Descending Aorta Perforation During Transseptal Puncture for Left Atrial Appendage Closure.

BACKGROUND: Aortic root injury during transseptal puncture (TSP) is a rare but well-recognized complication. However, injury to the descending aorta has not previously been described in this context. CASE SUMMARY: A 57-year-old woman underwent left atrial appendage closure. During TSP using the VersaCross Connect radiofrequency-powered system and a 15-F steerable sheath, an unrecognized perforation of the descending aorta occurred. Despite repeated transthoracic echocardiograms showing no pericardial effusion and initial hemodynamic stability, she subsequently developed a mediastinal hematoma…

Left atrial appendage occluder thrombosis caused embolic strokes: a case report.

BACKGROUND: Left atrial appendage (LAA) occlusion is an alternative for stroke prevention in atrial fibrillation (AF) patients who are unsuitable for long-term oral anticoagulation due to bleeding risks. While intended to reduce haemorrhagic stroke, LAA occluder devices can rarely lead to thrombus formation and subsequent embolic events. CASE SUMMARY: A 75-year-old woman with paroxysmal AF and cerebral amyloid angiopathy had an LAA occluder implanted due to concerns for high bleeding risk. She presented with mild left leg weakness; magnetic resonance imaging revealed embolic strokes in the le…

Direction-Controlled Retrieval of an Embolized Left Atrial Appendage Closure Device.

BACKGROUND: Embolization of left atrial appendage (LAA) closure devices is an uncommon but potentially serious complication. Conventional snaring techniques may not provide adequate control of device orientation, increasing the risk of anchor entrapment and vascular injury. CASE PRESENTATION: A 67-year-old man with persistent atrial fibrillation and recurrent colonic diverticular bleeding underwent LAA closure using a 31-mm WATCHMAN FLX Pro device. Despite satisfactory positioning based on the PASS (Position, Anchor, Size, Seal) criteria, device embolization to the aortic arch occurred shortl…

Left Atrial Appendage Closure: Between Imaging Precision and Uncertain Clinical Benefit.

Left atrial appendage closure has emerged as an established nonpharmacological strategy for stroke prevention in selected patients with atrial fibrillation, particularly those in whom long-term oral anticoagulation is problematic. Its contemporary role, however, is defined by a central tension between procedural precision and clinical uncertainty. Advances in multimodality imaging, including transesophageal echocardiography, cardiac computed tomography, three-dimensional echocardiography, fusion imaging, and emerging computational tools, now allow highly accurate assessment of left atrial appe…

Acute changes in left atrial appendage function with premature ventricular complexes.

OBJECTIVES: Left atrial appendage (LAA) dysfunction is a risk factor for stroke. Evidence shows that frequent premature ventricular complexes (PVCs) are associated with embolic stroke. Whether left atrial dysfunction is the bridging link between frequent premature ventricular complexes and stroke is unknown. MATERIALS AND METHODS: Patients with a structurally normal heart undergoing elective electrophysiology study were included. Transoesophageal echo was used to measure LAA flow velocities. To simulate PVCs in bigeminal rhythm, single paced beats were delivered from the right ventricle with …

Very delayed hemorrhagic pericardial effusion following percutaneous left atrial appendage occlusion.

UNLABELLED: Pericardial effusion is a serious complication following percutaneous left atrial appendage occlusion and is associated with adverse outcomes. While acute effusions occur from procedure associated macro-perforations, delayed effusions are less well understood. Reported here is a case of a hemorrhagic pericardial effusion presenting almost 6 months following left atrial appendage occlusion and successfully treated with corticosteroids. LEARNING OBJECTIVE: Delayed pericardial effusions following left atrial appendage occlusion occur due to microperforations. These pericardial effusi…

Zero-fluoroscopy left atrial appendage occlusion with the Watchman FLX device.

BACKGROUND: Percutaneous left atrial appendage occlusion (LAAO) is conventionally performed under fluoroscopic guidance. OBJECTIVE: This study aimed to evaluate the feasibility and safety of performing LAAO with the Watchman FLX device without the use of fluoroscopy. METHODS: Since June 15, 2023, we have been performing LAAO procedures with the Watchman FLX device without the use of fluoroscopy. The procedures were performed under general anesthesia with transesophageal echocardiographic imaging. The key safety endpoint was the occurrence of death, ischemic stroke, systemic embolism, open he…

Safety review of antithrombotic therapy options after left atrial appendage occlusion.

INTRODUCTION: Left atrial appendage occlusion (LAAO) is a viable alternative to anticoagulation for treatment in patients with non-valvular atrial fibrillation (NVAF) who cannot tolerate anticoagulation. Post-procedure patients are generally prescribed oral anticoagulation (OAC) for 45 days, while the device is undergoing endothelialization, following which patients are continued on antiplatelet agents. Recommendations for antithrombotic agents following LAAO arrived by consensus, which are not tolerated by all patients. AREAS COVERED: This review covers the safety profile of antithrombotic t…

Left atrial appendage closure: Current status, unresolved issues, and future perspectives.

Left atrial appendage closure (LAAC) is an established therapy for stroke prevention in patients with nonvalvular atrial fibrillation (AF) who cannot tolerate long-term oral anticoagulation (OAC). In Japan, LAAC was introduced in 2019 and has been performed in more than 10,000 cases a0s of 2025. Although indications remain limited to high-risk patients with OAC contraindications, procedural volume continues to rise. This review outlines the current status, remaining challenges, and future perspectives of LAAC, with emphasis on the WATCHMAN device. The WATCHMAN FLX and FLX Pro have enhanced pro…

Left atrial appendage occlusion: Percutaneous and surgical approaches in everyday practice.

Prophylactic left atrial appendage occlusion has been suggested as a means of reducing cardioembolism risk in patients with atrial fibrillation. Its clinical benefits have been discussed together with potential endocrine or hemodynamic adverse effects, with conflicting conclusions. We aimed to provide a thorough overview of the current literature and a recommendation for daily clinical decision-making. A comprehensive Medline search through PubMed was conducted to search for relevant articles, which were further filtered using the title and abstract. Sixty-five articles were selected as releva…

Intracardiac echocardiography-guided left atrial appendage occlusion: The path to independence.

INTRODUCTION AND OBJECTIVES: Transesophageal echocardiography (TEE) has been the standard method for guiding left atrial appendage occlusion (LAAO) procedures. Recently, intracardiac echocardiography (ICE) has emerged as an alternative to TEE due to several advantages, particularly the avoidance of general anesthesia. This analysis aims to assess the safety, feasibility and efficacy of ICE-guided LAAO procedures. METHODS: We performed a retrospective analysis of ICE-guided LAAO procedures, including a comparison of embolic and bleeding events with the predicted standard scores and a compariso…

Nonoperative Management of Iatrogenic Right Atrial Appendage Pseudoaneurysm During Catheter Ablation.

Atrial pseudoaneurysms are exceedingly rare. Cardiac pseudoaneurysms are at risk for rupture, which may be catastrophic and require emergent thoracotomy for definitive treatment. We report a case of right atrial appendage perforation during catheter ablation leading to tamponade and right atrium pseudoaneurysm.

Right atrial appendage rupture and cardiac tamponade secondary to blunt trauma.

BACKGROUND: Right atrial appendage rupture from blunt trauma is exceedingly rare, even more so when no other chest wall injuries are found. Very few cases have been documented with respect to survival from such an injury. PURPOSE: To highlight the optimal management of such cases, namely through timely and safe transport to a trauma centre, maintaining a high degree of clinical suspicion for tamponade, early diagnostic ultrasound use, pericardial decompression, haemorrhage control and situational control. CASE PRESENTATION: A case report delineating the diagnostic and therapeutic approach to…

Left atrial appendage occlusion with Watchman device after incomplete surgical ligation.

Non-valvular atrial fibrillation (NVAF) is the most common cause of cardioembolic stroke. The left atrial appendage (LAA) is the major source of cardiac emboli in patients with NVAF. Anticoagulation (AC) is the standard of care for stroke prevention in atrial fibrillation (AF), but many patients are intolerant of AC. Surgical exclusion of the LAA may result in incomplete closure and is associated with an increased risk of embolism. We report a case of a woman in her 50s with a history of persistent AF, mitral valve prolapse s/p repair with surgical LAA exclusion, and multifocal haemorrhagic st…

[CT Image-based Surgery Assist System for Left Atrial Appendage Occlusion].

At present, the standard left atrial appendage occlusion procedure mainly involves two-dimensional imaging methods such as X-ray fluoroscopy and transesophageal echocardiography to guide the operation, which will lead to underestimation of the three dimensional structure of the left atrial appendage and the surrounding tissue, thus adversely affects the surgery. To solve this problem, a surgery assist system for left atrial appendage occlusion based on preoperative cardiac CT images is developed. The proposed system realizes the left atrial appendage parameter measurement based on cardiac CT i…

Left Atrial Appendage Ostial Stenosis: A Case Report and Literature Review.

BACKGROUND Left atrial appendage (LAA) ostial stenosis is a rare cardiac condition usually identified by transesophageal echocardiography (TEE). LAA ostial stenosis can be classified into 2 types: one is idiopathic and the other is a complication after incomplete surgical LAA ligation. The former one is even rarer and only 12 cases have previously been reported. CASE REPORT An asymptomatic young woman was found at a medical checkup to have an abnormal jet signal in the left atrium on transthoracic echocardiography. TEE revealed that the abnormal signal originated from a narrowed orifice of the…
