[Diagnostic significance of serum bile acids].

The concentrations of bile acids in serum determined by a relatively simple enzymatic method ranged between 1.6 and 9.2 mumol/l in 60 normal volunteers and between 2.1 and 7.7. mumol/l in 14 patients with Gilbert's syndrome. In contrast, 48 patients with various liver diseases had - with 2 exceptions - markedly elevated serum bile acids (up to 140 mumol/l). Together with BSP retention, BSP disappearance and SGOT, the bile acid level in serum represented one of the most sensitive indices for detection of liver disease. The sensitivity of this test can be further increased by bile acid determina…

Serum bile acids in liver disease.

Serum bile acids have been measured in patients with a wide variety of liver diseases using a technique which separates the major individual conjugated and free bile acids. Total serum bile acids may be elevated up to 100 times the normal concentration in patients with liver disease and this increase consists largely of conjugated bile acids. The ratio of glycine-conjugated to taurine-conjugated bile salts is low in all types of liver disease and this is found particularly in the serum of patients with obstructive jaundice. There is a decrease in the ratio of trihydroxy:dihydroxy cholanic acid…

Early Spontaneous Common Bile Duct Perforation.

Non-iatrogenic, non-traumatic common bile duct (CBD) perforation is a rare finding in the adult population and is more commonly reported in the pediatric population. Etiologies of CBD perforation in adults include traumatic injury, increased intraductal pressure due to choledocholithiasis, and biliary duct necrosis from underlying inflammation such as chronic pancreatitis. We present the case of a 59-year-old woman who initially presented with acute perforated cholecystitis with pancreatitis with intra-operative findings of a CBD perforation, which is not well-described in the literature. Mana…

Pitfalls of extrahepatic bile duct cytology.

Cytological examination of extrahepatic bile ducts represents a challenging diagnostic field, often limited by sample quality and cellularity. Despite the availability of detailed classifications and cellular morphology descriptions, distinguishing benign from malignant lesions remains difficult in clinical practice. This paper reviews the current WHO classification system for pancreatobiliary cytopathology, with a focus on diagnostic categories specific to extrahepatic bile ducts. It discusses typical cytomorphological features, differential diagnoses, and the use of ancillary techniques such…

[Complication management after bile duct surgery].

Bile leakage and postoperative bile duct strictures or anastomotic stenosis after bilioenteric anastomosis are complex surgical complications, which are associated with increased morbidity and mortality. Detailed diagnostics and sophisticated decision-making is always requiered. Complex liver surgery (redo procedures, nonanatomic resections, etc.) and surgery involving the liver hilum or exposure of the intraparenchymal Glissonean sheath are risk factors for postoperative bile leakage. Bile leakage is defined as a bilirubin concentration at least three times greater than the serum bilirubin me…

Disparities in bile duct injury care.

BACKGROUND: An international group proposed a standardized terminology to report outcomes after bile duct repair. Data on this surgical complication vary depending on the center and country where patients are treated. The aim of this work is to show disparities in the care process of bile duct injury between patients from two different income-level countries, using a standard terminology of outcomes and clinical reporting. METHODS: A retrospective review comparing primary repair and re-repaired cases performed in an upper middle-income country (UMIC) versus primary repair cases treated in a h…
