The authors have studied the roentgenological changes of the bone substance in a defect making use of densitometric and planimetric methods of investigation in a study on 63 adult dogs (2 series of experiments) who had their defect of the upper part of the tibia replaced by a graft that was being removed in doses and a distraction regenerate that was being formed at the same time. It was established that the formation of the distraction regenerate lead to demineralization of the osseous graft which was favourable for an organotypic reconstruction of the freely replaced bone.

A patient with mandibular hypoplasia associated with Treacher Collins syndrome was treated by bilateral distraction osteogenesis. Since less than optimal length was provided by the first distraction, a second corticotomy was performed in the newly formed bone 6 months after the first distraction. Thus bone gained by distraction osteogenesis was subjected to distraction once again. New bone formation occurred after the second lengthening. This case illustrated that distraction osteogenesis may be applied to the mandible at the site of previous distraction.

Lengthening of the humerus has been considered to be particularly risky because of the danger of neurological damage. The Ilizarov technique makes it possible to avoid complications. We reviewed 160 diaphyseal and metaphyseal fractures of the femur and tibia in children treated between 1964 and 1980 at the Orthopaedic and Traumatological Clinic of the University of Rome. The initial documentation was complete in these cases and 96 presented for clinical and radiographic long-term follow-up. The Ilizarov method is an original technique of surgical lengthening of long bones. The constraints in compression and distraction obtained with this method favor the bony healing. The straining of extension induces stimulating consequences on the bone. The healing process is different from that of the fractures because the bony regeneration is accomplished by a membranous bony mechanism without intermediate fibrous or cartilaginous medium. The maturation occurs when the lamellated bone is present, at the end of the 4th month.
