, remains largely understudied especially in resource-poor settings. OBJECTIVE: This study determined how effectively the APGAR score is used to diagnose birth asphyxia in comparison to the gold standard (umbilical cord blood pH <7 with neurologic involvement) at Moi Teaching and Referral Hospital (MTRH), and identified healthcare provider factors that affect ineffective use of the score. METHO

Bumetanide potentiates the anti-seizure and disease-modifying effects of midazolam in a noninvasive rat model of term birth asphyxia.

Birth asphyxia and the resulting hypoxic-ischemic encephalopathy (HIE) are highly associated with perinatal and neonatal death, neonatal seizures, and an adverse later-life outcome. Currently used drugs, including phenobarbital and midazolam, have limited efficacy to suppress neonatal seizures. There is a medical need to develop new therapies that not only suppress neonatal seizures but also prevent later-life consequences. We have previously shown that the loop diuretic bumetanide does not potentiate the effects of phenobarbital in a rat model of birth asphyxia. Here we compared the effects o

Long-term outcome in a noninvasive rat model of birth asphyxia with neonatal seizures: Cognitive impairment, anxiety, epilepsy, and structural brain alterations.

OBJECTIVE: Birth asphyxia is a major cause of hypoxic-ischemic encephalopathy (HIE) in neonates and often associated with mortality, neonatal seizures, brain damage, and later life motor, cognitive, and behavioral impairments and epilepsy. Preclinical studies on rodent models are needed to develop more effective therapies for preventing HIE and its consequences. Thus far, the most popular rodent models have used either exposure of intact animals to hypoxia-only, or a combination of hypoxia and carotid occlusion, for the induction of neonatal seizures and adverse outcomes. However, such models

Significance of cervical ripening in pre-induction treatment for premature rupture of membranes at term.

AIM: This study aimed to determine whether mechanical cervical dilatation with a laminaria tent in women with premature rupture of membranes … p B. No significant differences were found in the Apgar scores, umbilical artery pH or frequency of asphyxia neonatorum between the groups. Mechanical cervical dilatation by laminaria tent insertion neither increased the incidence of infection nor contributed to improvement of the perinatal prognosis. CONCLUSION: Mechanical cervical dilatation does not provide any b …

[Prognosis of neonatal hypoxic-ischemic encephalopathy: diagnostic methods and treatment].

Hypoxic-ischemic encephalopathy results from perinatal asphyxia and manifests through severe metabolic acidosis or neonatal depression requiring resuscitation. Since this remains a significant cause of mortality and neurodevelopmental disorders such as cerebral palsy and epilepsy, a multimodal evaluation integrating clinical, biochemical, neurophysiological, and neuroimaging parameters is required to determine the prognosis. In terms of clinical assessment, various scales can be applied, these are especially helpful in predicting the risk of adverse outcomes. The gold standard treatment for mo

Incidence of neonatal asphyxia and associated risk factors: a systematic review and cumulative meta-analysis.

OBJECTIVES: To analyze the incidence of neonatal asphyxia and its associated related risk factors and provide reference for reducing the incidence and mortality of neonatal asphyxia. METHODS: PubMed, Web of Science, CNKI, and Wanfang were searched for cross-sectional/case-control studies (inception-December 31, 2024). After dual-independent screening (PROSPERO: CRD420251118897), 18 studies (n = 737,839 neonates) were included. Quality was assessed using AHRQ. Random/fixed-effects models were applied based on I² thresholds. Temporal trends were explored via sequential meta-analysis by publicat

[Early prediction of neurodevelopmental outcomes in neonatal hypoxic-ischemic encephalopathy].

Perinatal asphyxia can result in severe complications in neonates, with hypoxic-ischemic encephalopathy (HIE) being one of the most serious consequences. While whole-body hypothermia has reduced both mortality and neurodevelopmental impairment, significant challenges remain in clinical practice. One of the most pressing issues is the early prediction of long-term outcomes, which is crucial during parental consultations. Relying solely on clinical data from the first few days of life makes it difficult to accurately predict neurodevelopmental trajectories and recommend timely interventions. In

Asphyxia due to substandard obstetric care in Finland: An 11-year patient insurance registry study of compensated patient claims.

BACKGROUND: Asphyxia is a major cause of neonatal mortality, often linked to inadequate perinatal care, especially misinterpretation of cardiotocography. In Finland, the Patient Insurance Centre manages medical claims, offering a no-fault compensation model. Utilizing these claims could enhance obstetric safety and newborn health. AIM: The aim of this study was to analyze the compensated patient claims and identify the substandard care leading to asphyxia. METHODS: A nationwide retrospective registry-based study was conducted. Data included all compensated patient insurance claims for neonat

Early neonatal deaths with perinatal asphyxia of newborns ≥ 37-weeks in Brazil, 2000‒2020.

OBJECTIVE: To evaluate temporal and geographical trends of early Neonatal Mortality Rate (NMR) associated with perinatal asphyxia in newborns ≥ 37-weeks' gestation without congenital anomalies in Brazil, from 2000 to 2020. METHOD: The database was built based on live birth and death certificates, according to the municipality where birth occurred. Perinatal asphyxia was considered if ICD-10 codes were present in any line of the death certificate: intrauterine hypoxia (P20), birth asphyxia (P21) or neonatal aspiration of meconium (P24.0). Prais-Winsten method tested the temporal trend of early

Recent Progress in Neonatal Global Health Quality Improvement.

Quality improvement methodologies, coupled with basic neonatal resuscitation and essential newborn care training, have been shown to be critical ingredients in improving neonatal mortality. Innovative methodologies, such as virtual training and telementoring, can enable the mentorship and supportive supervision that are essential to the continued work of improvement and health systems strengthening that must be done after a single training event. Empowering local champions, building effective data collection systems, and developing frameworks for audits and debriefs are among the strategies th

Neonatal resuscitation practices in Uganda: a video observational study.

BACKGROUND: Neonatal mortality, often due to birth asphyxia, remains stubbornly high in sub-Saharan Africa. Guidelines for neonatal resuscitation, where achieving adequate positive pressure ventilation (PPV) is key, have been implemented in low-resource settings. However, the actual clinical practices of neonatal resuscitation have rarely been examined in these settings. The primary aim of this prospective observational study was to detail the cumulative proportion of time with ventilation during the first minute on the resuscitation table of neonates needing PPV at the Mulago National Referra

Experiences of community midwives receiving helping baby breathe training through the low dose high-frequency approach in Gujrat, Pakistan.

BACKGROUND: Pakistan's neonatal mortality rate has the highest proportion in the South Asian region and it is higher in the rural areas as compared to the urban areas. Poor resuscitation techniques and lack of basic newborn resuscitation skills in birth attendants are contributing factors towards neonatal deaths. Based on the significant outcomes of the Helping Baby Breath (HBB) training, similar training was implemented for Community Midwives (CMWs) in a low-resource setting in Gujrat, Pakistan, to improve their knowledge and skills. The training evaluation was conducted and participant feedb

Neonatal Encephalopathy: Beyond Hypoxic-Ischemic Encephalopathy.

Neonatal encephalopathy is a clinical syndrome of neurologic dysfunction that encompasses a broad spectrum of symptoms and severity, from mild irritability and feeding difficulties to coma and seizures. It is vital for providers to understand that the term "neonatal encephalopathy" is simply a description of the neonate's neurologic status that is agnostic to the underlying etiology. Unfortunately, hypoxic-ischemic encephalopathy (HIE) has become common vernacular to describe any neonate with encephalopathy, but this can be misleading. The term should not be used unless there is evidence of pe

Neonatal encephalopathy: Focus on epidemiology and underexplored aspects of etiology.

Neonatal Encephalopathy (NE) is a neurologic syndrome in term and near-term infants who have depressed consciousness, difficulty initiating and maintaining respiration, and often abnormal tone, reflexes and neonatal seizures in varying combinations. Moderate/severe NE affects 0.5-3/1000 live births in high-income countries, more in low- and middle-income countries, and carries high risk of mortality or disability, including cerebral palsy. Reduced blood flow and/or oxygenation around the time of birth, as with ruptured uterus, placental abruption or umbilical cord prolapse can cause NE. This s
