Biomechanical study of a modified application of Ilizarov external mini-fixator for metacarpal neck fractures: a comparative analysis.

BACKGROUND: Metacarpal neck fractures are common and there are numerous surgical methods available, but each has certain disadvantages and limitations. We modified the conventional Ilizarov external mini-fixator and this study is designed to compare the biomechanical stability of a modified Ilizarov external mini-fixator with conventional fixation methods for metacarpal neck fractures and to provide a basis for its clinical application. METHODS: Forty fresh porcine metacarpal specimens were used to create metacarpal neck fracture models. The specimens were randomly assigned to four fixation g

A novel technique for treating simple transverse patellar fractures using cannulated screws: a cadaveric and clinical study.

BACKGROUND: Tension band wiring (TBW) has conventionally been used for the open reduction and internal fixation of the patella. However, it suffers from distinct disadvantages such as large incision, implant irritation, and need for subsequent implant removal. Here, we propose a novel technique using closed reduction and percutaneous fixation with three cannulated screws (TCS), which may be an alternative to this established conventional technique. Although some researchers have proposed alternative methods including closed reduction and cannulated screw fixation, with or without additional wi

Accuracy of fluoroscopic examination in the treatment of Bennett's fracture.

BACKGROUND: Restoration of joint congruity is an important factor for the prevention of subsequent arthritis in patients with Bennett's fracture. Surgical treatment of Bennett's fracture is thus generally recommended for displaced intra-articular fractures to the proximal aspect of the thumb metacarpal. Fluoroscopic examination is used to evaluate the adequacy of closed reduction after pinning of Bennett's fracture. The purpose of this study was to determine the accuracy of fluoroscopy to determine the reduction of Bennett's fractures. METHODS: A model was created, to mimic a Bennett's fractu

[Current treatment standard for patella fractures in Germany].

BACKGROUND: The treatment of patella fractures is technically demanding. Although the radiological results are mostly satisfactory, this often does not correspond to the subjective assessment of the patients. The classical treatment with tension band wiring with K‑wires has several complications. Fixed-angle plate osteosynthesis seems to be biomechanically advantageous. OBJECTIVE: Who is treating patella fractures in Germany? What is the current standard of treatment? Have modern forms of osteosynthesis become established? What are the most important complications? MATERIAL AND METHODS: The

Problematic Pediatric Hand and Wrist Fractures.

Seymour fractures are open juxta-physeal fractures of the distal phalanx. A true lateral radiograph should be obtained for diagnosis, and treatment should include removal of the nail, irrigation and debridement of the fracture, and percutaneous Kirschner wire stabilization. Mallet fractures are more common in children than adults, and treatment is generally nonoperative for nondisplaced or minimally displaced fractures without volar subluxation of the distal phalanx; however, splinting compliance should be carefully assessed in younger populations. Phalangeal neck fractures have a limited pote

Intraarticular fractures of the foot. Talus and lesser tarsals.

In the absence of historical comparative data for the treatment of tarsal fractures, commonly abided maxims of trauma care are noteworthy. A displaced fracture involving an articular surface or a fracture interrupting a mechanical axis is treated by open reduction and internal fixation. Rigid fixation is followed as early as possible with active and passive mobilization. Unlike long bones, whose motion and fracture patterns (i.e., segmental, transverse, and oblique) are understood, the tarsal bones are small bones with complex shapes and restricted motion. As a result, tarsal injuries most oft

Acute and old ruptures of the extensor apparatus of the knee in adults (excluding knee replacement).

Rupture of the extensor apparatus of the knee in adults is infrequent and dominated by patellar fracture, which in our experience is six times as frequent as quadriceps or patellar tendon tear. Patellar fracture poses few diagnostic problems and treatment is now well codified. Tension-band osteosynthesis is generally used, involving two longitudinal K-wires and wire in a figure-of-eight pattern looped over the anterior patella; sometimes, for more complex fractures, cerclage wiring is added to the tension band. Non-union is rare and generally well tolerated. Quadriceps tendon tear mainly affec

Infectious complications of K-wire fixation in pediatric fractures: Risk factors and management at a tertiary care center.

INTRODUCTION: Kirschner wires (K-wires) are frequently used for pediatric fracture fixation. Infectious complications range from 1 % to 20 %, but the few available studies mostly focus on the adult population. The severity of infection varies from minor pin tract infections (PTIs) to severe osteomyelitis. Current literature on this topic is limited, especially regarding the pediatric population. METHODS: In this retrospective observational study, we enrolled children (0-18 years old) with traumatic bone fractures treated with smooth (unthreaded) K-wires at the Orthopedics Unit of Meyer Childr

Periprosthetic fracture fixation in osteoporotic bone.

Fixation techniques of periprosthetic fractures are far from ideal although the number of this entity is rising. The presence of an intramedullary implant generates its own fracture characteristics since stiffness is altered along the bone shaft and certain implant combinations affect load resistance of the bone. Influencing factors are cement fixation of the implant, intramedullary locking and extramedullary or intramedullary localization of the implant and the cortical thickness of the surrounding bone. Cerclage wires are ideally suited to fix radially displaced fragments around an intramedu

Displacement of the common peroneal nerve associated with upper tibial fracture: implications for fine wire fixation.

The constant anatomic position of the common peroneal nerve is relied on when performing fine wire external fixation in the upper tibia. We report the case of a sixty-two-year-old woman with a Schatzker Type V fracture of her right tibial plateau and upper-third diaphyseal fracture associated with displacement and shortening of the upper tibia. She was treated by minimal internal fixation of the intraarticular fracture and application of a Sheffield Hybrid External Fixator. During percutaneous insertion of the reference wire in the fibular head, a distal muscle twitch alerted the surgeon, and

[Osteosynthesis using the "palm-tree" nail technic in fractures of the surgical neck of the humerus].

Presented in 1974, this technique consists of internal fixation of the humeral neck by means of three divergent K-wires. These are introduced through a proximal entry point located on the lateral aspect of the humeral diaphysis at the lower end of the "V" shaped insertion of the deltoid which is one of the three subcutaneous areas of this bone. The two principles of this technique are: anchorage of the epiphysis by three divergent pins driven into the cancellous bone of the head and introduction of the pins from a very proximal point that allows easy access across the fracture without the disa

Successful outcome of distal radius non-union after open fracture osteomyelitis treated by external fixation and bone grafting in a cat: case report.

А clinical case of non-union in a cat after open fracture repair by intramedullary ostheosynthesis of the radius was described. The patient was presented with non-weight bearing lameness, fistulas with purulent discharge, swelling and severe pain. During the surgical revision, after bone sequestrum removal, the bone defect was filled with cancellous and cortical bone autografts. Osteosynthesis with a modified external bone fixator, made of Duracryl® Plus - a rapidly self-curing metacrylate polymer - and 6 Kirschner wires passing perpendicularly through both radial cortices was performed. The p

Single-handed volar locking plate fixation for distal radius fractures.

INTRODUCTION: Dorsally displaced distal radius fractures are among the most common orthopedic injuries, requiring surgical techniques that ensure optimal functional recovery while minimizing complications. The traditional method of volar locking plate osteosynthesis, although considered the gold standard, often requires a multidisciplinary surgical team. In the context of staff shortages and the need to optimize resources, this work presents a technique that can be performed by a single surgeon, particularly suited for young practitioners. The objective of this study is to demonstrate that dis

Combined Distal Interphalangeal Joint Arthrodesis With Proximal Interphalangeal Joint Arthroplasty or Arthrodesis: Technical Considerations.

Combined distal interphalangeal joint (DIP) arthrodesis with proximal interphalangeal joint (PIP) arthroplasty or arthrodesis presents unique challenges. Although less common than isolated surgery for the DIP and PIP joints, with an aging population, combined DIP and PIP procedures are an increasingly encountered occurrence. Anatomical and morphological studies have provided length and width measurement standards for the middle and distal phalanges, allowing for planning to assess the compatibility of strategies. Besides reviewing anatomical studies to provide length and width guidelines for h
