A Liquid Chromatography-Tandem Mass Spectrometry Method for the Quantification of Cystic Fibrosis Drugs (Caftors) in Plasma and Its Application for Therapeutic Monitoring.

Cystic fibrosis (CF) is a life-threatening disorder caused by mutations in the CFTR gene, leading to defective chloride ion transport and thickened mucus in the respiratory and gastrointestinal systems. CFTR modulators, including ivacaftor, lumacaftor, tezacaftor, and elexacaftor, have improved patient outcomes, but interindividual pharmacokinetic variability and potential drug-drug interactions require therapeutic drug monitoring (TDM) for optimal efficacy and safety. In this context, a liquid chromatography-tandem mass spectrometry (LC-MS/MS) method has been developed and validated for the s

N1303K (p.Asn1303Lys) variant: Expanding frontiers in the treatment of cystic fibrosis.

Cystic Fibrosis (CF) is an autosomal recessive genetic disease caused by variants in both copies of the CF transmembrane conductance regulator (CFTR) gene, with F508del (p.Phe508del) being the most common variant. While CFTR modulators have revolutionized treatment, their efficacy is mutation-specific, leaving patients with rare variants like N1303K without approved therapeutic options. We present the case of a 21-year-old female with severe CF due to homozygosity for the N1303K variant. Faced with rapid clinical decline and ineligibility for approved therapies, a forskolin-induced swelling as

In silico, in vitro and ex vivo characterization of cystic fibrosis transmembrane conductance regulator pathogenic variants localized in the fourth intracellular loop and their rescue by modulators.

BACKGROUND AND PURPOSE: Cystic fibrosis (CF) is due to loss-of-function variants of the CF transmembrane conductance regulator (CFTR) channel. The most effective treatment for people with CF carrying the F508del mutation is the triple combination of elexacaftor-tezacaftor-ivacaftor (ETI). ETI can correct the underlying defect(s) in other CFTR mutants. The use of disease-relevant predictive models such as patient-derived human nasal epithelial cells allow to investigate the response to CFTR modulators of specific genotypes, possibly supporting patients' access to treatment. EXPERIMENTAL APPROA

Varietal-dependent flavor dynamics in Piper nigrum L. under thermal processing: Unraveling key aroma compounds and processing adaptability mechanisms.

This study comprehensively evaluated the changes in flavor and quality of four pepper varieties (White Dapo, Black Dapo, White Kampot, Black Kampot) under roasting and boiling conditions. The analyses encompassed color difference, piperine content, glutamic acid levels, volatile compounds, sensory traits, and multivariate statistical analyses for identifying key flavor compounds. The results demonstrated that both pepper variety and processing method jointly determined the final quality, with black peppers being more suitable for high-temperature roasting (140-160 °C) and better retaining pipe

LC-MS/MS method for co-estimation of doxorubicin and piperine: formulation development and pharmacokinetic studies.

INTRODUCTION: Oral metronomic chemotherapy employs a low-dose combination of chemotherapeutics administered regularly to minimize toxicity while enhancing anticancer efficacy. The clinical utility of Doxorubicin (DOX) is limited due to severe cardiotoxicity. Interestingly, Piperine (PIP) has been explored to mitigate DOX-induced toxicity while enhancing its therapeutic efficacy. Solid lipid nanoparticles (SLNs) offer an efficient drug delivery approach to improve oral bioavailability and controlled release of DOX and PIP. AREAS COVERED: LC-MS/MS method was developed and validated per US-FDA b

In vitro modulator responsiveness of 655 CFTR variants found in people with cystic fibrosis.

BACKGROUND: In 2017, the US Food and Drug Administration initiated expansion of drug labels for the treatment of cystic fibrosis (CF) to include CF transmembrane conductance regulator (CFTR) gene variants based on in vitro functional studies. This study aims to identify CFTR variants that result in increased chloride (Cl-) transport function by the CFTR protein after treatment with the CFTR modulator combination elexacaftor/tezacaftor/ivacaftor (ELX/TEZ/IVA). These data may benefit people with CF (pwCF) who are not currently eligible for modulator therapies. METHODS: Plasmid DNA encoding 655

Huntington's disease (HD) is an incorrigible neuropsychiatric disorder with reduced cognition and motor abnormalities. Piperine (PIP) is an alkaloid with antioxidant, anti-inflammatory, and neuroprotective activities; however, poor therapeutic efficacy limits its further use. The current study focuses on the enhanced therapeutic potential of PIP@CM against an experimental zebrafish model of HD. PIP@CM was fabricated using spray drying technology, followed by solid-state investigations. We performed in vitro release and in vitro antioxidant activity (DPPH assay) of PIP and PIP@CMs. In addition,

Small-molecule correctors divert CFTR-F508del from ERAD by stabilizing sequential folding states.

Over 80% of people with cystic fibrosis (CF) carry the F508del mutation in the cystic fibrosis transmembrane conductance regulator (CFTR), a chloride ion channel at the apical plasma membrane (PM) of epithelial cells. F508del impairs CFTR folding causing it to be destroyed by endoplasmic reticulum associated degradation (ERAD). Small-molecule correctors, which act as pharmacological chaperones to divert CFTR-F508del from ERAD, are the primary strategy for treating CF, yet corrector development continues with only a rudimentary understanding of how ERAD targets CFTR-F508del. We conducted genome

Treatment effects of Elexacaftor/Tezacaftor/Ivacaftor in people with CF carrying non-F508del mutations.

BACKGROUND: In vitro studies have demonstrated rescue of CFTR function with Elexacaftor/Tezacaftor/Ivacaftor (ETI) in several mutations other than F508del. However, clinical efficacy was not tested in vivo in people with CF (pwCF) carrying mutations other than F508del. We report effects of treatment with ETI in pwCF with non-F508del mutations. METHODS: We retrospectively analyzed pwCF with non-F508del mutations who received treatment with ETI. We evaluated sweat chloride, nutritional status, spirometry, antibiotic treatment, and pulmonary exacerbations (PEx), at baseline and 3-6 months after

The Rapid Reduction of Infection-Related Visits and Antibiotic Use Among People With Cystic Fibrosis After Starting Elexacaftor-Tezacaftor-Ivacaftor.

BACKGROUND: People with cystic fibrosis (CF) routinely suffer from recurrent sinopulmonary infections. Such infections require frequent courses of antimicrobials and often involve multidrug-resistant organisms. The goal of this study was to identify real-world evidence for the effectiveness of elexacaftor-tezacaftor-ivacaftor (ELX/TEZ/IVA) in decreasing infection-related visits and antimicrobial use in people with CF. METHODS: Using IBM MarketScan data, we identified 389 enrollees with CF who began taking ELX/TEZ/IVA before 1 December 2019 and were enrolled from 1 July 2019 to 14 March 2020.
