Imaging techniques in the management of anorexia and bulimia nervosa.

This paper discusses the contribution offered by radiological techniques to the diagnosis of the medical and surgical complications of anorexia nervosa (AN) and bulimia (BN) with the aim of providing general indications as to their use and suggesting the best-suited techniques. In the broad field of the complications of malnourishment, the use of magnetic resonance (MR) instead of computed tomography (CT) in the assessment of brain atrophy provides much more information at a better cost-benefit ratio. Like brain atrophy, other complications may be chance radiographic findings, such as catharti…

Effect of inositol on bulimia nervosa and binge eating.

OBJECTIVES: This study aimed to determine whether inositol has therapeutic value in patients with bulimia nervosa and binge eating. METHOD: A double-blind crossover trial using 18 g inositol versus placebo was performed in 12 patients for 6 weeks in each arm. RESULTS: Inositol was significantly better than placebo on the Global Clinical Impression, the Visual Analogue Scale, and the Eating Disorders Inventory. DISCUSSION: Inositol is as therapeutic in patients with bulimia nervosa and binge eating as it is in patients with depression and panic and obsessive-compulsive disorders. This increa…

Self-destructiveness and serotonin function in bulimia nervosa.

Studies have linked bulimia nervosa (BN) to alterations in brain serotonin (5-hydroxytryptamine: 5-HT) activity and to heightened propensity for parasuicidality and self-injuriousness. The coincidence of self-destructiveness and 5-HT abnormality in BN is of interest, given documentation (in various populations) of an inverse association between 5-HT activity and potential for self-harm. The present study examined the connection between 5-HT status and self-destructiveness in BN. Structured interviews and self-report questionnaires were used to assess 40 bulimic and 21 normal-eater women for: (…

Negative feelings and the desire to eat in bulimia nervosa.

This study examines a broad range of negative feelings as possible antecedents of binge eating in bulimia nervosa (BN). Another goal is to explore the connection between negative feelings and the desire to eat as recorded continuously during two consecutive days. This is the first study comparing data from BN patients with a relevant clinical control group. Forty female BN patients, 40 female panic disorder (PD) patients, and 40 healthy women continuously recorded their feelings and the desire to eat while in their natural environment. Both patient groups reported more negative feelings than t…

Predictive validity of bulimia nervosa as a diagnostic category.

OBJECTIVE: The authors sought to investigate the predictive validity of bulimia nervosa as a diagnostic category. METHOD: More than 10 years after they appeared as patients with bulimia nervosa, 177 women (participation rate=79.7%) completed follow-up assessments. RESULTS: Among the women with a current eating pathology, most engaged in recurrent binge eating and purging. Anorexia nervosa and binge eating disorder were relatively uncommon. Eating disorder outcome was significantly related to the presence of mood, substance use, and impulse control disorders but not to the presence of anxiety…

Life events and prodromal symptoms in bulimia nervosa.

BACKGROUND: Little is known about the interaction of life events with prodromal symptoms in bulimia nervosa. METHODS: A semistructured research interview based on Paykel's Interview for Recent Life Events and on the Clinical Interview for Depression for eliciting prodromal symptoms was administered to 30 patients with bulimia nervosa and 30 healthy control subjects matched for sociodemographic variables. RESULTS: Patients reported significantly more stressful life events than controls. Most of the patients reported prodromal symptoms. Anorexia, low self-esteem, depressed mood, anhedonia, gen…

A study of temperament and personality in anorexia and bulimia nervosa.

Although temperament and personality traits could influence the development and course of eating disorders, only a few studies examined the similarities and differences in personality between anorexia and bulimia nervosa. We compared 72 patients with DSM-IV eating disorders and 30 healthy controls. Dimensions of personality and personality disorders were evaluated with the Eysenck's EPQ, Cloninger's TCI, and the SCID-II questionnaires. The rates of impulsivity and clinical features were evaluated using specific rating scales. A comorbid personality disorder was found in 61.8% of patients with …

Prior treatment received by patients with bulimia nervosa.

OBJECTIVE: This study examined the frequency with which subjects with bulimia nervosa (BN) presenting for treatment or follow-up studies had previously received either adequate pharmacologic treatment or cognitive-behavioral therapy (CBT). METHOD: Semistructured telephone screening of individuals who called in response to advertisements about research studies was used to establish diagnoses and to clarify prior pharmacologic and psychotherapeutic treatments for BN. RESULTS: Of 581 women responding, 353 (60.8%) had probable BN. Overall, 61.4% of these subjects had received psychotherapy (96.7…

Cognitive-behavioral therapy for bulimia nervosa: an illustration.

Cognitive-behavioral therapy for bulimia nervosa (BN) is a well-developed, theoretically grounded treatment for BN with the strongest empirical support for its efficacy of any form of treatment for BN. The treatment package comprises three distinct phases typically delivered over 20 weeks. Incorporating a variety of specific interventions, these three phases of treatment focus systematically on (i) dietary restraint, (ii) dysfunctional beliefs about body weight and shape, and (iii) reactions to recurrence of symptoms, which are thought to be the primary operative mechanisms that cause and main…

[Anorexia and bulimia--limits and options in clinical practice].

Even though there is no convincing evidence that eating disorders in general are on the rise, there has been an enormous increase in "treated" cases of patients with eating disorders. General practitioners detect only about 12% of all bulimia nervosa cases, and about 45% of all anorexia nervosa cases. As they often serve as gatekeepers to specialized care, they should receive better training in the recognition of eating disorders. The paper covers important aspects which are essential for the diagnosis and treatment of subjects with eating disorders: early detection, medical examination, motiv…

Serum electrolytes as markers of vomiting in bulimia nervosa.

OBJECTIVE: Patients with bulimia nervosa often have serum electrolyte abnormalities that result from vomiting and/or laxative or diuretic use. Thus, serum electrolytes could serve as an objective marker of such behaviors. METHOD: This study is a retrospective examination of serum electrolyte levels and vomiting behavior in 138 bulimic women, and of serum electrolyte levels in 70 control women. RESULTS: Logistic regression analyses demonstrated that only potassium values distinguished bulimic subjects from controls. Potassium levels below the lower limit of normal (3.7 mg/l) occurred only in …

Endogenous codeine and morphine in anorexia and bulimia nervosa.

The endogenous plasma alkaloids codeine and morphine were shown to be elevated in patients with anorexia nervosa and bulimia nervosa compared to control subjects. The role of these opioids in the pathophysiology of these eating disorders is discussed in relation to an auto-addiction opioid model. This model proposes that endogenous opioids are released during an initial period of dieting and reinforce a state of starvation dependence [1,2].

Physiological effects of bulimia nervosa on the gastrointestinal tract.

Bulimia nervosa is an eating disorder characterized by frequent bouts of binge eating accompanied by compensatory behaviour for preventing weight gain (purging). It is estimated that 3% to 5% of young women are affected by bulimia nervosa, and its prevalence is increasing. Bulimia nervosa afflicts both sexes and all races. It can lead to serious medical complications. The expression of the disease in the gastrointestinal tract may have a critical role in the diagnosis of bulimia nervosa. Physiological effects of bulimia nervosa on the gastrointestinal tract include dental caries and enamel ero…

[Obsessive-phobic disorders in anorexia and bulimia nervosa].

The data on 1500 patients with syndrome of anorexia and bulimia nervosa were summarised in terms of clinical-psychopathological, catamnestic and experimental-psychological investigation. This syndrome was considered as one disease which presented the variation of borderline mental pathology or the manifestation of schizophrenic process. Obsessive-phobic disorders were observed in majority of patients (95%). Such disorders were characterized by prevalence of obsessive phenomena with food content. It was noted that diagnosis and prognostic significance had either obsessive-phobic disturbances or…

Restraint, dieting, and the continuum model of bulimia nervosa.

The authors evaluated the continuity model of bulimia nervosa, which suggests that bulimia results from extreme weight concern and dieting practices. Individuals with bulimia, current dieters, restrained nondieters, and unrestrained nondieters were compared on measures of general psychopathology, eating-disorder-specific psychopathology, and overeating. Multiple methods, including questionnaires, clinical interviews, and food records, were used to collect data. The continuity and discontinuity models were tested with trend and regression analyses. The results of most analyses were consistent w…
