The pathogenesis of acute biliary pancreatitis: a controversial issue. Part I. The concept of biliopancreatic reflux.

The pathophysiologic mechanism by which gallstone migration through the sphincter of Oddi may produce acute pancreatitis is a controversial issue. According to one concept, biliopancreatic reflux following concrement obstruction of the common biliopancreatic channel results in contact of bile with pancreatic parenchyma which is known to produce acute pancreatitis. However, the entrance of bile into pancreatic tissue is usually prevented by the pancreatic duct sphincter, by the pancreatic exocrine secretion pressure and by the pancreatic duct mucosal barrier. A breakdown of these protective mec

Clinical, endoscopic, and functional studies in 408 patients with Barrett's esophagus, compared to 174 cases of intestinal metaplasia of the cardia.

OBJECTIVE: The pathophysiology of gastroesophageal reflux disease (GERD) has been studied extensively in patients with long-segment Barrett' … cantly higher lower esophageal sphincter pressures; and significantly shorter durations of acid and bile reflux. Between patients with SSBE and LSBE, significant differences were found in the frequency of hiatal hernia and duration of acid reflux (both greater in the patients with LSBE). Also, dysplasia was significantly more frequent in patients with LSBE th …

[Pancreatico-biliary maljunction and congenital dilatation of bile duct].

Congenital dilatation of bile duct (CDBD) or choledochal cyst has been demonstrated to be associated with pancreatico-biliary maljunction (PBM), and its various clinical signs and symptoms have been shown to be closely related with the presence of PBM. In almost all of the patients who were less than one year of age, the disease was of the cystic type and patients presented with either a palpable mass or jaunce as the main symptom. In patients with more than one year of age, the disease was of either the cystic or cylindrical type. A history of episodes of abdominal pain accompanied by elevate

Experimental study of the pathogenesis of choledochal cyst and pancreatitis, with special reference to the role of bile acids and pancreatic enzymes in the anomalous choledocho-pancreatico ductal junction.

To investigate the mechanisms initiating pancreatic enzyme activation followed by the development of a choledochal cyst and/or pancreatitis under anomalous choledocho-pancreatic ductal junction (ACPDJ), choledocho-pancreatic end-to-side ductal anastomosis was successfully performed in 40 puppies as an experimental model of ACPDJ. As a result, reflux of pancreatic juice into the common bile duct readily and continuously occurred, and all pancreatic enzymes in bile obtained from the common bile duct were activated. Total bile acids increased about 2 months after surgery, and the ratio of taurode

Conversion to Resectional Roux-en-Y Gastric Bypass for Refractory GERD With Predominant Non-Acid Reflux After Single-Anastomosis Duodenojejunal Bypass Diagnosed by Multichannel Intraluminal Impedance-pH Monitoring.

Metabolic and bariatric surgery has become routine clinical practice for severe obesity in Japan, and sleeve gastrectomy is the most performed procedure. However, bariatric procedures, including a sleeve component, may cause post-operative gastroesophageal reflux disease (GERD) due to disruption of the angle of His and increased intra-gastric pressure. Sleeve-plus procedures such as single-anastomosis duodenojejunal bypass with sleeve gastrectomy (SADJB-SG) can also cause non-acid reflux related to bile and pancreatic juices. Consequently, acid-suppressive therapy is often ineffective. We repo

A Non-Invasive Technique for Long-Term Monitoring of Gastroesophageal Reflux-A Pilot Study.

Many people suffer from gastric or gastroesophageal reflux disorder (GERD) due to a malfunction of the cardia, the valve between the esophagus and the stomach. GERD is a syndrome caused by the ascent of gastric juices and bile from the stomach. This article proposes a non-invasive impedance measurement method and demonstrates the correlation between GERD and impedance variation between appropriately chosen points on the patient's chest. This method is presented as an alternative to the most widely accepted diagnostic techniques for reflux, such as pH-metry, pH-impedance measurement, and esopha

Long-term functional outcomes of Roux-en-Y versus Billroth I reconstructions after laparoscopic distal gastrectomy for gastric cancer: a propensity-score matching analysis.

BACKGROUND: Only a few reports discuss long-term functional outcomes and changes observed in patients over several years following a distal … ood and remnant gastritis in the R-Y group at each annual postoperative follow-up. The incidence of bile reflux and reflux esophagitis in the B-I group gradually increased over the years and showed a significant difference at the culmination of the 5-year postoperative follow-up, compared to the R-Y group. CONCLUSIONS: Roux-en-Y reconstruction was superior t …

Effect of intraesophageal bile and acid (HCl) perfusion on the action of the lower esophageal sphincter.

We examined the effect of acid and bile on the lower esophageal high pressure zone (LEHPZ) in a combined manometric, pH testing and morphologic study in 12 cats. LEHPZ pressures and swallowing waves (SW) were simultaneously recorded before, during and after perfusion of the esophagus with control fluid, acid (0.1 n HCl) natural bile and a mixture of acid and bile. Endoscopy was performed and biopsies taken before and after the experiments. Bile caused a rise of LEHPZ and SW pressure and mild esophagitis. Acid and bile acid mixture produced a marked progressive decline of LEHPZ and SW pressure

Spontaneous choledochal cyst perforation in pregnancy with co-existent chronic pancreatitis.

Choledochal cysts are a congenital disorder of the bile duct, usually presenting in childhood. As they cause weakening of the wall of the bile duct, any distal obstruction, infection or reflux of amylase-rich fluid in the biliary tract can predispose to spontaneous perforation. A 25-year-old pregnant woman at 35 weeks of gestation presented with acute abdominal and fetal distress. On emergency laparotomy, common bile duct perforations were detected. T-tube drainage and caesarean section were performed. A t-tube cholangiogram revealed the diagnosis of a choledochal cyst with an abnormal pancrea

[Laryngopharyngeal reflux disease (LPRD) in children].

LPRD in children is characterized by symptoms, clinical findings, and sequelae caused by the reflux of gastric acid, bile acid or refluxate containing pepsin beyond the esophagus. For diagnostic procedures and therapy of gastroesophageal reflux disease (GERD) in children and adults widely accepted guidelines have been established. However, diagnosis and therapy of LPRD in children remains a continuous issue of inter- and intradisciplinary discussions. Although both GERD and LPRD in children are reflux-induced diseases, these two entities are different in symptoms, cause, diagnostic procedures,

A case of acute recurrent pancreatitis caused by biliopancreatic reflux without pancreaticobiliary maljunction.

Acute recurrent pancreatitis (ARP) is a clinical condition characterized by repeated episodes of acute pancreatitis. In this case study, a 62-year-old man was diagnosed with mild pancreatitis five years ago, with alcohol intake initially considered the cause. Since then, he experienced three episodes of pancreatitis despite ceasing alcohol consumption completely. Consequently, the patient was diagnosed with ARP. Various diagnostic and imaging tests were performed to determine the etiology of his condition, including blood tests, magnetic resonance cholangiopancreatography, endoscopic ultrasono

Postoperative Pancreatitis Following Intraoperative Cholangiography: A Case Report.

Intraoperative cholangiography (IOC) is frequently performed during laparoscopic cholecystectomy to delineate biliary anatomy and detect choledocholithiasis. While generally safe, postoperative pancreatitis following IOC is an uncommon and under-recognized complication. We report the case of a 41-year-old male patient who presented with postprandial pain on a background of symptomatic gallstones. Preoperative blood tests, including liver function tests, were unremarkable, and ultrasound demonstrated gallstones with a non-dilated common bile duct (CBD). He underwent an uncomplicated laparoscopi

Pathophysiology and management of gastroesophageal reflux disease.

Gastroesophageal reflux is a physiological phenomenon but becomes pathological if troublesome symptoms and/or complications occur. Gastroesophageal reflux disease (GERD) has different phenotypes ranging from non-erosive reflux disease (NERD), through reflux esophagitis and Barrett's esophagus, and can present with either typical symptoms such as regurgitation and heartburn, or extra-esophageal symptoms such as cough and laryngitis. In the diagnosis of GERD endoscopy, empirical PPI test, and pH impedance testing all have their own position. Although proton pump inhibitors (PPIs) are very effect

Sump Syndrome: A Rare Complication of Choledochojejunostomy.

Before the period of endoscopic retrograde cholangiopancreatography (ERCP), individuals with biliary tract diseases would undergo side-to-side choledochoduodenostomy, and sump syndrome used to develop as a complication of this procedure. There is retention of bile along with debris or calculi, and refluxed duodenal contents in the common bile duct, which leads to biliary and pancreatic complications. This syndrome's pathophysiology often results when the distal common bile duct below the anastomosis becomes a blind pouch (sump), leading to stasis of bile, food debris, and bacteria, which can l

Progressive biliary pathology associated with common pancreato-biliary channel.

A female infant who presented with transient obstructive jaundice and who was shown to have mild fusiform dilatation of the common bile duct at the age of 18 months was followed up with hepatobiliary ultrasound scans over a period of 17 years. Enlarging gallbladder polyps were identified during the last 2 years of follow-up, and endoscopic retrograde cholangio-pancreatography (ERCP) showed a common pancreato-biliary channel with minimal bile duct dilatation. A high concentration of pancreatic amylase was detected in the bile. Hepaticojejunostomy and cholecystectomy were performed. Histological

Clinical evaluation of a low junction of the cystic duct.

The point of the junction of the cystic duct with the common hepatic duct was studied by means of various preoperative and intraoperative cholangiographic procedures and by gross intraoperative examinations in 468 surgical patients with biliary diseases. The cystic duct entered the hepatic duct at a very low position and was consequently long in 39 patients. The clinical significance of this abnormally low junction of the cystic duct was studied in comparison with 358 patients with gallstones with a normal cystic duct-hepatic duct junction. In the low-junction group with a short common bile du

Experimental analysis of the ill effect of anomalous pancreaticobiliary ductal union.

An ideal canine model of anomalous pancreaticobiliary ductal union, similar to a human anomaly, was devised. Direct anastomosis of the dorsal pancreatic duct and the choledochus was employed (dorsal pancreaticocholedochostomy). The ventral pancreatic duct was not manipulated. Cylindrical choledochal dilatation results in 13 out of 20 adult dogs and in 4 out of 6 puppies. Dilatation of the intrahepatic biliary tree and thickening of the choledochal wall in a hepatic direction were observed in high ratio among adult dogs. Puppies had less dilatation. The activation of pancreatic enzymes was stud
