Intravenous propafenone for termination of reentrant supraventricular tachycardia. A placebo-controlled, randomized, double-blind, crossover study.

To assess the antiarrhythmic efficacy of intravenous propafenone, 20 patients with inducible sustained supraventricular tachycardia received … o (p less than 0.01). Propafenone prolonged refractoriness and slowed conduction of the atrium, the atrioventricular node, and accessory atrioventricular bypass tracts, and these effects provided antiarrhythmic action to halt tachycardia. No adverse effects were observed in any patient. We conclude that intravenous propafenone is safe and effective in the acute treatme …

Supraventricular Tachycardia in Adult Congenital Heart Disease: Mechanisms, Diagnosis, and Clinical Aspects.

Supraventricular arrhythmias represent a major source of morbidity in adults with congenital heart disease (ACHD). Anatomic variants and post-operative changes contribute to a unique electrophysiologic milieu ripe for the development of supraventricular tachycardia. Intra-atrial reentrant tachycardia is the most prevalent mechanism. Atrioventricular reciprocating tachycardia is common in lesions associated with accessory pathways. Abnormal anatomy complicates the management of atrioventricular nodal reentrant tachycardia. Tachycardia mediated by twin atrioventricular nodes is rare. Focal tachy…

Multifocal Purkinje-like tumor of the heart. Occurrence with other anatomic abnormalities in the atrioventricular junction of an infant with junctional tachycardia, Lown-Ganong-Levine syndrome, and sudden death.

A 13-month-old boy with intermittent preexcitation (Lown-Ganong-Levine type) and paroxysmal tachycardias of 280 to 300 beats per minute died suddenly with cardiac arrest. At necropsy the only abnormalities were in the heart. There were multifocal Purkinje cell tumors in the conduction system, including one directly at the bifurcation of the His bundle. In addition, a fault in the central fibrous body was filled with an accessory communication between the mid-portion of the atrioventricular node and both the interventricular septum and the His bundle. This accessory communication connected with…

Diagnosis and Management of Common Types of Supraventricular Tachycardia.

Supraventricular tachycardia refers to rapid rhythms that originate and are sustained in atrial or atrioventricular node tissue above the bundle of His. The condition is caused by reentry phenomena or automaticity at or above the atrioventricular node, and includes atrioventricular nodal reentrant tachycardia, atrioventricular reciprocating tachycardia, and atrial tachycardia. Most persons with these tachyarrhythmias have structurally normal hearts. Sudden onset of an accelerated heart rate can cause palpitations, light-headedness, chest discomfort, anxiety, dyspnea, or fatigue. The history is…

Wide QRS-complex tachycardia with variable VA-conduction: what is the mechanism?

We describe the case of a 16-year-old woman with a surgically corrected tetralogy of Fallot presenting with recurrent wide-QRS-complex tachy … iled mapping showed cranial to caudal depolarization of the His bundle, leading to the diagnosis of atrioventricular node re-entrant tachycardia. The tachycardia was abolished by radiofrequency catheter ablation of the slow AV nodal pathway. We conclude that variable VA conduction can occur in patients with atrioventricular node re-entrant tachycardia. The atrial tissu …

Symptomatic sick sinus syndrome in children and adolescents as the only manifestation of cardiac abnormality or associated with unoperated congenital heart disease.

Sick sinus syndrome (SSS) occurs infrequently in children who have not undergone cardiac surgery. The symptoms, electrocardiograms, and elec … th nonsurgical SSS may have abnormalities not only of the sinus node but also of the atrium and the atrioventricular node, it is recommended that patients with symptomatic SSS be evaluated by electrophysiologic study. The proper choice of antiarrhythmic drug therapy or permanent pacing procedure depends on a complete analysis of the cardiac conduction system.

Clinical outcomes of leadless pacemaker: a systematic review.

INTRODUCTION: Transvenous pacemakers are associated with a significant amount of complications. Leadless pacemakers (LP) are emerging as an … as procedure or device related. Four studies presented the strategy of using a combined approach of atrioventricular node ablation (AVNA) and LP implantation. CONCLUSIONS: Leadless pacemakers seem to have a relatively low complication rate. Mesothelioma of the AV node.

A 21-month-old boy died suddenly six months after presenting with heart murmur and second-degree atrioventricular (AV) block. Autopsy revealed a mesothelioma completely replacing the AV node and extending into the central fibrous body. The bundle of His and bundle branches were not involved. Mesothelioma of the AV node is a rare cause of supra-His (AV) block, arrhythmias, and sudden death.

[Sinus node dysfunction associated with cardiac sarcoidosis: A case report].

INTRODUCTION: Sarcoidosis is a multisystemic granulomatous disease of unknown cause occurring in young adults. Cardiac sarcoidosis patients are at increased risk for atrioventricular blocks and ventricular arrhythmias. Sinus node dysfunction is scarcely reported. OBSERVATION: We report a case of cardiac sarcoidosis revealed by a sinus node dysfunction and focus on cardiac and thoracic imaging to guide diagnosis. CONCLUSION: Sinus node dysfunction may be the first manifestation of cardiac sarcoidosis. In unexplained sinus node dysfunction in young patients, advanced cardiac imaging is a key t

New insights into atrioventricular nodal anatomy, physiology, and immunochemistry: A comprehensive review and a proposed model of the slow-fast atrioventricular nodal reentrant tachycardia circuit in agreement with direct potential recordings in the Koch's triangle area.

Atrioventricular nodal reentrant tachycardia (AVNRT) is the most frequent regular tachycardia in humans. In this review, we describe the mos … ring slow-fast AVNRT. Further studies and technical improvements in recording the potentials of the atrioventricular node structures are necessary to confirm our initial results.

AV dissociation, an inevitable response.

BACKGROUND: The independent activation of the atria and ventricles, AV dissociation, is a common phenomenon that occurs during a wide variety of electrophysiologic circumstances. The clinical significance of AV dissociation is often misunderstood. METHODS: This article examines the basis and clinical implications of AV dissociation. RESULTS: AV dissociation is often an obligatory, secondary phenomenon, and should not be construed as the primary disorder; it may be due to either the AV conduction system being completely blocked (3° AV block) or the P wave and the QRS complex being generated f

[Pathophysiologic principles of ventricular late potentials--mechanisms].

The majority of sustained ventricular tachycardias are caused by a reentry mechanism. Conditions prerequisite to a reentry circuit are electrophysiological changes, namely conduction delay and block which are incurred by virtue of changes in the myocardial tissue due to scarring, ischemia or fatty infiltration and result in an abnormal action potential, increased intracellular resistance and reduction in intercellular coupling (intercalated discs). Additionally, the principle of anisotropy, that is, the varying velocity of conduction parallel and perpendicular to the fiber axis (increased axia

Nonreentrant atrioventricular nodal tachycardia: The great simulator.

We present the case of a healthy 12-year-old boy without structural heart disease who was followed for several years for an arrhythmia with diverse and incorrect initial diagnoses, refractory to flecainide, metoprolol and verapamil. Thorough reevaluation of the electrocardiographic recordings led to a presumptive diagnosis of a non-reentrant atrioventricular nodal tachycardia, subsequently confirmed with electrophysiology study. Radiofrequency ablation was performed during the procedure and the arrhythmia resolved. Double conduction through the atrioventricular node often simulates other arrhy…

[Use of the electric stimulation of the heart in sick sinus syndrome (achievements and disputable problems)].

Experience with cardiac electrostimulation for the weak sinus node syndrome is presented. Between 1982 and 1985, 296 primary pacemaker implantations were performed, or 21.44% of all primary implantations performed over that period. The weak sinus node syndrome was diagnosed on the basis of ECG, Holter's monitoring and the atrial stimulation test. Initial atrioventricular conductivity disorders were recorded in 22.1%. Atrioventricular conductivity deteriorated in 16.1% of patients after 2 to 8 years of follow-up. The right ventricle was the principal stimulus application site in 267 patients, r

Influence of temperature on the atrioventricular conduction time for spontaneous and imposed rhythms.

In the present work data are obtained about the changes in the atrioventricular conduction time under the effect of two parameters: temperature and frequency of the sinus node. The experiments are carried out on preparations from the right atrium of rabbit heart. Temperature changes are obtained by regulation of the temperature of the nutrient medium in the 23--36 degrees C range. The excitation frequency of the sinus node was changed by imposition of artificial (external) rhythms. Data are obtained about the presence of frequency- and temperature-dependent components of the atrioventricular (
