A 6-year-old boy presented for surgery for phimosis. The anaesthetic technique included intravenous induction with thiopentone and neuromuscular blockade with cisatracurium. Severe persistent bronchospasm and central cyanosis followed the administration of these drugs. The relationship between allergens in a domestic environment and asthma has been extensively studied and it is only recently that studies have suggested the possibility of the role of chemical pollutants in the internal environment in the genesis of asthma. The pollutants studied are oxides of nitrogen (nitrogen dioxide NO2), volatile organic components (COV), formaldehyde, ozone (O3) and sulphur dioxide (SO2). The level of nitrogen dioxide in the interior of houses may be greater than those met outside. Normal values are 400 mcg per metre3 per hour and 150 mcg per metre3 in twenty four hours.

Fifteen children presented with persistent cough as the sole complaint. Physical examination was entirely within normal limits. Personal and family histories of atopy and blood eosinophilia suggested an allergic diathesis. Chest radiographs showed hyperinflation in many and spirometry showed bronchospasm in all the children studied. The added observation that theophylline and salbutamol were effective suggests that the condition is a manifestation of asthma.

We diagnosed a presumably IgE-mediated allergic hypersensitivity reaction comprising generalized erythema and bronchospasm 4 h after the first-time application of transdermal fentanyl.
