Family structure, neonatal infection, and hay fever in adolescence.

OBJECTIVE: To determine whether increased numbers of siblings and infection in early life protect against allergic sensitisation. DESIGN: H … ias and reflects an environmental influence on allergic sensitisation. The effects of sibship size, birth order, and infant feeding are consistent with a protective influence of postnatal infection. The first month of life and the first postnatal exposure to allergen are not the critical periods during which this protective effect is determined.

Behavioral states of premature infants: implications for neural and behavioral development.

Nine premature and 28 full-term infants were observed in their homes for 7 hr when they were 2, 3, 4, and 5 weeks post-term, and the sleeping and waking states displayed by these infants were compared. For these comparisons, the observation day was divided into two mutually exclusive contexts: times when the baby was alone and times when the baby was with the mother. The premature infants spent more time alone (mean of 5.4 hr a day) than the full-terms (4.6 hr). Over the total 7-hr day, the premature infants spent more time in alert, nonalert waking activity, and sleep-wake transition than the

Adult sensory capacities as a function of birth risk factors.

This study examined the relationship between birth risk factors and sensory capacity in 1245 young adults (mean age = 19.9 years). Nine birth risk factors were included (long labour, breech birth, breathing difficulty, instrument delivery, Caesarian delivery, multiple birth, premature birth, low birth weight, and high-risk birth order) and six sensory capacities were tested (Snellen visual acuity, stereopsis, color discrimination, pure-tone hearing, speech recognition, and sound localization). Mild birth stressors were strongly predictive of reduced visual acuity and stereoscopic discriminatio…

Subjectivity on Childbearing in High School Students.

PURPOSE: This study was to explore subjectivity on childbearing in high school students. METHODS: A Q-methodology which provides a method of analyzing the subjectivity of each type was used. Forty-three high school students classified 40 selected Q-statements into 9 points standard. The obtained data were analyzed by using the pc-QUANL program. RESULTS: High school students' subjectivity on childbearing were analyzed into two types: Type 1 turned out to be 'FOLS (family oriented life style)' and Type 2 'CINK (couple important no kid)'. CONCLUSION: In order to resolve such problems as low bi

Birth weight for gestational age among Flemish twin population.

OBJECTIVE: The aim of this study was to develop birth weight references for twins. Mean birth weights of individual twins are lower than those of singletons, hence singleton birth weight curves may not be suitable to assess twin birth weights. STUDY DESIGN: Twin birth weight curves were developed according to gestational age, gender, parity and mode of -conception. The curves are based on population-based data of 40,494 twins born in Flanders, Belgium between 1987 and 2007. RESULTS: A different growth potential was found comparing the birth weights of twins and singletons. Twins deviate from

Risk factors for perinatal and neonatal mortality in cases with congenital anomalies of the kidney and urinary tract: a nested cohort study.

BACKGROUND: We aimed to investigate predictive factors associated with perinatal and neonatal mortality in cases with congenital anomalies of the kidney and urinary tract (CAKUT). METHODS: This study included a cohort of neonates with CAKUT born at a tertiary hospital between 1996 and 2021. Controls were matched with CAKUT cases by sex, time, and place of birth at a ratio of approximately 2:1. The covariates included in the analysis were sex, gestational age, birth weight, neonatal classification, and birth order. CAKUT was categorized into four phenotypes: urinary tract dilatation, lower uri…

In-utero and early life exposures in relation to risk of breast cancer.

OBJECTIVES: In response to a hypothesis by Trichopoulos that risk of adult breast cancer is related to high estrogen exposure in utero, studies have been undertaken using proxy indicators of prenatal estrogens. The epidemiologic studies addressing these early factors will be reviewed, consistency with proposed biologic mechanisms will be addressed and recommendations for future research will be presented. METHODS: All studies identified in the literature addressing these in utero and early life factors related to adult breast cancer will be included in the review. The study results will be su

Birth injury of the cranium and central nervous system.

Birth injury of the scalp, skull and central nervous system (CNS) is a well-recognized complication of a difficult delivery. The rate of birth trauma has dropped precipitously and now accounts for less than 2% of neonatal deaths. Despite this dramatic decrease in birth-trauma mortality significant injuries still occur. A variety of risk factors clearly predispose certain infants to birth-related injury. Recent neuroradiology studies indicate that intracranial hemorrhage, even in asymptomatic infants, is not rare. Pathologists' (neuropathologists and forensic pathologists) appreciation of the s

Redesigning Clinical Decision Support for Retinopathy of Prematurity Screening After Alert Failure.

Retinopathy of prematurity (ROP) is the leading cause of preventable childhood blindness. Guidelines recommend screening for infants with gestational age at birth <31 weeks or birth weight ≤1,500 g. However, ensuring timely screening during readmissions after birth is challenging. To analyze the performance of an interruptive alert at a large academic pediatric hospital for identifying premature infants needing ROP screening upon hospital readmission and to describe how data informed the transition to a non-interruptive dashboard. The alert appeared for patients 1 to 365 days of age hospitalized

Identifying Key Predictors of Neurodevelopmental Sequelae Absence at 12 Months in Infants with Congenital CMV: Insights from a Single-Center Retrospective Study.

BACKGROUND: Congenital cytomegalovirus (cCMV) is the most frequent infective cause of morbidity and long-term sequelae in infancy. Despite great attention to this clinical condition, potential prognostic factors for neurodevelopmental sequelae in cCMV patients are poorly understood, therefore hampering optimization of clinical care and management of cCMV patients. METHODS: A retrospective single-center cohort study was conducted on 66 infants with congenital CMV infection in order to identify among clinical presentation at birth potential predictors of the absence of neurodevelopmental sequel

An analysis of birth weight by gestational age using a computerized perinatal data base, 1975-1992.

OBJECTIVE: To develop birth weight-for-gestational age nomograms based on a computerized perinatal data base collected prospectively from 1975-1992. METHODS: Using information from over 60,000 singleton deliveries (January 1975 through October 1992) at the MetroHealth Medical Center in Cleveland, Ohio, standard curves for normal birth weights were computed. Nomograms were developed for the overall population and for subgroups determined by factors known to affect fetal growth, including sex, race, smoking status, and gestational diabetes. The nomograms included the tenth, 50th, and 90th perce

[Impact of iatrogenic preterm birth on newborn morbidity].

The incidence of premature births is not decreasing in developed countries, but rather shows a tendency to increase. This rise is primarily due to an increase in the number iatrogenic late preterm births at a gestational age between 34 + 0 and 36 + 6 weeks. Late preterm births account for up to 80% of all preterm births and these newborns have higher morbidity and mortality rates and a higher rate in neurodevelopmental long-term impairment in comparison to term infants. In order to reduce iatrogenic preterm birth, it is necessary to apply standardised treatment algorithms especially in cases o
