Biofeedback therapy for fecal incontinence after surgery for anorectal malformations: preliminary results.

Eight patients with fecal incontinence after surgery for anorectal malformations received one to four sessions of biofeedback therapy. The physiologic status of the anorectum before and after biofeedback therapy was investigated by anorectal manometry and electromyography of the external sphincter. The effectiveness of biofeedback treatment and the indications for its use in patients with fecal incontinence were investigated. Three of the four patients responded well to biofeedback therapy following three or four training sessions. However, in the other five patients who had only one or two se

Acoustic neuroma: postoperative vertigo and the mechanisms of compensation.

Ninety-five percent of patients with acoustic neuroma experience some degree of vertigo preoperatively, postoperatively or both. Postoperative vertigo is often a debilitating condition that results when the eighth cranial nerve is severed during surgery to resect a tumor growing around the vestibular portion of this nerve. Thus, surgery for acoustic neuroma affects the membranous labyrinth, which is the functional part of the vestibular apparatus and is primarily concerned with equilibrium. Compensation for impaired equilibrium may be by biofeedback in the vestibular pathways, visual control,

Relaxation pretraining, pulse wave velocity and thermal biofeedback in the treatment of essential hypertension.

Twelve white male subjects with a physician's diagnosis of essential hypertension participated in the study over a 3- to 4-month period (23-38 sessions). Subjects were assigned to one of four sequences of training in which the order of presentation of pulse wave velocity (PWV) or thermal feedback, and presence or absence of relaxation pretraining were varied. The overall effectiveness of the treatment program was demonstrated by average blood pressure decreases of 8.8 mm Hg/4.1 mm Hg from initial session measures to follow-up one month after completion of the program. Systolic pressures were s

Kegel's exercises with biofeedback therapy for treatment of stress incontinence.

True stress incontinence due to a weakened pelvic floor is one of the most frequently cited urologic complaints of multiparous women past age 40. One treatment modality currently used to treat stress incontinence is exercising the pubococcygeus muscle. Combining biofeedback therapy with a vaginal probe (perineometer) helps patients identify the muscle, provides immediate feedback and assists the nurse and patient in assessing problem resolution. This article describes an intervention program using biofeedback, and measured Kegel's exercises on a small number of women with symptoms of stress in

EMG biofeedback in stroke: a 1-year follow-up on the effect of patient characteristics.

Outcomes from emg biofeedback training to 28 upper and 26 lower extremities among 34 patients who were hemiplegic were reassessed at varying intervals and at 1-year after completion of feedback sessions. Examinations took into account both neuromuscular measures of muscle activity and total limb function. There was virtually no deterioration in performance among the 19 upper and 20 lower extremities in which the outcome had previously been graded as "success" or "moderate." Therefore, age, sex, affected side, duration of injury or previous rehabilitation, or the presence of proprioceptive invo

Treatment of Tietze's syndrome pain through paced respiration.

A paced respiration methodology and self-control procedure was developed to reduce respiratory irregularities associated with disabling Tietze's syndrome pain. A 46-year-old disabled male patient who was diagnosed as having Tietze's syndrome and coronary heart disease was trained in respiratory control during five training sessions. Treatment was directed at producing normal respiratory activity and reducing involuntary deep inspirations. Training led to within- and between-sessions reductions in respiratory irregularity and pain frequency, and to increases in self-reported activity levels. Tw

Electrophysiological studies in two patients with cherry red spot--myoclonus syndrome.

Two unrelated patients with the cherry red spot--myoclonus syndrome, a rare chronic neuronal storage disorder that begins in childhood with progressive myoclonus, cherry red spots at the macula, and easily controlled seizures, but no dementia, have been investigated electrophysiologically in order to characterize the myoclonic and electroencephalographic features of this syndrome. Phenomenologically, the disease most closely resembles the Ramsay Hunt syndrome, although certain unique features are noted and the patients are not photosensitive. Pathologically and clinically, the disease is relat

Biofeedback training to overcome poststroke foot-drop.

The technique has exciting potential for elderly hemiplegic patients, even those who are confined to a nursing home. There are limiting factors, of course; the method is time-consuming and the initial outlay for equipment is high. But the rewards can be well worth the time, effort, and cost involved. The recovery potential of some geriatric patients after a cerebrovascular accident may seem bleak because of multiple disabilities, e.g., paralysis, delayed reflexes, aphasia. Fortunately, these disorders do not necessarily decree failure of biofeedback training. Neither does advanced age. The str

The Physiological and Clinical-Behavioral Effects of Heart Rate Variability Biofeedback in Adolescents with Autism: A Pilot Randomized Controlled Trial.

Adolescents with autism present lower levels of cardiac vagal modulation. It was hypothesized that Heart Rate Variability Biofeedback (HRVB) increases cardiac vagal modulation in adolescents with autism, resulting in positive effects on physiological and psychosocial parameters. It was also hypothesized that home-based HRVB training is feasible. In a single-blind, randomized sham-controlled pilot trial, adolescents with autism performed supervised HRVB (n = 24) or sham training (n = 20). Subsequently, half of the adolescents received HRVB training at home, whereas the other subset did not prac

Emotional health in children and adolescents with cystic fibrosis.

Although modern therapeutic procedures have considerably improved the survival and the quality of life of children with cystic fibrosis, the relevant psychological aspects have been still insufficiently considered similarly to the other chronic diseases. The aim of this research was to evaluate the emotional health: psychological characteristics and adjustment of CF children and their family coping. The study comprises 25 CF children, mean age 13.13 ± 2.29 years (23 boys and only 2 girls), selected from total 60 actually treated children for CF. Children were examined in the period of improved

Tension-type headache: psychosomatic clinical assessment and treatment.

Tension-type headache (TTH) is an ill-defined nosographic entity. The classification of headaches according to the Headache Classification Committee of the International Headache Society is closer to clinical reality with respect to the past classification. From an aetiopathological standpoint, hypotheses trying to explain primary headache disorders in a unified way are interesting. The treatment of this disorder requires a thorough diagnostic framing, which should take into account its psychosomatic aspects, focusing on the individual patient and his history. In fact, the many studies which a

Phantom limb pain. Mechanism-based management.

Clinically significant phantom limb pain occurs among a large minority of amputees. Most suffer at least occasional episodes. It is becoming increasingly apparent that although psychological factors are not important in the cause of phantom pain, a variety of physiologic problems related to the residual limb are critical to it. Referred pain, initiated by decreased blood flow and microspasms in the residual limb along with problems caused by prosthetics account for most of the phantom pain problems seen clinically. Some pain is referred to the phantom pain from other sites in the body such as

Differential efficacy of pelvic floor muscle training in primary versus acquired premature ejaculation: an 8-week comparative study using non-invasive biomechanical assessment.

BACKGROUND: Premature ejaculation (PE), including lifelong (LPE) and acquired (APE) subtypes, negatively affects men's sexual, psychological, and relational well-being. AIM: This study aimed to investigate pelvic floor muscle function differences between healthy individuals and premature ejaculation patients and evaluate the efficacy of pelvic floor muscle training in improving premature ejaculation subtype symptoms using an innovative assessment approach. METHODS: In this 8-week prospective study, 199 men were enrolled (LPE, n = 66; APE, n = 83; healthy controls [HC], n = 50). PE participan

Psychophysiological and Dual-Task Effects of Biofeedback and Neurofeedback Interventions in Airforce Pilots: A Pilot Study.

(1) Background: Neurofeedback (NFB) and biofeedback (BFB) have been shown to reduce stress, enhance physiological self-regulation, improve cognitive performance, and accelerate response times. Stimulating the sensorimotor rhythm (12-15 Hz) is particularly effective in improving working memory and selective attention. However, most studies on air force pilots focus on addressing post-traumatic stress disorder rather than investigating how these interventions might enhance performance and safety during flights, as explored in the present study. (2) Methods: Twelve Spanish Air Force fighter pilot

The Experience of People With Urinary Incontinence Using Invasive Devices in Pelvic Floor Muscle Training: A Qualitative Study.

BACKGROUND AND PURPOSE: Pelvic floor muscle training (PFMT) is a first-line conservative treatment for urinary incontinence (UI), often involving invasive devices such as vaginal or anal biofeedback. However, these devices can cause discomfort, negatively impacting PFMT engagement. We aimed to explore the experiences of individuals with UI regarding the use of invasive devices during PFMT. METHODS: A qualitative study was conducted using semi-structured interviews. Participants were recruited through purposive sampling and interviews were conducted online. Data were analysed using Reflexive T

Stress Control for Military, Law Enforcement, and First Responders: A Systematic Review.

Military personnel, police officers, firefighters, and other first responders must prepare for and respond to life-threatening crises on a daily basis. This lifestyle places stress on personnel, and particularly so on military personnel who may be isolated from support systems and other resources. The authors conducted a systematic review of studies of interventions designed to prevent, identify, and manage acute occupational stress among military, law enforcement, and first responders. The body of evidence consisted of 38 controlled trials, 35 cohort comparisons, and 42 case studies with no c

Diagnostic and Management Strategies for Patients with Chronic Prostatitis and Chronic Pelvic Pain Syndrome.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common and often heterogenous condition that can have severe consequences on patient quality of life. In this review, we describe the pathophysiology, diagnostic work-up, and treatment of patients with CP/CPPS incorporating the most recent literature. Studies have demonstrated that CP/CPPS involves a complex pathophysiology, including infectious, immunologic, neurologic, endocrinologic, and psychologic etiologies, with frequent intersections between the different entities. Despite robust research assessing a variety of therapeutic

The emotional ear in stress.

Stress of some kind is encountered everyday and release of stress hormones is essential for adaptation to change. Stress can be physical (pain, noise exposure, etc.), psychological (apprehension to impending events, acoustic conditioning, etc.) or due to homeostatic disturbance (hunger, blood pressure, inner ear pressure, etc.). Persistent elevated levels of stress hormones can lead to disease states. The aim of the present review is to bring together data describing morphological or functional evidence for hormones of stress within the inner ear. The present review describes possible multiple
