We describe a patient who had lipothymoma with red cell aplasia, hypogammaglobulinemia, and lichen planus. Parathymic syndromes described in association with lipothymomas also include myasthenia gravis, hyperthyroidism, lymphangioma, aplastic anemia, chronic lymphocytic leukemia, and Hodgkin's disease. The behavior of lipothymoma is generally benign, although local recurrence was noted in 1 patient who had an incomplete resection. An intriguing relationship between idiopathic mitral valve prolapse (MVP) and cardiomyopathy has been reported in adult cases of MVP. This paper reports a girl with idiopathic MVP, who presented with progressive cardiomegaly and cardiomyopathic findings or, cardiac biopsy; right ventricular endomyocardial biopsy at the age of 15 years showed myocardial hypertrophy, disarray, myocardial degeneration, interstitial fibrosis and endocardial thickening. To our knowledge, this is the first case of idiopathic MVP in childhood that has been shown to have positive biopsy findings for cardiomyopathy.

A 6-month-old girl died suddenly without any previous symptoms of heart failure. Autopsy examination showed cardiomegaly (97 g) with a severely fibrotized myocardium. The left coronary artery was originating from the pulmonary artery. Histologically, the myocardium showed myocardial infarcts of different ages, as well as grossly thickened arterial branches due to increased flow in left-right shunt. We suggest that rare anomalies of the coronary arteries should be considered in the autopsies of suspected sudden infant death syndrome cases.

Four young horses of various breeds and suffering from atrial fibrillation died of heart failure. All had markedly high pulmonary arterial pressure, right-sided cardiomegaly, and lack of histologic lesions in the right atrium or pulmonary parenchyma. Three horses had hypertrophy and/or necrosis of the tunica media of the pulmonary vasculature. Clinical signs of disease, physiologic data, and pathologic findings indicated that these horses had primary pulmonary hypertension with secondary right-sided cardiac ventricular hypertrophy and dilatation, atrial fibrillation, and heart failure.

A 24-year-old female patient was admitted to clinic with a 10-year history of dyspnoea. A chest radiograph showed mild cardiomegaly and echocardiography revealed classic findings of Fallot's tetralogy (TOF). Multidetector-computed tomography (MDCT) angiography was performed to evaluate the additional vascular anomalies. MDCT undoubtedly revealed left pulmonary arterial atresia as well as complex intracardiac and vascular anatomic features of TOF. A case is presented of a 47-year-old man who died as a result of sevoflurane abuse. Sevoflurane was identified and confirmed by headspace gas chromatography-mass spectrometry. The heart blood sevoflurane concentration was 16 mg/L, and the peripheral blood sevoflurane concentration was 8.0 mg/L. No drugs or other volatile substances were found in the heart blood. The medical examiner ruled that the cause of death was cardiac arrhythmia due to sevoflurane toxicity. Cardiomegaly was listed on Part II of the death certificate. The manner of death was undetermined.

Heart failure in the last month of pregnancy or with in 5 months after delivery has long been recognized as peripartum cardiomyopathy. Additional criteria include the absence of other identifiable causes of heart failure and the absence of other prior heart disease. Estimate of the incidence vary from 1 in 1300 to 4000 pregnancies. The mortality rates of this disorder in the acute phases and subacute phases range from 25 to 50 percent the prognosis is especially poor in patients with cardiomegaly persisting > 6 months and in patients with low left ventricular ejection fraction.

A P wave of 7.5 mm in lead I and 12.5 in V1 was detected in a 28-year-old man, with a progressive cardiomegaly since the age of 14 years. At last admission he had minor symptoms, and a systolic murmur consistent with tricuspid regurgitation. The electrocardiogram showed an extremely tall P wave and a QRS of a very low amplitude; T waves were inverted on the precordial leads. These ECG features, and subsequent investigations, were consistent with right ventricular cardiomyopathy with massive tricuspid regurgitation, and right atrial abnormality.

We have described a case of coronary thrombosis in a neonate. The diagnosis should be considered in a term infant who presents with a shock-like state with poor perfusion soon after birth. The chest roentgenogram may vary from a normal pattern to generalized cardiomegaly with pulmonary congestion. The electrocardiogram may show a pattern varying from severe right ventricular hypertrophy to anterior left myocardial injury. Prognosis is usually poor, and the infant may die within 24 hours after delivery. The etiology of the thrombus formation remains obscure.
