BACKGROUND: Anterior inferior cerebellar artery (AICA) aneurysms are rare with no optimal consensus on management strategies. When treatment requires parent artery occlusion, postoperative ischemic or neuropathic complications can occur. This study evaluated the feasibility of direct revascularization of premeatal AICA using the middle meningeal artery (MMA) as the donor through an anterior petrosectomy window.
METHODS: Ten specimens were used for surgical simulation of MMA-AICA bypass. MMA was harvested from the dura down to the foramen spinosum. An anterior petrosectomy approach was perform

OBJECT: The aim of this study was to determine the risk factors associated with the development of postoperative ischemic complications after surgical revascularization used to treat pediatric patients with ischemic moyamoya disease.
METHODS: The clinical, imaging, and perioperative data from 170 procedures in 90 children who underwent indirect revascularization surgery were retrospectively reviewed. To clarify the risk factors, cases with identified ischemic complications and those without such events were compared. For this study, a postoperative ischemic complication was defined as a newly

Most recent studies on the effectiveness of cerebral revascularization have focused on the treatment of atherosclerotic internal carotid artery occlusive disease. The goal of the present study was to assess neurological function in 11 severe atherosclerotic middle cerebral artery (MCA) disease patients with transient ischemic attacks (TIAs) and hemodynamic compromise and determine the efficacy of superficial temporal artery-middle cerebral artery (STA-MCA) bypass. There were eight patients with MCA occlusion and three with severe MCA stenosis. After the bypass procedure, all 11 patients experi…

This study reports cerebral angiographic findings observed after stent retriever thrombectomy that is suggestive of a higher risk of hemorrhagic transformation (HT). A woman aged 65-75 presented with a right middle cerebral artery syndrome confirmed by non-contrast CT brain scan. Endovascular revascularization using the Solitaire device was placed across the thromboembolic occlusion and thromboembolectomy was performed. Angiography showed complete recanalization of the left internal carotid artery, anterior and middle cerebral artery branches. Twelve hours following the procedure the patient h

OBJECT: Moyamoya is a rare, chronic disease that leads to the progressive narrowing and/or occlusion of the distal internal carotid and proximal cerebral arteries. Chronic cerebral ischemia ensues due to insufficient collateral blood supply. One potential treatment consists of the restoration of regional cerebral blood flow by direct or indirect revascularization surgery. The extent of neovascularization, especially in indirect procedures such as encephalomyosynangiosis (EMS), is currently evaluated with conventional angiography. Because this method is invasive and carries some risks, the auth

Moyamoya disease is a rare progressive cerebral vasculopathy characterized by narrowing or occlusion of the internal carotid arteries, leading to the formation of fragile collateral vessels and a high risk of ischemic events, especially in pediatric patients. Although direct revascularization techniques are commonly used, they are technically challenging in children due to vascular fragility and anatomy. Indirect revascularization techniques favor the formation of new blood vessels through alternative flow routes. Their efficacy is usually assessed using the Matsushima scale, which measures th

BACKGROUND: High-flow bypasses with parent vessel occlusion are required for complex aneurysms originating from the ICA or MCA. Traditional graft options are the saphenous vein however, it has a higher rate of thrombus formation than an arterial graft. The radial artery is commonly used, however, tapers in its distal course and is contraindicated in some patients while the lateral circumflex femoral artery is an excellent graft vessel and easy to harvest.
METHOD: We present our technique and experience harvesting the lateral circumflex femoral artery as an alternative graft vessel for high-fl

An unexpected rupture at the aneurysm neck, with or without adjacent arterial injury or compromise of distal branches during microsurgical clipping, can be a challenging surgical problem to resolve. In this presented case of a neurologically intact 65-year-old woman, elective clipping of an unruptured right middle cerebral artery bifurcation aneurysm was complicated by an unexpected M2 tear at the neck, involving the origin of the frontal M2. Attempts to seal the tear directly, using various techniques, failed; therefore, it was ultimately managed with sacrifice of the vessel and a salvage sid

OBJECTIVE: Complex anterior cerebral artery (ACA) aneurysms are still technically challenging to treat. Bypass surgery is needed to achieve aneurysm obliteration and ACA territory revascularization. Severe atherosclerosis of aneurysm walls can cause clip slippage, intraoperative rupture, postoperative ischemic events. How to assess the atherosclerotic changes in vascular walls by high-resolution vessel wall magnitude resonance imaging (VWI) is the key question in complex ACA aneurysm surgical management.
METHODS: This retrospective single-center study included eight patients diagnosed with co

There have been a number of anastomosis methods of bypass techniques reported for moyamoya disease. However, there are yet no randomized controlled trials conducted on the anastomosis method. Retrograde blood flow of the superficial temporal artery (STA) may be used as one of the donor options. Here, we examined the tolerability of retrograde bypass using a distal stump of the parietal STA (dsPSTA). Anastomosis between the dsPSTA and middle cerebral artery (MCA) was performed for consecutive patients with moyamoya disease whose parietal STA was visualized to be longer than 10 cm using contrast

BACKGROUND: Moyamoya disease (MMD) is considered a progressive disease with an ongoing risk of recurrent stroke. However, there is a lack of long-term observational data to quantify the extent of the stroke risk.
METHODS: This study aimed to provide insight into the long-term stroke risk in MMD and explore possible risk factors for stroke. Records from all patients diagnosed with MMD in 13 clinical departments from 6 different Danish hospitals between 1994 and 2017 were retrospectively reviewed until 2021.
RESULTS: The cohort comprised 50 patients (33 females and 17 males). Patients were fol

BACKGROUND: Middle cerebral artery (MCA) M1-related dissecting aneurysms involving the M1 segment are difficult because of the involvement of M1 perforators and the short duration of ischemia tolerance during bypass.
METHOD: We report a case of MCA M1-2 dissecting aneurysm resection and reanastomosis under bypass blood flow protection. Histological staining was used to show aneurysm pathological characteristics.
CONCLUSION: This case demonstrates the value of distal bypass for blood flow protection in MCA M1-2 dissecting aneurysm dissection and reanastomosis. Pathological staining showed int

OBJECTIVE: To present the technique of extra-intracranial bypass surgery using the orifice of maxillary artery bypass, to evaluate the advantages and disadvantages of this and alternative revascularization options.
MATERIAL AND METHODS: Radial artery graft harvesting was performed at the 1st stage. Simultaneously, the second team of surgeons performed a combined (submandibular and anterior) access to the donor artery (mandibular segment of maxillary artery behind the ramus of the mandible). Craniotomy and mobilization of potential recipient arteries (M2-M3 segments of the middle cerebral arte

Superficial temporal artery-middle cerebral artery(STA-MCA)bypass surgery is necessary for vascular reconstruction in patients with hemodynamic ischemia, including moyamoya disease and complex aneurysms. This report introduces the fundamental techniques of STA-MCA bypass surgery practiced in my institute. Preoperative and perioperative management is important to minimize complications. During the procedure, the operator should keep a bloodless surgical field to facilitate the bypass. The vascular lumen and intima must be carefully observed during anastomosis, especially in moyamoya cases. In a
