Cystic dilation of the biliary tract in 38 Vietnamese children is reported. Most had symptoms and signs of cholangitis and biliary obstruction. The surgical management is discussed, and for choledochal cyst resection of the cyst with hepaticojejunostomy Roux-en-Y is recommended. Postoperative cholangitis occurred in 16% of the children. Previous studies of the descriptive epidemiology of biliary tract cancers have not differentiated among different types of biliary tract cancer because until recently the International Classification of Diseases (ICD) did not classify them separately. Both percutaneous and endoscopic cholangiography identify strictures, stones, and other causes of biliary obstruction. With a radiologic diagnosis, benign and malignant causes of biliary obstruction can often be differentiated, but cholangiography does not provide a pathologic diagnosis. Analysis of the surgical treatment of 212 children is given. The patients were divided into 3 groups: 158 children with atresia of bile ducts, 29 children with abnormality of the cervix-duct segment of the gall bladder and 25 children with cystic transformation of the choledochus. On the basis of newer literature and of own experiences the author describes the present state of dietetics in diseases of the extrahepatic biliary ducts. The details of the basic diet in these diseases are discussed (nutrition of patients with limited fat and raw fibre supply). In acute cases the parenteral nutrition is to be recommended. Repeated interventions in carcinoma of the ducts were carried out upon 14 patients. The operations happened to be very beneficial for the most of patients and raised the survival period by 3 month to 2 years 4 months. The use of ultrasound in the non-invasive investigation of the biliary tract and pancreas is discussed. Its accuracy in detecting gallstones in the non-acute condition is as accurate as conventional radiography, which is dependent on the excretion of contrast medium by the liver.
