The diagnosis and management of inhalation injury: An evidence based approach.

INTRODUCTION: Smoke inhalation injury (II) is an independent risk factor for mortality in burns and its management is inherently complex. We aim to make recommendations for best practice in managing II and its sequelae by reviewing all available current evidence in order to provide an evidence-based approach. METHODS: We conducted a systematic search of the Cochrane database and Embase using PRISMA guidelines with no patient population exclusion criteria. Published work was reviewed and evidence levels graded. RESULTS: We identified 521 abstracts for inclusion. Of the 84 articles identified

Association of Acute Kidney Injury with Ammonia Poisoning: A Case Report.

Ammonia may cause poisoning due to inhalation or ingestion. Renal involvement in ammonia poisoning has been reported only once. A 30-year-old male working in an ice factory was accidentally exposed to liquid ammonia from a leaking hose, following which he had burns over his face and neck and severe abdominal pain. On day 2, he had deranged renal function, which was progressive. He was referred to us due to persistent renal dysfunction. A kidney biopsy was performed due to slow recovery of renal failure, which was suggestive of acute tubular necrosis. He was managed conservatively and showed gr

Carbon monoxide poisoning and pulmonary injury from the mixture of formic and sulfuric acids.

CONTEXT: The inhalation of carbon monoxide produced by the incomplete combustion of carbon remains a popular method of suicide. A much less common method of producing carbon monoxide for suicide is by mixing formic and sulfuric acids. CASE DETAILS: We describe a patient who attempted suicide by mixing formic and sulfuric acids. He presented with a depressed level of consciousness, chemical burns of his airway and skin, and respiratory distress. He was found to have a metabolic acidosis, a carboxyhemoglobin of 36.8%, hyperkalemia, and rhabdomyolysis. His hospital course was notable for copious

A multicenter study analyzing the association of vitamin D deficiency and replacement with infectious outcomes in patients with burn injuries.

Vitamin D (25OHD) deficiency is associated with poor outcomes in intensive care populations. The primary objective of this 7-center study was to determine if 25OHD deficiency is associated with infectious outcomes in adult burn patients. Generalized linear mixed modeling was used to control for center effect, percent total body surface area burn (% TBSA), age, and presence of inhalation injury. A total of 1147 patients were initially included (admitted January 2016 through August 2019). After exclusions, 234 (56.8%) in the deficient (25OHD<20 ng/mL) and 178 in the non-deficient group (25OHD ≥

Myocardial effects of isoflurane in healthy infants and small children. Assessment by continuous oesophageal aortic blood flow echo-Doppler.

BACKGROUND: In paediatric healthy patients and in real peroperative conditions, the cardiovascular effects of isoflurane have been poorly described. METHODS: We have evaluated the myocardial effects of 1% end-expired concentration (EEC) of isoflurane in 25 healthy infants or small children undergoing superficial surgical therapy for small burns with a continuous aortic blood flow echo-Doppler device. Aortic blood flow (ABF) was measured with a small oesophageal probe specially designed for infants. The aortic flowmeter was connected with satellite devices to visualize the haemodynamic profile

Factors Associated With Early Versus Late Mortality Following Pediatric Burn Injury in a Resource-Limited Setting.

INTRODUCTION: Although burn injury survival has improved significantly in high-income countries (HICs) over the past 3 decades, low- and middle-income countries (LMICs) continue to experience high mortality rates. Prior studies have attributed burn mortality to factors, such as total body surface area burned, inhalation injury, and under-resuscitation, primarily focusing on late deaths in HICs. This study examines the characteristics of early versus late burn deaths in LMICs. METHODS: We conducted a retrospective analysis of pediatric patients admitted to the Kamuzu Central Hospital (KCH) bur

Examining 1:1 vs. 4:1 Packed Red Blood Cell to Fresh Frozen Plasma Ratio Transfusion During Pediatric Burn Excision.

Blood transfusions following major burn injury are common due to operative losses, blood sampling, and burn physiology. While massive transfusion improves outcomes in adult trauma patients, literature examining its effect in critically ill children is limited. The study purpose was to prospectively compare outcomes of major pediatric burns receiving a 1:1 vs. 4:1 packed red blood cell to fresh frozen plasma transfusion strategy during massive burn excision. Children with >20% total body surface area burns were randomized to a 1:1 or 4:1 packed red blood cell/fresh frozen plasma transfusion rat

A pilot survey of Southeastern firefighters: Safety practices, use of protective gear, and injury.

INTRO: There were over 60 firefighter line of duty deaths and roughly 30,000 injuries in the United States in 2016. Modern thermoprotective gear has reduced the risks firefighters face from both thermal and inhalation injury, but must be used properly to be effective. The purpose of this study is to examine gear use and associated injury in firefighters. METHODS: Surveys were distributed with questions about demographics, gear usage, and maintenance practices. If previously injured, firefighters described the injury, treatment, and recovery. RESULTS: Of the 50 surveys distributed, 37 were re

Septicaemia after burn injury: a comparative study.

Seventy-nine (8.4%) patients during June 1992-May 1996 (Group-1) and 68 (7.2%) patients from June 1996 to May 2000 (Group-2) who developed septicaemia at the burns unit of Al-Babtain Centre for Plastic Surgery and Burns, Kuwait, were retrospectively studied and compared. The mean age of 26 years, male predominance, flame burns as main aetiology and mean burn percentage of >or=40% was observed in both the groups. Both groups revealed extensive flame burn, inhalation injury, intubation and difficult resuscitation as the risk factors. The proportion of satisfactory resuscitation increased signifi

Corneal burns with eyelid sparing and their treatment.

In a flash injury the eyes are protected by the blink reflex. With our patients the eyes were open looking for a means of escape, exposing the globe to injury by hot smoke and protecting the eyelids. It is essential to examine the eyes in any facial burn, especially if there is any history of being injured in an enclosed space. If there are any signs of corneal injuries then the assistance of an ophthalmologist should be sought.

[Pulmonary mucormycosis in a patient with diabetes mellitus: favorable evolution under medical treatment].

Pulmonary mucormycosis is a relatively uncommon but an important opportunistic fungal infection in immunocompromised patients. It may be acquired by inhalation of spores into the lungs. The pathogenic hallmark of mucormycosis is vascular invasion resulting in thrombosis and tissue infarction/necrosis. The diagnosis can be difficult since presentation does not differ from other forms of pneumonia. Risk factors include neutropenia, haematologic malignancies, uncontrolled diabetes mellitus, skin burns and deferoxamine therapy in dialysis patients. Early recognition, control of underlying diseases

Smoke inhalation injury from newer synthetic building materials--a patient who survived 205 days.

A 25-year-old factory worker sustained inhalation injury and 2 per cent deep burns while fighting a fire in his factory (LSI factory) which was made of new synthetic building materials. He became unconscious and inhaled noxious substances from the smoke. Although he survived the acute stages following injury, his pulmonary function subsequently deteriorated with a damaged trachea and bronchi, repeated attacks of pneumonia, atelectasis, pneumothorax and lung fibrosis, leading to death with hypercapnia (PaCO2 more than 100 mmHg) after 205 days in hospital. Autopsy revealed scarring contraction a

Pneumonia Risk in Urgently Intubated Burn Patients.

BACKGROUND: Pneumonia is a major cause of morbidity and mortality in burn patients with inhalation injuries. An increased risk of pneumonia has been demonstrated in trauma and burn patients urgently intubated in the field vs. emergency departments (EDs). OBJECTIVES: To compare intubation setting (field vs. ED) and subsequent development of pneumonia in burn patients and to evaluate the indication for urgent intubation outside the hospital setting. METHODS: A retrospective medical records review was conducted on all intubated patients presenting with thermal (study group, 118 patients) or tra

[Etiology of combined inhalational hydrocyanic acid and carbon monoxide poisoning].

A young man was found dead in a kitchen, that was partly burnt. Autopsy revealed, as cause of death, a combined intoxication following inhalation of carbon monoxide and hydrocyanic acid. Own investigations on the pyrolysis of pieces of furniture found in the kitchen (plastic plates containing melamine and plates containing formaldehyde) showed, that hydrocyanic acid was liberated through combustion of such substances and inhaled by the victim. The poisoning picture is discussed, and discussion includes especially considerations on the peculiar sensitivity of the brain toward the action of hydr
